Inferior vena cava injuries--a continuing challenge.

The mortality from abdominal vena cava trauma remains in excess of 33% despite advances in prehospital and intraoperative care. During the 7-year period ending December 1981, 58 patients with vena cava injuries were treated at our institution. Thirty-nine (67%) were due to gunshot wounds, nine to stab injuries, and ten to blunt trauma. Overall mortality was 38%. Predictors of poor survival were: 1) presence of shock upon hospital arrival; 2) multiple abdominal vascular injuries; and 3) injuries in the retrohepatic segment. Only two (17%) of 12 patients survived retrohepatic wounds despite vari

Popliteal vascular trauma. A community experience.

Preventing amputation continues to be a significant challenge when popliteal vascular injuries occur. A retrospective review of cases from the San Diego County (California) Trauma System identified 108 patients with 76 blunt and 32 penetrating arterial injuries. The limb preservation rate was 88%; there were 13 amputations. The fracture-dislocated knee injury and close-range shotgun blasts were particularly limb threatening. In general, the trauma system achieved rapid evaluation of injuries and early operative intervention. All 13 patients who underwent amputations presented with signs of sev

Avoidance and management of neurovascular injuries in total hip arthroplasty.

Neural injuries that occur after total hip arthroplasty (THA) can be classified as involving either the central nervous system or peripheral nerves. Central nervous system changes after THA may be attributed to increased appreciation of fat embolism syndrome associated with THA. Certain maneuvers such as impacting the acetabulum, femoral reaming, and cement pressurization can force marrow fat into the venous system. When there is an associated right to left shunt, paradoxical embolization can occur, which may account for previously unexplained cases of confusion and mental status changes after

Pathogenesis of vasculopathy in systemic sclerosis and its contribution to fibrosis.

PURPOSE OF REVIEW: In patients with systemic sclerosis (SSc), vascular manifestations precede skin and organ fibrosis. There is increasing evidence demonstrating a pathogenic link between early vascular injury and subsequent development of tissue fibrosis. RECENT FINDINGS: Our knowledge of cellular and molecular mechanisms underlying a unique relationship between SSc-related vasculopathy and fibrosis has changed over the last few years. There is increasing evidence showing viral infection as a potential trigger elucidating vascular injury. Due to defective vascular repair machinery, this init

Epidemiology of Vascular Injury Within the U.S. Department of Defense Trauma Registry From 2007-2023.

Background: Since WWII, the incidence of vascular trauma in U.S. military casualties has steadily risen, with the highest rates having been reported from the recent conflicts in Iraq and Afghanistan. Within civilian populations, the incidence of vascular trauma is lower, but with disproportionately higher rates of mortality, morbidity, and resource use for these injuries. Analyses of trauma databases have informed technological advances that best help our U.S. Military Health System (MHS) manage vascular injuries. We sought to perform a retrospective cohort analysis in military casualties with

Imaging of penetrating injuries of the head and neck:current practice at a level I trauma center in the United States.

Penetrating neck injuries are commonly related to stab wounds and gunshot wounds in the United States. The injuries are classified by penetration site in terms of the three anatomical zones of the neck. Based on this zonal classification system, penetrating injuries to the head and neck have traditionally been evaluated by conventional angiography and/or surgical exploration. In recent years, multidetector-row computed tomography (CT) angiography has significantly improved detectability of vascular injuries and extravascular injuries in the setting of penetrating injuries. CT angiography is a

Inclusion of open injuries in an updated Schenck classification of knee dislocations based on a global Delphi consensus study.

OBJECTIVES: Knee dislocations (KDs) are complex injuries defined as incongruity of the tibiofemoral joint, which leads to tears of two or more of the main stabilising knee ligaments, and they are often associated with damage to surrounding soft tissue or neurovascular structures. A classification system for these injuries should be simple and reproducible and allow communication among surgeons for surgical planning and outcome prediction. The aim of this study was to formulate a list of factors, prioritised by high-volume knee surgeons, that should be included in a KD classification system. M

Determining the prognosis for limb salvage in major vascular injuries with associated open tibial fractures.

Nineteen patients with open tibial fractures and associated arterial injuries were reviewed. Vascular injuries were documented by either arteriography (74%) or clinical examination and direct visualization of injuries intraoperatively. Arterial injuries were divided into those proximal to and those distal to the trifurcation. Proximal injuries involved the popliteal artery in all cases, whereas distal injuries involved the anterior tibial artery, posterior tibial artery, peroneal artery, or combinations of these arteries. All patients' limbs were initially considered salvageable. The eventual

Vascular trauma in the groin: contrast between iliac and femoral injuries.

Vascular injuries to the groin are common and often life-threatening. Injuries above the inguinal ligament, to the iliac system, are associated with a 37 percent mortality. Associated intraabdominal injuries are common. These patients must be identified promptly, given broad-spectrum antibiotics and taken immediately to the operating room for exploration through a midline incision. Injuries below the inguinal ligament are usually to the femoral vessels and are rarely fatal. These patients may be evaluated more extensively before leg exploration. Associated intraabdominal injuries are unusual,

Injuries to the azygous venous system.

BACKGROUND: Azygous venous system injuries are rare. Although not commonly classified as a thoracic great vessel, the azygous system manifests morbidity and mortality that is similar to that of other great vessel injuries. METHODS: Over a 40 year period, data were retrospectively collected and charts reviewed from a vascular injury database. RESULTS: A total of 22 injuries to the azygous venous system were identified with 21 to the azygous vein and 1 to the hemiazygous vein. All were secondary to penetrating trauma, with 19 from gunshot wounds and 3 from stab wounds. Eight of 22 patients die

Vascular injuries due to cerclage passer: Relevant anatomy and note of caution.

PURPOSE: The cerclage passer instrument was introduced for aiding percutaneous reduction in spiral femur fractures, thereby overcoming the risk of devascularization of the fracture ends commonly associated with traditional open cerclage wiring techniques. This may, however, predispose to iatrogenic vascular injuries owing to the proximity of the vessels and several other factors. We report four cases with femoral artery (superficial and deep) injuries caused by the cerclage passer in proximal femur and distal femur shaft fractures and incidence of these injuries in our institution. METHODS: R

Further insight into systemic sclerosis from the vasculopathy perspective.

Systemic sclerosis (SSc) is an autoimmune disease characterized by immune dysfunction, vascular system dysfunction, and tissue fibrosis. Vascular injury, vascular remodeling, and endothelial dysfunction are the hallmark pathological changes of the disease. In the early stages of SSc development, endothelial cell injury and apoptosis can lead to vascular and perivascular inflammation, oxidative stress, and tissue hypoxia, which can cause clinical manifestations in various organs from the skin to the parenchymal organs. Early diagnosis and rational treatment can improve patient survival and qual

Open injuries and obesity as emerging risk factors for vascular injury in knee dislocations: A retrospective study.

PURPOSE: Knee dislocations (KDs) are rare in orthopaedic trauma but can have devastating effects. Failing to diagnose a concomitant vascular injury can lead to amputation. Different methods are used for the diagnosis of a vascular injury and the best approach is under debate. This study aims to examine the risk factor(s) of vascular injury in patients with KDs, examine variables that differ between obese patients and non-obese patients with KDs, and analyse the modalities used to identify vascular injuries in the KD population. METHODS: The electronic patient medical record system at a major

Angiography and transcatheter arterial embolization of vascular injuries of the face and neck.

The condition of patients sustaining penetrating neck trauma often appears deceptively stable, even when major structures have been injured. The clinician must identify patients who require treatment and limit invasive procedures in those without significant injuries. Angiography is often used to search for vascular damage following penetration of the neck and face. The charts of 401 hemodynamically stable patients with penetrating cervicofacial wounds who were evaluated by angiography followed, when necessary by either transcatheter arterial embolization and observation or surgery were review

Severe upper limb injuries with or without neurovascular compromise in children and adolescents--analysis of 32 cases.

The healing and regeneration capacity of the injured tissues in childhood, adolescence, and adult life differs significantly. As a result, the prognosis of compound injuries of the upper limb in different age groups varies; therefore, the decision making and management of these cases should be age-specific. This article presents a series of 32 patients aged 1.5-14 years, with compound injuries of the upper limb that have been treated in our hospital during the period of the last 6 years. Ten of the above cases involved major vascular lesions that required revascularization or replantation. The

A Review of Radiation-Induced Vascular Injury and Clinical Impact.

The number of cancer survivors continues to increase because of advances in therapeutic modalities. Along with surgery and chemotherapy, radiotherapy is a commonly used treatment modality in roughly half of all cancer patients. It is particularly helpful in the oncologic treatment of patients with breast, head and neck, and prostate malignancies. Unfortunately, among patients receiving radiation therapy, long-term sequalae are often unavoidable, and there is accumulating clinical evidence suggesting significant radiation-related damage to the vascular endothelium. Ionizing radiation has been k

[Vascular complications after cranio-facial trauma].

Despite recent advances in automobile safety, facial trauma remains a common event. Cranio-facial trauma, which usually occurs within a context of multiple trauma, can, in some patients, lead to serious life-threatening vascular complications. Such injury usually involves the carotid system (hemorrhage, aneurysm, dissection). Management of these vascular injuries must be given the same priority as other multiple trauma injuries. We present few typical cases illustrating carotid-cavernous or vertebro-vertebral fistulae, false aneurysms, arterial dissections or oro-facial bleeding. Bleeding is g
