Evidence-based Diagnosis and Treatment of Vasa Previa.

Vasa previa is an abnormality of the umbilical cord and fetal membranes that affects ∼1 in 1300 pregnancies. The diagnosis is made by visualization of velamentous fetal vessels coursing within the membranes over the cervix unprotected by Wharton jelly or placenta. When it is not diagnosed prenatally, it is associated with a high risk of fetal death. Prenatal diagnosis of vasa previa using ultrasound, followed by close surveillance, and appropriately timed late preterm delivery by cesarean is associated with intact survival in >95% of cases. In this review, we review epidemiology, risk factors,…

Analysis of the ultrasonographic findings predictive of vasa previa.

OBJECTIVES: To clarify the ultrasonographic findings indicative of prenatal vasa previa. METHODS: The variables associated with placental and umbilical cord abnormalities were retrospectively analysed in cases with and without vasa previa. RESULTS: Consecutive subjects were divided into those with vasa previa (10) and controls (4682). Abnormal placental forms and placenta previa/low-lying placenta were associated with vasa previa [odds ratio (OR) 21.9 and 28.0]. While the frequency of velamentous cord insertion was 1.6% in the controls, it was 90% in the cases with vasa previa (OR 552). In a…

Velamentous insertion of the umbilical cord and vasa previa.

We report 31 cases with velamentous insertion of the umbilical cord of whom two had coexistent vasa previa. The prevalence of the former was 0.22% and of the latter 0.014%. Perinatal mortality was low, 3.2%, but there was a high rate of complications during pregnancy, such as threatened abortion, antepartum and intrapartum bleeding, premature delivery and fetal distress. Fifteen patients underwent caesarean section. Four of the term infants were small for gestational age. One infant had congenital malformations. The two cases with vasa previa are described in detail. We conclude that velamento…

Monochorionic twins with ruptured vasa previa: double trouble!

Velamentous cord insertion and vasa previa occur more frequently in monochorionic twin pregnancies than in singleton pregnancies. Both have been linked with poor perinatal outcome due to the increased risk of rupture of the velamentous vessels. We present a case of acute fetal distress in 2 fetuses in a monochorionic twin pregnancy caused by ruptured vasa previa that was not detected antenatally. Both infants were severely anemic at birth. Acute blood loss in twin 1 through the ruptured vessels, led to an acute feto-fetal transfusion from the co-twin through the placental vascular anastomoses.…

Vasa previa: prenatal detection by three-dimensional ultrasonography.

OBJECTIVE: To describe three-dimensional (3D) ultrasonography (US) for the antepartum diagnosis of vasa previa. DESIGN: This was a descriptive study of two pregnant women who were suspected to have vasa previa by conventional gray-scale ultrasonography. Three-dimensional studies were also performed during the early third trimester to further investigate the possibility of this condition. RESULTS: In the first case, 3D US provided gray-scale multiplanar and surface-rendered views of an aberrant vessel over the internal cervical os. For the second case, a 'flight-path' technique allowed the ex…

Current Evidence on Vasa Previa without Velamentous Cord Insertion or Placental Morphological Anomalies (Type III Vasa Previa): Systematic Review and Meta-Analysis.

Vasa previa carries a high risk of severe fetal morbidity and mortality due to fetal hemorrhage caused by damage to unprotected fetal cord vessels upon membrane rupture. Vasa previa is generally classified into types I and II. However, some cases are difficult to classify, and some studies have proposed a type III classification. This study aimed to review the current evidence on type III vasa previa. A systematic literature search was conducted, and 11 articles (2011-2022) were included. A systematic review showed that type III vasa previa accounts for 5.7% of vasa previa cases. Thirteen wome…

Vasa previa with bipartite placenta and velamentous cord insertion.

Vasa previa is a rare but potentially life-threatening obstetric condition characterized by fetal vessels traversing the membranes over or near the internal cervical os. Without prenatal diagnosis, it carries a perinatal mortality rate exceeding 50% due to the risk of Benckiser hemorrhage upon membrane rupture. We present the case of a 24-year-old primigravida diagnosed with vasa previa associated with a bipartite placenta and velamentous cord insertion at 32 weeks of gestation. The diagnosis was initially made at the referring hospital in Bordeaux and confirmed at our institution using transv…

Study protocol for first national vasa previa perinatal registry.

Vasa previa is a rare but potentially catastrophic obstetric condition characterized by unprotected fetal blood vessels over or adjacent to the internal cervical os. Although advances in prenatal ultrasound have dramatically improved neonatal survival through planned cesarean delivery, many aspects of vasa previa remain poorly understood. Existing studies are limited by small sample sizes, single-center designs, heterogeneous diagnostic criteria, and inconsistent reporting of clinical outcomes, highlighting the need for large, systematically collected multicenter datasets. Here we describe, to…

Vasa previa: Perinatal outcomes in singleton and multiple pregnancies.

Vasa previa (VP) is a rare and life-threatening condition for the fetus. It is associated with increased perinatal mortality rates. The current study sought to retrospectively analyze the perinatal outcomes of VP in singleton and multiple pregnancies between January 1, 2013 and December 31, 2019 at a tertiary hospital in west China. One hundred and fifty-seven cases of VP were identified, including 131 singletons, 23 twins and 3 triplets. VP in 20 cases was diagnosed at delivery. There were 183 live births. Neonatal mortality was significantly higher in cases with no prenatal diagnosis (9.7% v…

Prenatal Diagnosis of Vasa Previa: Outpatient versus Inpatient Management.

OBJECTIVE: The aim of this study was to compare the pregnancy outcome of two different management strategies: outpatient versus inpatient in women with prenatal diagnosis of vasa previa. MATERIALS AND METHODS: This is a retrospective cohort study conducted at a single tertiary center. Women with a prenatally diagnosed vasa previa between January 2007 and June 2017 were included. Obstetric and neonatal outcomes were compared between two management strategies: elective admission at 34 weeks of gestation or outpatient management unless there were signs of labor or premature contractions. RESULT…

Vasa previa: clinical presentations, outcomes, and implications for management.

OBJECTIVE: To review experience with diagnosis, clinical associations, and outcomes of vasa previa in a single institution. METHODS: This was a retrospective review of all identified vasa previa cases from January 1 1990, to June 30, 2010. RESULTS: Sixty cases of vasa previa were identified (53 singletons, seven twins); 56 cases were diagnosed before delivery. An abnormal cord insertion or abnormal placental location was present in 55 cases. Missed diagnoses were attributed to technical and observer factors. Preterm bleeding was encountered in 25 (42%) case group participants. Seven case gro…

Vasa previa and postpartum hysterectomy in maternal Rh alloimmunization.

Velamentous insertion of the cord, or vasa previa, is a malady where fetal vessels tranverse membranes ahead of the fetal part. The incidence of vasa previa is 1: 2000-3000 deliveries. Fetal mortality is over 50-75%. Early diagnosis is needed because these deliveries require emergency cesarean section; it is especially more common with placenta percreta, uterine atony and hemorrhage. Intravascular infusion of red blood cells (RBCs) into the fetus is one of the most successful means of in utero therapy for severe fetal anemia caused by RBC alloimmunization. We performed four fetal intrauterine …

Vasa previa: prenatal diagnosis, natural evolution, and clinical outcome.

OBJECTIVE: To describe the prenatal ultrasonographic diagnosis, natural evolution, and clinical outcomes of vasa previa in a large population at a single institution. METHODS: We attempted to view the internal cervical os of 93,874 women with second- and third-trimester pregnancies during an 8-year period. Echogenic parallel or circular lines near the cervix, seen by gray-scale ultrasonography, raised the possibility of vasa previa. Diagnosis was confirmed by Doppler and endovaginal studies if aberrant vessels over the internal cervical os were suspected. Abnormal placental morphology and vel…

Antepartum testing for vasa previa: current standard of care.

Vasa previa is a rare accident in obstetrics with high fetal mortality. There are simple tests available to distinguish fetal from maternal blood. Recently it has been called into medicolegal question whether patients with third-trimester bleeding need to be tested for fetal bleeding. We surveyed 100 community hospitals and all the medical schools in the United States in an attempt to answer the question. It appears that the standard of care in the United States does not require such testing.

In utero laser treatment of type II vasa previa.

Vasa previa, defined as fetal vessels coursing within the membranes between the presenting part and the cervix, occurs in approximately 1:2500-5000 pregnancies. Type II vasa previa consists of fetal vessels crossing over the internal os connecting a bilobed placenta or a succenturiate lobe with the main placental mass. These vessels are prone to compression during labor or may tear when membranes rupture potentially resulting in fetal exsanguination and neonatal death. This complication could be avoided altogether if the vessels could be obliterated in utero. The purpose of this communication …
