The role of chorioretinal biopsy in the management of posterior uveitis.

BACKGROUND: The ideal management of intraocular inflammation may require a tissue diagnosis. Diagnostic vitrectomy is a well-established method for acquiring such tissue. However, in some patients, the diagnostic pathology is limited to the choroid and retina and vitrectomy may yield no useful information. In such cases, a more aggressive surgical approach to obtain tissue may be useful. METHODS: The authors performed chorioretinal biopsies on seven patients with progressive chorioretinal lesions of unknown etiology. Indications for biopsy included: (1) macular-threatening lesions unresponsiv

Optical Coherence Tomography Findings in Infectious Posterior Uveitis.

PURPOSE: To describe and illustrate the main optical coherence tomography (OCT) findings of infectious uveitis. METHODS: Narrative review. RESULTS: Posterior segment OCT in patients with infectious uveitis reveals posterior hyaloid face precipitates, superficial retinal precipitates and infiltrates, foveolitis, retinitis, neuro-retinitis, choroidal granulomas, and choroiditis as main imaging biomarkers. Some of these features are specific to the underlying causing etiology and may support the diagnosis and the initiation of treatment. Some OCT features disappear completely with resolution; s

Optical coherence tomography diagnostic signs in posterior uveitis.

A diagnostic sign refers to a quantifiable biological parameter that is measured and evaluated as an indicator of normal biological, pathogenic, or pharmacologic responses to a therapeutic intervention. When used in translational research discussions, the term itself often alludes to a sign used to accelerate or aid in diagnosis or monitoring and provide insight into "personalized" medicine. Many new diagnostic signs are being developed that involve imaging technology. Optical coherence tomography is an imaging technique that provides in vivo quasi-histological images of the ocular tissues and

Case Report: Paraneoplastic melanoma-associated retinopathy mimicking posterior uveitis.

INTRODUCTION: Melanoma-associated retinopathy (MAR) is a rare paraneoplastic ocular manifestation of cutaneous or mucocutaneous melanoma. We describe a novel case of MAR that mimicked an inflammatory or infiltrative retinopathy. PATIENT PRESENTATION: A 68-year-old female developed bilateral, painless, floaters over six months with accompanying photopsia. Bilateral posterior uveitis was noted, and bilateral vitrectomies were non-diagnostic for malignancy. A systemic evaluation revealed a subcutaneous lesion on the medial aspect of her right thigh. Biopsy of the lesion was consistent with melan

The Great Imitator: Ocular Syphilis Presenting as Posterior Uveitis.

BACKGROUND: Syphilis is often known as the "Great Imitator". The differential diagnosis of posterior uveitis is broad with ocular syphilis being particularly challenging to diagnose as it presents similarly to other ocular conditions such as acute retinal necrosis. CASE REPORT: A 34-year-old woman with multiple sexual partners over the past few years presented with painful and progressively worsening unilateral vision loss for 2 weeks. Several months prior, she had reported non-specific symptoms of headache and diffuse skin rash. Despite treatment with oral acyclovir for 3 weeks, her vision p

[Comparative laser tyndallometry and fluorophotometry in anterior and posterior uveitis].

UNLABELLED: Recent studies have sought to quantify aqueous flare by laser flare measurement. An increase in aqueous flare caused by a rise of protein concentration was frequently found both in anterior and posterior segment disease. This has been interpreted as a break-down of the blood--aqueous barrier (BAB). By measuring the diffusion coefficient of the BAB compared to the aqueous flare value in patients with anterior and posterior uveitis, the extent to which the increase in flare value was related to a possible break down of the BAB was examined. PATIENTS: Thirty-nine normal eyes (23-78 y

Results of vitreous surgery for posterior complications of chronic uveitis.

To determine surgical results and predictive factors of final visual acuity, a total of 30 eyes in 30 uveitis patients who underwent vitreous surgery including pars plana vitrectomy were followed for at least 6 months and various preoperative factors and postoperative results were analyzed. Our surgical indications were vitreous opacity, traction retinal detachment, combined rhegmatogenous-traction detachment. Preoperatively detached retina was finally reattached in 15 (83.3%) of 18 eyes. Final visual acuity improved in 19 (63.3%) of 30 eyes, but decreased in 3 eyes compared with the initial a

[Posterior uveitis in systemic diseases. A pathologic and epidemiologic study].

In the course of 2943 autopsies, during which the eyes were also removed, investigations were carried out concerning inflammatory lesions of the posterior uveal tract. Posterior uveitis was found in 202 cases (6.9%) of subjects whose mean age was 74.5 years. In 98% of the cases this was a non-granulomatous inflammation. The frequency of the associated systemic diseases in the cases of uveitis was compared with the frequency of such diseases in an accurately chosen control group. The results of our investigation showed that diabetes mellitus and inflammatory diseases, particularly rheumatoid ar
