Cardiac implantable electronic device and vascular access: Strategies to overcome problems.

For arrhythmia treatment or sudden cardiac death prevention in hemodialysis patients, there is a frequent need for placement of a cardiac implantable electronic device (pacemaker, implantable cardioverter defibrillator, or cardiac resynchronization device). Leads from a cardiac implantable electronic device can cause central vein stenosis and carry the risk of tricuspid regurgitation or contribute to infective endocarditis. In patients with end-stage kidney disease requiring vascular access and cardiac implantable electronic device, the best strategy is to create an arteriovenous fistula on th

Evidence-based practice in the management of vascular access devices for home parenteral nutrition therapy.

Catheter-related bloodstream infection and catheter occlusion are potential significant complications of parenteral nutrition therapy. The increased incidence and associated morbidity, mortality, increased costs, and quality-of-life issues experienced with these adverse events necessitate specialized management of vascular access devices. The host coagulation response to biomaterials and the associated development of biofilm on vascular devices are complex phenomena. Multiple interventions are required to prevent access of bacteria to both intraluminal and extraluminal catheter surfaces, and t

Position Statement on Vascular Access Safety for Percutaneous Devices in AMI Complicated by Cardiogenic Shock.

In the United States, the frequency of using percutaneous mechanical circulatory support devices for acute myocardial infarction complicated by cardiogenic shock is increasing. These devices require large-bore vascular access to provide left, right, or biventricular cardiac support, frequently under urgent/emergent circumstances. Significant technical and logistical variability exists in device insertion, care, and removal in the cardiac catheterization laboratory and in the cardiac intensive care unit. This variability in practice may contribute to adverse outcomes observed in centers that re

Comparing the Push-Pull Versus Discard Blood Sample Method From Adult Central Vascular Access Devices.

This study demonstrates the feasibility of replacing the discard blood sampling method for central vascular access devices with the push-pull method. A comparative, within-subject design was used to evaluate 61 unique, paired blood samples from 1 adult outpatient oncology clinic. A 21-measure laboratory panel was conducted on each of the paired samples. Interpretation showed a small mean bias and excellent agreement between the methods. Blood samples obtained using the push-pull method were within clinically acceptable ranges. No hemolysis was noted by laboratory evaluation of 59 samples.

Hemodialysis vascular access flow measurements by AFM® and Transonic® devices: A bicentric comparative study.

OBJECTIVE: Clinical monitoring remains central to vascular access care, while surveillance of vascular access blood flow (Qa) provides information for detecting dysfunction. Ultrasound dilution (UD), such as Transonic®, is widely used for Qa measurement but may be limited by workflow, personnel, and time constraints. Alternatively, conductivity-based approaches integrated into dialysis machines enable automated intradialytic estimation of Qa without interrupting treatment. This bicentric study evaluated agreement between an automated ionic dialysance-based method, AFM®, and UD, and assessed it

Vascular Closure Devices After Femoral Access in Adult EP Procedures: Updated Systematic Review and Meta-Analysis.

BACKGROUND: Femoral vascular access is integral to diagnostic and therapeutic electrophysiology (EP) procedures but is associated with bleeding and access-site complications. Manual compression and figure-of-eight sutures remain widely used for hemostasis; however, they are resource intensive and delay ambulation. Vascular closure devices (VCDs) have been introduced to facilitate rapid hemostasis and improve procedural efficiency, yet evidence specific to adult EP procedures remain conflicting. OBJECTIVES: To systematically evaluate the safety and efficacy of vascular closure devices compared

Epidemiology and Clinical Insights of Catheter-Related Candidemia in Non-ICU Patients with Vascular Access Devices.

INTRODUCTION: Vascular access devices (VADs), namely peripheral VADs (PVADs) and central venous VADs (CVADs), are crucial in both intensive care unit (ICU) and non-ICU settings. However, VAD placement carries risks, notably catheter-related bloodstream infections (CRBSIs). Candida spp. is a common pathogen in CRBSIs, yet its clinical and microbiological characteristics, especially in non-ICU settings, are underexplored. METHODS: We conducted a monocentric, retrospective observational study at Luigi Sacco Hospital from 1 May 2021 to 1 September 2023. We reviewed medical records of non-ICU adul
