Early vascular grafting to prevent upper extremity necrosis after electrical burns.

Presented in this paper are the surgical considerations in the management of 8 electrically injured patients, with special reference to the care for wrist damage associated with impaired blood flow, and the prospect for effective recovery. Sound surgical principles, such as early diagnosis and repair of artery damage, complete débridement of dead tissue and affective wound closure, in conjunction with intensive general treatment and overall planning, are necessary for maximally rehabilitated limbs. The electrically injured seem to have benefited a great deal from the procedures outlined, with

Influence of graft vascularization on graft survival following homologous limbo-keratoplasty.

PURPOSE: Limbo-keratoplasty enables visual improvement and limbal stem cell transplantation at the same. During follow-up, most grafts show vascularization of the limbus. However, it is unclear whether vascularization is harmful due to immunologic effects or helpful to nourish the limbal stem cells and is therefore necessary for a clear graft. The aim of our study is to analyze the influence of graft vascularization on graft survival following homologous limbo-keratoplasty. METHODS: In this retrospective study, we assessed all consecutive limbo-keratoplasties performed in our hospital. All ey

Serratus anterior intercostal nerve graft: a new vascularized nerve graft.

We present our investigative and clinical experience with a new vascularized nerve graft: the serratus anterior intercostal nerve graft. The serratus branch of the thoracodorsal arterial system was injected with silicone rubber injection compound in seven fresh cadavers (N = 11 injected specimens) after the composite serratus-intercostal structures were harvested. Microdissection of selected vascular territories was then performed. Our findings reconfirmed the previously described vascular connections between the thoracodorsal system and the intercostal vessels via periosteal vessels. We also

Vascularized nerve grafts and vascularized fascia for upper extremity nerve reconstruction.

Since 1976, experimental and clinical studies have suggested the superiority of vascularized nerve grafts. In this study, a 27-year experience of the senior author is presented regarding vascularized nerve grafts and fascia for complex upper extremity nerve reconstruction. The factors influencing outcomes as well as a comparison with conventional nerve grafts is presented. Since 1981, 21 vascularized nerve grafts, other than vascularized ulnar nerve, were used for reconstruction of nerve injuries in the upper extremity. Indications were prolonged denervation time, failure of the previously use

Biomimetic hydrogel design strategies for vascular grafts and vascularized tissue constructs.

Biomimetic design strategies offer rational approaches for reconstructing functional vascular structures within hydrogel platforms. Hydrogels provide unique advantages through tissue-like hydration, tunable architectures, and biochemical functionalization capacity. These properties enable implementation of design principles derived from native vasculature. This review proposes an integrated analytical framework extracting design principles from native vascular architecture and demonstrating their application across two conventionally separate research directions, namely, vascular graft enginee

Peripheral vascular disease as predictor of outcome after coronary artery bypass grafting.

BACKGROUND AND AIMS: Understanding and objective assessment of risks is crucial in cardiac surgery. The aim of this study was to assess the influence of peripheral vascular disease (PVD) on morbidity, mortality and outcome in coronary artery bypass grafting (CABG) patients. MATERIAL AND METHODS: The ankle-brachial pressure index (ABPI) was used as indicator of PVD and was measured in 178 CABG patients. Two groups were established: 1. normal ABPI (0.9-1.3) (n = 136) and 2. lowered ABPI (< 0.9) (n = 35). The mean follow-up was 26 months. RESULTS: The presence of PVD was 20.5 %. Patients with P

Vein harvest ischemia: a peripheral vascular complication of coronary artery bypass grafting.

Lower-extremity ischemia can lead to impaired healing of saphenous vein excision sites in patients with significant peripheral vascular disease (PVD). Five patients who required infrainguinal revascularization for wound necrosis of the harvest site after coronary artery bypass grafting are described. The male/female ratio was 2:3 with a mean age of 67 (range 45-87) years. The most commonly associated problems were insulin-dependent diabetes mellitus (80%) and congestive heart failure (60%). The saphenous vein was harvested from the thigh and leg in three patients and exclusively from the leg i

A Rare Vascular Challenge: Brachial Artery Collateral Pseudoaneurysm Managed With Vein Grafting.

Upper extremity pseudoaneurysms are uncommon, and involvement of collateral branches of the brachial artery is particularly rare. A 78-year-old woman without antecedent trauma presented with a new pulsatile mass and progressive dysfunction of the proximal arm. Duplex ultrasonography provided sufficient diagnostic and planning information, demonstrating a pseudoaneurysm arising from a collateral branch of the brachial artery measuring 38 mm × 29 mm × 46 mm, with an estimated neck diameter of 3 mm and neck length of 5 mm, and a thin peripheral mural thrombus. Given lesion size and compressive fe

High-Density Fat Grafting Assisted Stromal Vascular Fraction Gel in Facial Deformities.

Autologous fat grafting is commonly used for soft tissue augmentation and reconstruction, this technique is limited by a high rate of graft absorption. The stromal vascular fraction gel (SVF-gel) grafting for facial volume augmentation can exert a positive effect on skin rejuvenation, but its major limitation is the low rate of conversion of Coleman fat. The purpose of our study was to investigate a novel surgery using performing high-density fat in combination with SVF-gel in the treatment of hemifacial atrophy, or Romberg diseases. From October 2017 to October 2019, 13 patients with hemifaci

Development of vascular substitutes for low-flow peripheral bypass grafting - a review.

The development of a low-flow vascular prosthesis is a very topical issue. The authors present a pathway for the development of a prosthesis with optimal properties based on the idea of mimicking the characteristics of a biological model (saphenous vein graft) and programming these properties in the model of the prosthetic substitute. The vascular prosthesis presented consists of three layers - a non-absorbable scaffold representing vascular “media”, and two absorbable collagen layers - pseudointima and pseudoadventitia. The basic methods of physical testing are presented - the sin

Double-strut free vascular fibular grafting for reconstruction of the lower extremities.

This article reports our experience on the treatment of long-bone defects with double-strut vascular fibular graft in 11 patients. The defects were 6-12 cm in length while 10-25 cm of vascular fibular grafts were harvested. Nine patients achieved solid union in 6 months, and two patients required additional procedures to achieve solid union. No major complications were observed, except for three patients. who had knee stiffness due to long-term immobilization and one patient who had experienced a stress fracture because of strenuous exercise at 10 months after grafting procedures. Staged coronary artery bypass grafting after percutaneous angioplasty for intracranial vascular stenosis.

OBJECTIVE: Cerebrovascular disease is commonly associated with coronary artery disease and is a major risk factor for cardiac surgery. Concomitant coronary artery bypass grafting and carotid endarterectomy may reduce the risk of stroke; however, this staged operation is effective only for extracranial lesions. The strategy for on-pump coronary artery bypass grafting for patients with intracranial vascular stenosis is still controversial. METHODS: The subjects were 157 consecutive candidates for coronary artery bypass grafting who underwent computed tomography and digital subtraction cerebral

Small Diameter Vascular Grafts Made in Minutes.

Vascular trauma demands rapid intervention to avert severe outcomes, yet current surgical treatments, including invasive autologous grafts and limited, potentially degrading synthetic prostheses, often fail to accommodate the requirements of variable vessel dimensions and shapes. To address these limitations, we present an additive manufacturing strategy for rapid, point-of-care fabrication of customizable vascular grafts using Focused Rotary Jet Spinning (FRJS). This technique enables the rapid, point-of-care fabrication of customizable small-caliber vascular grafts within minutes. FRJS facil

Vascularized fibular graft for pediatric mandibular reconstruction.

BACKGROUND: Although vascularized bone grafts have become well accepted in adults, especially following ablative head and neck procedures, there are few long-term reports of their use in pediatric patients. METHODS: In this study, the authors analyzed the outcomes of 18 free fibula grafts in 16 patients aged 10 months to 21 years (mean, 12 years) with an average follow-up of 5 years. Eleven patients had cancer-related defects, four had craniofacial anomalies, and one had a posttraumatic deformity. All patients with congenital malformations had been followed since birth, and the others had bee

Long bone reconstruction with vascularized bone grafts.

The vascularized fibula may be used for long bone reconstruction reliably and successfully. Since its description by Taylor, the fibula flap has evolved to solve a myriad of long bone reconstructive dilemmas. The flap is used routinely for non-unions, postoncologic resections, and congenital defects. It provides faster and more reliable union than nonvascularized grafts. When constructed in a double-barrel configuration, it offers the ability to bear weight. The vascularized fibula can be used to maintain growth by virtue of transferring the physis. The vascularized fibula is thus both the wor

Subconjunctival bevacizumab for vascularized rejected corneal grafts.

We describe 3 patients who experienced corneal neovascularization following keratoplasty and were treated with a single subconjunctival injection of 2.5 mg bevacizumab. Although there was immediate regression of the corneal vascularization and haze, as well as improvement in the anterior chamber reaction, the effects were short-lived and starting from the second week, the corneal vessels began to progress. All 3 cases ended in permanent graft failure. The series suggests a possible role for bevacizumab in the management of graft rejection with vascularization.

Iliac graft vascularization for femoral head osteonecrosis.

UNLABELLED: We determined the long-term clinical and radiologic outcomes of patients with osteonecrosis treated with a combination of autologous cancellous bone impaction and pedicled iliac bone block transfer perfused by the ascending branch of the lateral femoral circumflex artery. We retrospectively reviewed 197 patients (226 hips) operated on from 1985-1998 who had a mean age of 38 years (range, 19-65 years) and an average followup of 12.5 years (range, 6-19 years). Fourteen hips (13 patients) (6%) had conversion to a total hip arthroplasty because of progressive collapse, severe pain, or
