Valganciclovir, an oral prodrug of the anti-cytomegalovirus (CMV) agent ganciclovir, was evaluated in a single-arm open-label safety study. AIDS patients (median CD4 lymphocyte count of 140 cells/microL) with treated CMV retinitis (N = 212) received 900-mg once-daily valganciclovir maintenance therapy with courses of 900-mg twice-daily valganciclovir induction therapy as needed to treat progression. After a median treatment duration of 372 days, the adverse event profile was similar to that reported for intravenous (IV) and oral ganciclovir. Adverse event rates of note were diarrhea (35%), nau

As a result of the low oral bioavailability of ganciclovir, a prodrug was developed to improve the bioavailability of ganciclovir. This study was designed to investigate the fasting, single-dose pharmacokinetics as well as the absolute and relative bioavailability of a valine ester prodrug of ganciclovir, valganciclovir, as compared to oral and intravenous ganciclovir in asymptomatic HIV+ and CMV+ subjects. In this open-label, randomized, three-period crossover study, 18 subjects received, in random order, single oral doses of valganciclovir 360 mg and ganciclovir 1000 mg and an intravenous in

Mortality among adults hospitalized with advanced HIV disease (AHD; CD4 <200 cells μl-1 or a WHO stage 3 or 4 disease) is >20%; there is an urgent need to evaluate strategies to reduce mortality within this extremely high-risk group. Approximately 50% of adults with CD4 counts <100 cells μl-1 have detectable cytomegalovirus (CMV) viraemia, and high-level CMV viraemia is associated with greater than double the hazard of death in this population. Despite this, there are no current treatment guidelines to inform treatment of CMV viraemia without end-organ disease in the context of AHD. The NIRVAN

BACKGROUND: Valganciclovir is the only approved antiviral for cytomegalovirus (CMV) prevention in pediatric solid organ transplantation (SOT). Additional approaches may be needed to improve outcomes.
METHODS: A multicenter retrospective study from 2016 to 2019 was conducted of pediatric SOT recipients in whom at least 3 months of valganciclovir prophylaxis was planned. Episodes of CMV DNA in blood (DNAemia), CMV disease, drug-related toxicities, as well as other infections in the first year posttransplant and demographic and clinical data were collected. CMV DNAemia in the first year after pr

OBJECTIVES: Valganciclovir (VGCV) is used as prophylaxis against cytomegalovirus (CMV) infection after pediatric living donor liver transplantation (LDLT). The purpose of this study was to examine the efficacy of 1 year of preemptive VGCV administration compared with a shorter administration after pediatric LDLT.
METHODS: VGCV was administered to 56 children who underwent LDLT. CMV and Epstein-Barr virus (EBV) antibody status, pp65 antigenemia, and other laboratory data were assessed at 1 year after LDLT. Patients were divided into the 1-year group (n = 32) (patients who had 1 year of VGCV ad

Described is the case of a 21-year-old male patient who presented with a severe Epstein-Barr virus (EBV) hepatitis. The initial diagnosis was challenging, as the patient did not have the typical features of the mononucleosis syndrome and despite the severity of the hepatitis, the initial serology was negative. In addition the liver biopsy did not show the mononuclear cell infiltration typically seen in EBV hepatitis. Later, measurements of EBV DNA showed high titers and the patient received a course of oral valganciclovir, following which he made a rapid clinical and serological response. This

Cytomegalovirus (CMV) reactivation is a serious complication after allogeneic hematopoietic cell transplantation (alloHCT), with heightened risk in recipients receiving post-transplant cyclophosphamide (PTCy)-based prophylaxis. Although letermovir reduces clinically significant CMV infection (csCMVi), its prohibitive cost limits access, highlighting the need for affordable alternatives. Unlike solid organ transplantation, there are no published studies reporting the utility of valganciclovir for CMV prophylaxis following PTCy-based alloHCT. The aim of this study was to evaluate the deliverabil

Cytomegalovirus (CMV) infection in immunocompetent adults is generally asymptomatic or causes a mild, self-limited illness, but it may occasionally present as CMV mononucleosis with prolonged fever. We report a case of primary CMV infection in an immunocompetent adult presenting with prolonged fever and subsequent clinical improvement following antiviral therapy. A 26-year-old woman presented with a three-week history of intermittent fever reaching 39°C. Laboratory findings revealed atypical lymphocytes and elevated liver enzymes. Serological findings were consistent with primary CMV infection

UNLABELLED: <p>Objective: The primary goal of this study was to evaluate the pharmacokinetics (PK) of ganciclovir (GCV) and its prodrug valganciclovir (VGCV) in a large cohort of lung transplant recipients. The secondary goal was to determine the probability of target attainment (PTA) for different pharmacokinetic/pharmacodynamic (PK/PD) targets.
SUBJECTS AND METHODS: A population PK analysis was carried out using NONMEM version 7.4.0. In total, 379 GCV concentrations (194 and 185 concentrations sampled after VGCV and GCV administration, respectively) were obtained from 110 patients and were

OBJECTIVES: Valganciclovir (VGCV) has been shown to improve sensorineural hearing loss (SNHL) and neurological outcomes in patients with neonatal symptomatic congenital cytomegalovirus (cCMV) infection. However, reports on the pharmacokinetics, efficacy and safety of oral VGCV are limited. The aim of this study is to evaluate the pharmacokinetics of VGCV for use in the treatment of cCMV.
METHODS: This was a single-center, retrospective observational study conducted at Saitama Children's Medical Center in Japan between 2012 and 2017. CMV DNA copy number, maximum plasma VGCV concentration (Cmax

We conducted a clinical trial of oral valganciclovir, a drug with in vitro activity against Kaposi sarcoma (KS)-associated herpesvirus (KSHV), in classic KS. Five human immunodeficiency virus-seronegative participants received valganciclovir for up to six 4-week cycles at doses used for cytomegalovirus infection. None of the study subjects showed an objective response; KS progressed in 4 subjects after 1-4 cycles and remained stable in 1 subject after 6 cycles. KS biopsies showed minimal lytic KSHV antigen and gene expression at baseline and no treatment-associated changes. Although valgancicl

PURPOSE: Data remain limited on the most appropriate valganciclovir (VGCV) dosing strategy for cytomegalovirus (CMV) prophylaxis and treatment in pediatric organ transplant recipients. Therefore, the objective of this study was to describe methods used to calculate VGCV dosing among pediatric transplant centers.
METHODS: A survey of pharmacists was conducted to assess VGCV dosing strategies for CMV prophylaxis and treatment among pediatric organ transplant centers in the U.S.
FINDINGS: Of 54 centers that perform pediatric organ transplants, 22 (40.7%) centers responded to the survey. Fourtee

INTRODUCTION: Cytomegalovirus (CMV) infection is a significant cause of morbidity and mortality among heart transplant (HT) patients. Various prophylactic and preemptive treatment regimens have been used for its prevention. We sought to assess the impact of oral valganciclovir on CMV prophylaxis in HT patients.
PATIENTS AND METHODS: A retrospective analysis of 536 consecutive HT patients at our institution allowed selection of subjects eligible for prophylaxis based on CMV serology (donor positive/recipient negative). Treatment compliance, rates of preemptive therapy and treatment for CMV dis

Primary infection by cytomegalovirus (CMV) commonly occurs subclinically or manifested by a self-limited mononucleosis-like syndrome in immunocompetent subjects. Severe clinical pictures are uncommon. We present a case of acute myopericarditis and hepatitis in a previously healthy 32-year-old man with primary CMV infection, assessed by serology and positive pp65 antigenemia. He was successfully treated with a course of oral valganciclovir therapy, with an immediate clinical response and normalization of laboratory tests. The literature on simultaneous presentation of CMV pericarditis and hepat

Background/Objectives: Neutropenia is a common adverse effect of oral valganciclovir (VGCV) treatment in infants with congenital cytomegalovirus infection (CCMVI), with an estimated prevalence of 20%. However, its clinical course and associated factors, including the influence of VGCV dosage, remain inadequately characterized. Methods: We conducted a single-center retrospective cohort study of infants treated with VGCV for symptomatic congenital CMV infection (CCMVI) at the Kobe University Hospital between 1 April 2009 and 31 March 2017. Detailed descriptive analyses of neutropenia were perfor

BACKGROUND: Valganciclovir (2/d) therapy for 6 months in neonates with symptomatic congenital cytomegalovirus (cCMV) infection improves hearing and neurodevelopmental outcome. The only reported adverse event was neutropenia. Since 2009, our protocol for symptomatic cCMV infection was a 1-year treatment of 2/d for the first 3 months followed by 9 months of 1/d.
METHODS: A retrospective study. Infants with cCMV treated with valganciclovir for 1 year were recruited. Data of drug-related hematologic adverse events were collected.
RESULTS: One hundred sixty infants were eligible; 46 (28.8%) had e

BACKGROUND: Suicide gene therapy for malignant gliomas has shown encouraging results in the latest clinical trials. However, prodrug application was most often restricted to short-term treatment (14 days), especially when replication-defective vectors were used. We previously showed that a substantial fraction of herpes simplex virus thymidine kinase (HSV-TK) transduced tumor cells survive ganciclovir (GCV) treatment in an orthotopic glioblastoma (GBM) xenograft model. Here we analyzed whether these TK+ tumor cells are still sensitive to prodrug treatment and whether prolonged prodrug treatmen

Liver transplantation : official publication of the American Association for the Study of Liver Diseases and the International Liver Transplantation Society2018eng
Liver transplantation recipients (LTRs) who are seropositive for cytomegalovirus (CMV) (recipient seropositive [R+]) are at intermediate risk for CMV disease. A preventative strategy following transplant is considered standard of care. Current guidelines recommend high-dose valganciclovir (VGCV; 900 mg/day adjusted for renal function) for prophylaxis given limited data on the efficacy and safety of low-dose VGCV (450 mg/day adjusted for renal function). We describe our experience using low-dose VGCV prophylaxis for R+ LTRs at our institution. A single-center, retrospective study was conducted

OBJECTIVES: To determine whether valganciclovir 450 mg every 48 h for cytomegalovirus (CMV) prophylaxis provides appropriate ganciclovir exposure in solid organ transplant recipients during continuous renal replacement therapy (CRRT).
PATIENTS AND METHODS: Ganciclovir pharmacokinetics was intensively studied in two lung transplant recipients under valganciclovir 450 mg every 48 h over one dosing interval. In vitro experiments using blank whole blood spiked with ganciclovir further investigated exchanges between plasma and erythrocytes.
RESULTS: Ganciclovir disposition was characterized by ap
