Validation of the Vaccination Confidence Scale: A Brief Measure to Identify Parents at Risk for Refusing Adolescent Vaccines.

OBJECTIVE: To validate a brief measure of vaccination confidence using a large, nationally representative sample of parents. METHODS: We analyzed weighted data from 9018 parents who completed the 2010 National Immunization Survey-Teen, an annual, population-based telephone survey. Parents reported on the immunization history of a 13- to 17-year-old child in their households for vaccines including tetanus, diphtheria, and acellular pertussis (Tdap), meningococcal, and human papillomavirus vaccines. For each vaccine, separate logistic regression models assessed associations between parents' mea

COVID-19 Vaccine Refusal and Medical Distrust Held by Correctional Officers.

This study explores COVID-19 vaccine acceptance among prison security staff and the extent to which they trust varied sources of information about the vaccines. Cross-sectional survey data were obtained from a state-wide sample of corrections officers (COs, hereafter; n = 1208) in February 2021. Group differences, disaggregated by demographic characteristics, were examined using F-tests and t-tests. Despite the comparatively limited risk of contracting the virus, non-security staff reported they would accept a COVID-19 vaccine at no cost (74%), compared to their more vulnerable CO counterparts

Understanding the perceived logic of care by vaccine-hesitant and vaccine-refusing parents: A qualitative study in Australia.

In terms of public health, childhood vaccination programs have benefits that far outweigh risks. However, some parents decide not to vaccinate their children. This paper explores the ways in which such parents talked about the perceived risks and benefits incurred by vaccinating (or not vaccinating) their children. Between 2013-2016 we undertook 29 in-depth interviews with non-vaccinating and/or 'vaccine hesitant' parents in Australia. Interviews were conducted in an open and non-judgmental manner, akin to empathic neutrality. Interviews focused on parents talking about the factors that shaped

Influenza vaccine refusal among healthcare workers: A cross-sectional study.

BACKGROUND: Seasonal influenza poses a persistent public health burden, and vaccination of healthcare workers is a key preventive measure. Despite availability, influenza vaccination coverage among healthcare workers remains suboptimal, often influenced by vaccine hesitancy and risk perception. This study aimed to investigate the motivations underlying influenza vaccine refusal among healthcare workers in a large tertiary university hospital in Italy during the 2025/2026 vaccination campaign. METHODS: A cross-sectional design was employed. During the 2025/2026 influenza vaccination campaign,

Understanding the reasons for refusal of polio vaccine by families in Quetta Block, Pakistan.

BACKGROUND: Global polio eradication is a goal yet to be achieved in countries like Pakistan. In recent years, the Polio Eradication Initiative has been making steady progress with good campaign coverage and low numbers of polio cases. However, in 2019 Pakistan reported 146 cases compared to 12 in 2018. A major factor cited for this regression was a surge in vaccine refusals by parents and caretakers. AIMS: To assess the reasons for the refusal of polio vaccination in Quetta Block, Balochistan. METHODS: The study was conducted using data acquired from 2 polio vaccination campaigns over 3 mon

Vaccine Hesitancy and Refusal: Behavioral Evidence from Rural Northern Nigeria.

It is widely believed that vaccine hesitancy is prevalent in African countries, although this belief is without rigorous evidence. Our field experiment in rural northern Nigeria behaviorally measured the prevalence of vaccine hesitancy-the non-uptake of vaccines despite their availability due to non-monetary factors directly associated with vaccination. We randomly assigned two tasks to women: answering a short survey at their house vs. additionally receiving a free tetanus vaccine by submitting a voucher. The differences in their completion rates captured vaccine hesitancy, showing the rate t

Developing a three-dimensional narrative to counter polio vaccine refusal in Charsadda.

BACKGROUND: Endemic polio in Pakistan is threatening the Global Polio Eradication Initiative (PEI). In recent years, vaccine refusals have surged, spiking polio cases. The current study was conducted to understand the ethnic, religious and cultural roots of vaccine refusals in Charsadda District and explore the remedial options. METHODS: We conducted 43 in-depth interviews with parents who had refused polio vaccines for their children and the PEI staff. Interviews were audio-recorded, written in verbatim and analysed with Atlis.ti. We conducted a thematic analysis of our data. RESULTS: The f

Dangers of vaccine refusal near the herd immunity threshold: a modelling study.

BACKGROUND: Childhood vaccination remains the focus of heated public debate. Parents struggle to understand the potential risks associated with vaccination but both parents and physicians assume that they understand the risks associated with infection. This study was done to characterise how modern vaccination practices have altered patient risks from infection. METHODS: In this modelling study, we use mathematical analysis to explore how modern-era vaccination practices have changed the risks of severe outcomes for some infections by changing the landscape for disease transmission. We show t

Refusal of oral polio vaccine in northwestern Pakistan: a qualitative and quantitative study.

BACKGROUND: Refusal of the oral polio vaccine (OPV) is a difficulty faced by the Polio Eradication Initiative (PEI) in multiple endemic areas, including the Khyber Pakhtunkhwa Province (KPP), Pakistan. In 2007, we investigated community perceptions of the OPV and estimated the prevalence of OPV refusal in three districts in Swat Valley, KPP, a polio-endemic area. METHODS: Qualitative data concerning community perceptions were collected by focus group discussions among lady health workers (LHWs) and mothers with children <1 year old and by key informant interviews with local health managers an

COVID-19 Vaccination Refusal and Non-Uptake Among People Living with HIV in the United States.

More than a million individuals in the United States are living with HIV and constitute a particularly vulnerable group for morbidity and mortality associated with COVID-19. Despite the early availability and prioritization of this group for COVID-19 vaccination, notable levels of vaccine refusal and non-uptake have been reported among this group. This scoping review aimed to investigate the nature and extent of COVID-19 vaccine refusal and non-uptake among people living with HIV (PLWH) in the USA. A comprehensive literature search was performed utilizing multiple scholarly databases for studi

Reasons for Childhood Vaccine Refusal: A Cross-Sectional Study in South-Eastern Türkiye.

Background/Objectives: Vaccine hesitancy has intensified worldwide, and measles and pertussis have resurged across the WHO European Region. In Türkiye, a regulation introduced in May 2024 made documentation of every parental refusal mandatory, rendering 2025 the first calendar year for which provincial refusal data can be considered systematically ascertained. Methods: We conducted a retrospective, registry-based cross-sectional study of all vaccine refusal notifications submitted to the Batman Provincial Health Directorate between 1 January and 31 December 2025, covering all six districts of

Emergence of Vaccine-Preventable Diseases: The Perfect Storm of Hesitancy, Refusal, a Pandemic, and War.

The landscape of pediatric vaccination has changed dramatically due to changing attitudes toward immunizations and recent world events. The rise of vaccine hesitancy and refusal related to the concurrent rise of social media and anti-vaccination messages with misinformation campaigns have led to populations of children being unimmunized or under-immunized. These populations have been left vulnerable to the rapid spread of vaccine-preventable infection. Additionally, the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and the clinical syndrome known as coronavirus disease 2019 (COV

The Importance of Prioritizing Pre and Posttransplant Immunizations in an Era of Vaccine Refusal and Epidemic Outbreaks.

Vaccine-preventable infections are occurring at epidemic rates both nationally and internationally. At the same time, rates of vaccine hesitancy and refusal are increasing across the country leading to decreased herd immunity. For immunosuppressed transplant recipients, this situation poses great risk. Currently, 1 in 6 pediatric solid organ transplant recipients is hospitalized with a vaccine-preventable infection in the first 5 years posttransplant. For many recipients, these infections result in significant morbidity, mortality, and increased hospitalization costs. Surprisingly, despite thi

Prevalence and determinants of parental refusal of human papillomavirus vaccination in Morocco: A multicenter cross-sectional study.

INTRODUCTION: Vaccination against the human papillomavirus (HPV) is an effective way to avert cervical cancer. However, acceptance in Morocco remains inadequate. The aim of this study was to measure the prevalence of parental HPV vaccine refusal and the risk factors associated with it. METHODS: Between 3 March and 30 September 2025, a multicenter cross-sectional survey was conducted among Moroccan parents of girls aged 11-14 who attended health facilities. The study looked at sociodemographic factors, knowledge regarding HPV and cervical cancer, and vaccination attitudes. A multivariate logis

Local infectious disease experience influences vaccine refusal rates: a natural experiment.

Vaccination has been critical to the decline in infectious disease prevalence in recent centuries. Nonetheless, vaccine refusal has increased in recent years, with complacency associated with reductions in disease prevalence highlighted as an important contributor. We exploit a natural experiment in Glasgow at the beginning of the twentieth century to investigate whether prior local experience of an infectious disease matters for vaccination decisions. Our study is based on smallpox surveillance data and administrative records of parental refusal to vaccinate their infants. We analyse variatio

Parental delay or refusal of vaccine doses, childhood vaccination coverage at 24 months of age, and the Health Belief Model.

OBJECTIVE: We evaluated the association between parents' beliefs about vaccines, their decision to delay or refuse vaccines for their children, and vaccination coverage of children at aged 24 months. METHODS: We used data from 11,206 parents of children aged 24-35 months at the time of the 2009 National Immunization Survey interview and determined their vaccination status at aged 24 months. Data included parents' reports of delay and/or refusal of vaccine doses, psychosocial factors suggested by the Health Belief Model, and provider-reported up-to-date vaccination status. RESULTS: In 2009, a

A vaccine emergency-when to overrule parental refusal of vaccination at birth for prevention of vertical transmission of hepatitis B virus?

Hepatitis B virus (HBV) is a potentially chronic infection that can be transmitted from mother to child with the risk of developing cirrhosis, liver failure and hepatocellular carcinoma. There is a safe and effective vaccine to prevent vertical transmission that is recommended to be given as soon as possible after birth and within 24 hours. When a woman with HBV refuses the birth dose of HBV vaccine for her baby, infectious diseases and safeguarding teams are asked to provide urgent opinions on whether this crosses the threshold for triggering child protection mechanisms. We consider a low-infec

Promoting COVID-19 vaccine acceptance: recommendations from the Lancet Commission on Vaccine Refusal, Acceptance, and Demand in the USA.

Since the first case of COVID-19 was identified in the USA in January, 2020, over 46 million people in the country have tested positive for SARS-CoV-2 infection. Several COVID-19 vaccines have received emergency use authorisations from the US Food and Drug Administration, with the Pfizer-BioNTech vaccine receiving full approval on Aug 23, 2021. When paired with masking, physical distancing, and ventilation, COVID-19 vaccines are the best intervention to sustainably control the pandemic. However, surveys have consistently found that a sizeable minority of US residents do not plan to get a COVID
