Gender Disparities in Secondary Patency of Arteriovenous Fistulas: Insights into Hemodialysis Outcomes and Long-Term Vascular Access Viability.

Background: Autologous arteriovenous fistulas (AVFs) are the preferred vascular access modality for hemodialysis due to superior durability and lower infection rates. However, long-term secondary functional patency remains a critical determinant of dialysis success and patient survival. Although sex-related differences in vascular access outcomes have been reported, their impact on long-term secondary patency remains insufficiently clarified. Objective: The primary objective of this study was to evaluate long-term gender differences in the secondary functional patency of AVFs and to determine

Vascular surgical society of great britain and ireland: duplex surveillance does not enhance infrainguinal prosthetic bypass graft patency.

BACKGROUND: All patients discharged from this unit with a patent infrainguinal polytetrafluoroethylene (PTFE) bypass are entered into a graft surveillance programme. Previous internal audit had suggested that duplex surveillance was not worthwhile in this patient group. The aim of this study was to compare the fate of infrainguinal PTFE grafts before and after stopping duplex surveillance. METHODS: Between January 1986 and December 1994, 220 grafts (141 above-knee popliteal, 69 below-knee popliteal, ten tibial) were entered into the duplex surveillance programme. Between January 1995 and June

One-year patency rate of native arteriovenous fistulas reconstructed by vascular stripping in hemodialysis patients with venous neointimal hyperplasia.

OBJECTIVE: There are limited therapeutic measures for stenosis of arteriovenous fistulas (AVFs) due to venous neointimal hyperplasia (VNH). In the current retrospective study, we reviewed the clinical data of hemodialysis patients who underwent AVF reconstruction by VNH stripping. The primary measure of interest was the secondary patency rate of the restored AVF. METHODS: The study included hemodialysis patients who underwent AVF reconstruction by VNH stripping (group A), AVF reconstruction proximal to the original fistula (group B), or creation of a new AVF (group C). Patency was evaluated i

Patients undergoing infrainguinal bypass to treat atherosclerotic vascular disease are underprescribed cardioprotective medications: effect on graft patency, limb salvage, and mortality.

INTRODUCTION: Established American Heart Association guidelines recommend the use of statin drugs, angiotensin converting enzyme (ACE) inhibitors, and antiplatelet agents in patients with systemic atherosclerosis, such as those undergoing operative intervention to treat peripheral atherosclerotic disease. Many of these patients have not received treatment of coronary heart disease and have not been prescribed these medications. Whether statin drugs and ACE inhibitors confer an improvement in graft patency, limb salvage, and operative mortality is unknown. METHODS: Consecutive patients who und

Obesity is an independent risk factor for primary patency loss in 12 months: A 3-year retrospective study of vascular access outcomes.

BACKGROUND: Obesity is common among patients with end-stage renal disease; however, its impact on vascular access (VA) outcomes remains uncertain. We aimed to evaluate the association between body mass index (BMI) and VA patency. METHODS: We retrospectively analyzed 396 patients who underwent arteriovenous fistula (AVF) or arteriovenous graft (AVG) creation (2021-2024), stratifying them by BMI (cutoff of 30 kg/m²). Primary outcomes included primary patency and assisted primary patency, secondary patency, and patient survival. RESULTS: We analyzed 396 patients (mean age 66.0 ± 14.2 years; 62.

Evaluating pulsatile flushing by pushing method and catheter size for educating nurses on peripheral vascular patency: a computational fluid dynamics simulation analysis.

BACKGROUND: Maintaining the patency of peripheral intravenous catheters is essential for preventing occlusion and catheter-related complications. Clinical guidelines and field education emphasize the effectiveness of pulsatile flushing for this purpose. However, due to the absence of standardized methods, its implementation among clinical nurses remains low. This study aimed to compare the effects of pulsatile flushing according to different infusion methods and catheter sizes to support the education of clinical nurses on intravenous infusion therapy. The findings aim to provide foundational

Case Report: Maintaining a balance between vascular access patency and stable dissection status in a hemodialysis patient with unrepaired type A aortic dissection.

INTRODUCTION: Type A aortic dissection (AD) is a lethal situation with high mortality within short time after onset. We present here a rare hemodialysis patient whose condition was comorbid with unrepaired type A AD. The challenge we face is whether low-molecular-weight heparin (LMWH) should be used during dialysis. CASE PRESENTATION: A 72-year-old man with a history of hemodialysis for 2 years and 7 months sought medical attention due to thrombosis of the dialysis catheter. He had been diagnosed with an unrepaired type A aortic dissection (involving the aortic root, the ascending aorta, the

Equivalent secondary patency rates of upper extremity Vectra Vascular Access Grafts and transposed brachial-basilic fistulas with aggressive access surveillance and endovascular treatment.

OBJECTIVES: The 2006 update of the DOQI guidelines has stated that in patients with end-stage renal disease, autogenous radial-cephalic, or brachial-cephalic fistulas are the preferred access modalities, followed by transposed brachial-basilic (TBB) fistulas and prosthetic arteriovenous (AV) grafts. AV grafts are in general least preferred; however, there is very limited data comparing directly the last two modalities. The aim of the present study is to compare outcomes of the TBB fistula and the Vectra Vascular Access Graft. METHODS: Seventy-six patients had a prosthetic brachial-axillary Ve

Angiotensin Converting-Enzyme Inhibitors, Angiotensin Receptor Blockers, and Calcium Channel Blockers Are Associated with Prolonged Vascular Access Patency in Uremic Patients Undergoing Hemodialysis.

BACKGROUND: Vascular access failure is a huge burden for patients undergoing hemodialysis. Many efforts have been made to maintain vascular access patency, including pharmacotherapy. Angiotensin converting enzyme inhibitor (ACE-I), angiotensin receptor blocker (ARB), and calcium channel blocker (CCB) are known for their antihypertensive and cardio-protective effects, however, their effects on long-term vascular access patency are still inconclusive. DESIGN, SETTING, PARTICIPANTS AND MEASUREMENTS: We retrospectively enrolled patients commencing maintenance hemodialysis between January 1, 2000,

Magnetic resonance angiography in diagnostic long-term follow-up of primary patency of the MOTIV drug-eluting bioresorbable vascular scaffold in the region below the knee: 5 years of experience.

PURPOSE: Treatment of peripheral artery disease (PAD) in the region below the knee (BTK) is dissatisfying as failure of treated target lesions (TLF) is frequent and diagnostic imaging is often challenging. In the BTK-region metallic drug-eluting stents (mDES) yielded best results concerning primary patency (PP), but also annihilate signal in magnetic resonance angiography (MR-A). A recently introduced non-metallic drug eluting bioresorbable Tyrocore® vascular scaffold (deBVS), that offers an option for re-treatment of lesions due to its full degradation within 3-4 years after placement, was in

Vascular access creation in octogenarians: The effect of age on outcomes.

BACKGROUND: The prevalence of end-stage renal disease is accelerating among older age groups. Patient-specific factors in the elderly patient group might advocate for a different vascular access creation approach, in which patency, risk of nonmaturation, and time to cannulation with accompanied prolonged catheter use should be of primary importance. The aim of this study was to determine which vascular access has the best outcomes and to determine whether age is associated with different outcomes after vascular access surgery. METHODS: Data were obtained from a prospectively maintained databa

[Value of preserved saphenous vein graft for the creation of access ports in hemodialyzed patients: report of 309 cases].

INTRODUCTION: Biological grafts are rarely used for the creation of vascular access, despite their many advantages. Our group prefers to use distal vascular accesses with interposition of a biological graft when direct access cannot be achieved. The objective of this study was to determine the long-term patency and the complication rate of a series of 309 vascular access procedures using a preserved saphenous (PS) vein homograft. MATERIALS AND METHODS: 410 (27%) of the 1,500 vascular access procedures performed by our group required the use of a graft, including 376 PS vein homografts (25%).
