Uterine contraction frequency in the last hour of labor: how many contractions are too many?

OBJECTIVE: Increased frequency of uterine contractions is a component in the cluster of causal conditions that can lead to fetal hypoxia and acidosis and increase the risk for neonatal neurologic injury. For most international obstetrical societies, 5 contractions per 10 min averaged over 30 min is considered as the upper limit of normal uterine activity. We hypothesize that it might be safer to adopt an upper limit of 4 contractions per 10 min. METHODS: We reviewed our 1970's research database containing 475 patients with closely monitored and well-documented labor and neonatal assessments t

Simulating uterine contraction by using an electro-chemo-mechanical model.

Contractions of uterine smooth muscle cells consist of a chain of physiological processes. These contractions provide the required force to expel the fetus from the uterus. The inclusion of these physiological processes is, therefore, imperative when studying uterine contractions. In this study, an electro-chemo-mechanical model to replicate the excitation, activation, and contraction of uterine smooth muscle cells is developed. The presented modeling strategy enables efficient integration of knowledge about physiological processes at the cellular level to the organ level. The model is impleme

TXA2 mediates LPA1-stimulated uterine contraction in late pregnant mouse.

Lysophosphatidic acid (LPA) is known to increase uterine contraction in the estrus cycle and early pregnancy, however, the effect of LPA in late pregnant uterus and its mechanisms are not clear. In the present study, we show the LPA receptor subtypes expressed and the mechanism of LPA-induced contractions in late pregnant mouse uterus. We determined the relative mRNA expression of LPA receptor genes by quantitative PCR and elicited log concentration-response curves to oleoyl-L-α-LPA by performing tension experiments in the presence and absence of nonselective and selective receptor antagonists

[Diagnosis of fetal heart rate without referring to uterine contraction].

The purpose of this study is to evaluate intrapartum fetal condition only with reference to fetal heart rate (FHR) change. Sixty-five cases who had no severe complications were examined at 36 weeks to 42 weeks of gestation. Major cardiotocographic abnormalities were baseline bradycardia and severe variable deceleration. Late deceleration was not observed. The conventional program of automatic FHR diagnosis and a new computer program excluding uterine contraction (UC) algorithm were used. Digitalized FHR and UC data stored in a floppy disk were reproduced and analysed by either of the programs

Accuracy of electromyometrial imaging of uterine contractions in clinical environment.

Clinically, uterine contractions are monitored with tocodynamometers or intrauterine pressure catheters. In the research setting, electromyography (EMG), which detects electrical activity of the uterus from a few electrodes on the abdomen, is feasible, can provide more accurate data than these other methods, and may be useful for predicting preterm birth. However, EMG lacks sufficient spatial resolution and coverage to reveal where uterine contractions originate, how they propagate, and whether preterm contractions differ between women who do and do not progress to preterm delivery. To address

Mechanotransduction mechanisms for coordinating uterine contractions in human labor.

This review presents evolving concepts of how the human uterus contracts in pregnancy, with emphasis on the mechanisms of long-distance signaling. Action potential propagation has historically been assumed to be the sole mechanism for signaling and tissue recruitment over both short and long distances. However, data in animals and humans indicate that a single action potential does not travel distances greater than a few centimeters. To address this enigma, a long-distance signaling mechanism based on hydraulic signaling and mechanotransduction is developed. By combining this mechanism for lon

Role of uterine contraction in regeneration of the murine postpartum endometrium.

The endometrium undergoes continuous repair and regeneration without scarring throughout the reproductive life of women. However, the mechanisms responsible for this complete restoration remain mostly unexplored. We hypothesized that the ischemic state and local hypoxia present after parturition may create a special microenvironment for endometrial healing, and that this ischemia might be caused by reduction in organ volume via postpartum uterine contraction. Here, we developed a mouse model using a combination of cesarean section and the administration of a beta 2 adrenergic receptor agonist

Frequency of uterine contractions and the risk of spontaneous preterm delivery.

BACKGROUND: The measurement of the frequency of uterine contractions has not been useful for reducing the rate of preterm delivery in randomized trials. Nonetheless, ambulatory monitoring of contractions continues to be used in clinical practice. METHODS: We assessed the frequency of uterine contractions as a predictor of the risk of spontaneous preterm delivery before 35 weeks of gestation. We enrolled women with singleton pregnancies between 22 and 24 weeks of gestation. The women used a contraction monitor at home to record contraction frequency twice daily on 2 or more days per week from

Imaging the dynamics of murine uterine contractions in early pregnancy†.

Uterine muscle contractility is essential for reproductive processes including sperm and embryo transport, and during the uterine cycle to remove menstrual effluent. Even still, uterine contractions have primarily been studied in the context of preterm labor. This is partly due to a lack of methods for studying the uterine muscle contractility in the intact organ. Here, we describe an imaging-based method to evaluate mouse uterine contractility of both the longitudinal and circular muscles in the cycling stages and in early pregnancy. By transforming the image-based data into three-dimensional

A myofibre model for the study of uterine excitation-contraction dynamics.

As the uterus remodels in preparation for delivery, the excitability and contractility of the uterine smooth muscle layer, the myometrium, increase drastically. But when remodelling proceeds abnormally it can contribute to preterm birth, slow progress of labour, and failure to initiate labour. Remodelling increases intercellular coupling and cellular excitability, which are the main targets of pharmaceutical treatments for uterine contraction disorders. However, the way in which electrical propagation and force development depend on intercellular coupling and cellular excitability is not fully

[Influence of naloxone on uterine contractions in patients with primary dysmenorrhea].

UNLABELLED: Dysmenorrhea is a common condition among women in childbearing age. An increased uterine contractions, resulting among others from increased vasopressin and oxitocin secretion, is considered as a main cause of the primary dysmenorrhea. The endogenous opioids play the important role in the control of oxytocin and vasopressin release from the pituitary gland. Naloxone is a selective opioid receptor antagonis. So far, there is not much data on naloxone effect on uterine contractions. The aim of study was to determine the influence of naloxone on uterine contractions in patients with p

Malignant hypercalcemia in pregnancy: effect of pamidronate on uterine contractions.

BACKGROUND: Hypercalcemia in pregnancy can have devastating effects on both mother and fetus. Pamidronate, a bisphosphonate and inhibitor of bone resorption, has been used to treat hypercalcemia, but it is considered contraindicated in pregnancy. CASE: A 40-year-old multigravida presented at 24 weeks of gestation with metastatic breast cancer. After unsuccessful chemotherapy, at 28 weeks of gestation she had deteriorating renal function, frequent contractions, and a calcium level of 17.6 mg/dL. When conventional treatment of hypercalcemia failed, pamidronate dramatically decreased both the ca

Synchronization analysis of the uterine magnetic activity during contractions.

BACKGROUND: Our objective was to quantify and compare the extent of synchronization of the spatial-temporal myometrial activity over the human uterus before and during a contraction using transabdominal magnetomyographic (MMG) recordings. Synchronization can be an important indicator for the quantification of uterine contractions. METHODS: The spatialtermporal myometrial activity recordings were performed using a 151-channel noninvasive magnetic sensor system called SARA. This device covers the entire pregnant abdomen and records the magnetic field corresponding to the electrical activity gen

[Inhibition of spontaneous uterine contractions by magnesium. Magnesium tocolysis].

A state of magnesium deficiency has been established in the population of highly industrialized countries in the last few years. This results among other disturbancies, in increasing prematurity. Therefore the tocolytic activity of magnesium was investigated in 29 uterine strips, which were obtained during cesarean section. A highly significant (p less than 0.001) reduction of the uterine activity from 100 to 0 (0-21), 0 (0-0) and 0 (0-0) (median, 20th and 80th percentile) 10, 20 and 60 min after the application of magnesium was observed.

[Tolerance of the fetus with congenital anemia to induced uterine contractions].

A study of the fetal cardiac rate was made during uterine contractility induced by Pose's test in a group of 22 isoimmunized women carrying fetuses with variable degrees of hemolytic anemia. A high percentage of "positive tests" and other alterations in the fetal cardiac rate recordings were found. The risks of delivery and the need to examine the clinical value of different anomalies in the fetal cardiac rate are noted.

Hilbert-Huang transform in detecting and analyzing the uterine contraction activities.

OBJECTIVE: The diagnosis of labor is currently one of the most difficult problems encountered by obstetrical healthcare providers. A major health problem is the increase in the rate of preterm delivery, which is responsible for 75% of all deaths in newborns. In addition, preterm delivery is associated with several cognitive and health problems in later life and enormous costs for the health system. A better understanding of myometrial activities could help to reduce preterm deliveries and the costs associated with prematurity in the following years. Therefore, the objective of this study was t
