Cerebral vasculitis.

The noninfectious, inflammatory vasculitides include giant cell arteritis, Takayasu disease, Churg-Strauss angiitis, Wegener disease, polyarteritis nodosa, microscopic polyangiitis, Buerger disease, amyloid-β-related angiitis, and isolated vasculitis of the central nervous system. While these disorders are relatively uncommon, they produce a variety of neurologic diseases including muscle disease, mononeuropathy multiplex, polyneuropathy, cranial nerve palsies, visual loss, seizures, an encephalopathy, venous thrombosis, ischemic stroke, and intracranial hemorrhage. The multisystem vasculitide

Ocular vasculitis.

Vasculitis is the inflammation of blood vessels that leads to loss of perfusion and ischemia with necrosis. When this occurs in the orbit, the consequences are typically very conspicuous and can be devastating with decreased quality of life and loss of vision. Systemic vasculitides are often related to ophthalmic disorders, which can serve as the first diagnostic manifestation of potentially life-threatening disease. Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitides (e.g., granulomatosis with polyangiitis), Behcet's disease, rheumatoid arthritis, sarcoidosis, and systemic lupu

Vasculitis mimics.

PURPOSE OF REVIEW: There are many disorders that may closely resemble the clinical, radiologic and/or pathologic features of the primary vasculitides. In this review, we focus on recently described and under-recognized syndromes that may mimic vasculitis. RECENT FINDINGS: Hereditary causes of large-artery aneurysms such as Marfan's syndrome have long been recognized; recent years have seen a greater understanding of the genetics of Marfan's and other such disorders, including Loeys-Dietz syndrome and Ehler-Danlos syndrome type IV. Under-recognized mimics of medium-vessel vasculitis include se

Cryoglobulinemic vasculitis.

PURPOSE OF REVIEW: Cryoglobulinemic vasculitis is an immune-complex-mediated systemic vasculitis involving small-medium-sized vessels. A causative role of hepatitis C virus in over 80% patients has been definitively established, with heterogeneous geographical distribution. This review focuses on recent etiopathogenetic, clinico-diagnostic, and therapeutical studies. RECENT FINDINGS: Hepatitis C virus cannot be integrated into the host genome; it may exert a chronic stimulus to the immune system. The interaction between hepatitis C virus envelope protein E2 with B-cell CD 81 receptor may incr

Pulmonary vasculitis.

Pulmonary vasculitis describes a number of distinct disorders that are pathologically characterized by the destruction of blood vessels. The clinical manifestations of each disorder are defined by the size, type, and location of the affected vasculature. The clinical approach to these disorders rests upon an astute clinician considering the diagnosis and identifying the specific patterns of clinical, radiologic, laboratory, and pathologic abnormalities. Lung involvement is most commonly seen with the primary, idiopathic, small-vessel, or antineutrophil cytoplasmic antibody-associated vasculiti

Pulmonary vasculitis.

This review summarizes the radiological manifestations of the vasculitides of proven or presumed immunologic origin in which the inflammatory reaction is directed primarily against the vessel wall. These include Wegener's granulomatosis, Churg-Strauss syndrome, Takayasu's arteritis, Behçet's syndrome, Goodpasture's syndrome, and microscopic polyangiitis. Chest radiography is used routinely in the initial evaluation and follow-up of these patients. The radiographic findings however are nonspecific and need to be interpreted together with the clinical findings. Computed tomography (CT) plays an

CNS vasculitis.

Vasculitis of the central nervous system can be of several varieties depending upon the vessel(s) involved and type of disorder. One can see primary CNS vasculitis as a distinct entity which is primarily manifested as central nervous system injury in a vascular distribution or the vasculitic process can be secondary to a systemic disorder such as systemic lupus erythematosus (SLE) or polyarteritis nodosa (PAN). The inflammation of the CNS vessels can be immune mediated or infectious in nature and a number of "triggers" have been identified including hypersensitivity states. It is quite probabl

Introduction. Vasculitis.

The vasculitides commonly create diagnostic and management problems, and a biopsy diagnosis of vasculitis usually does not stand alone in the proper medical management of a patient. Indeed, a conclusive diagnosis usually cannot be made without correlation with the patient's clinical history, physical findings, and/or angiographic findings. Nonetheless, a definitive diagnosis depends on histologic examination because few vasculitic syndromes have specific clinical and laboratory findings. Moreover, the histopathologic diagnosis depends on pathologist experience, tissue selection, sample size, c

[Livedo vasculitis].

Twenty one patients with livedoid vasculitis are presented. Diagnosis was based on the clinical, histopathologic and direct immunofluorescent findings. The majority of patients were women (2:1), aged up to 30 years. Except for two patients with chronic venous insufficiency, the rest were with idiopathic-primary livedoid vasculitis. They all reacted favorably to the therapy with pentoxifylline, dipyridamole and acetylsalicylic acid. It is necessary to observe the difference to the "real" vasculitis of the skin, which requires more aggressive therapy.

Pulmonary vasculitis.

Pulmonary vascular inflammation may be seen in a variety of primary lung diseases and in the setting of numerous systemic illnesses. This article reviews those entities in which pulmonary vasculitis represents a central feature of the pathologic process (Wegener's granulomatosis, Churg-Strauss syndrome, and pulmonary capillaritis). In addition, features of pulmonary involvement in other systemic vasculitides (Giant Cell Arteritis, Takayasu's Arteritis, and Behçet's disease) are described. Finally, general principles for the treatment of vasculitis are reviewed.

[Retinal vasculitis].

PURPOSE: The purpose of this study is to determine aetiologies of vasculitis of retina in Morocco. MATERIAL AND METHODS: This is a retrospective study on 108 cases of vasculitis of retina admitted in the ophthalmology department, for a 5-year period (1987-1993). Our patients had a complete ophthalmological examination : fluorescine angiography, colour vision, general examination and biological study. RESULTS: Many aetiologies have been found, predominantly Behcet's disease. The neovascularization of retina which complicates the vasculitis was detected in 30 % of our patients. A laser photoco

[Candida vasculitis].

Apart from direct skin attacks, yeasts of the Candida group hold a place that is much discussed in the occurrence of immuno-allergic reactions of the skin, because of their usual saprophytic characteristics. We report here on observation of a patient of 62 years who presented with a secondary purpuric maculo-papular eruption making a tri-symptom of Gougerot, for which we have incriminated the role of the immunogenicity of Candida. Skin histology confirmed the presence of an angiitis and necrotising capillarity. An intradermal test with candidine reproduced the skin symptoms and histology, whil

Renal vasculitis.

During most of the 20th century, the medical literature on systemic vasculitis has been dominated by "splitters," who sought to characterize the precise clinical and pathologic differences among these diseases. However, in the past decade in particular, those interested in renal vasculitis have tended to become "lumpers," who stressed overlap, particularly among patients with microscopic polyarteritis, necrotizing and crescentic glomerulonephritis, and Wegener's granulomatosis. The discovery of antineutrophil cytoplasmic antibodies has provided additional evidence of commonality among these re

[Immunocomplex vasculitis].

The clinician suspects immunocomplex vasculitis when he cannot find a common denominator for the concurrent affection of various neighbouring organs and tissues. These affections result from the inflammatory response of the organism to the deposition of non-phagocytized circulating immunocomplex into the vascular wall. Despite the delicacy of attempts to classify them, the author tried to differentiate them into non-progressing immunocomplexes vasculitis, which recedes, and progressing immunocomplex vasculitis which may be either primary or secondary. Among primary ones he includes nodose poly

Cutaneous vasculitis.

Cutaneous vasculitis comprises a wide spectrum of clinical syndromes and histopathologic findings which share the common theme of vascular inflammation and changes in the adjacent tissue. This article discusses several types of cutaneous vasculitides including leukocytoclastic vasculitis, Henoch-Schönlein purpura, urticarial vasculitis, livedoid vasculitis, and granulomatous vasculitides. The clinical patterns of these types of vasculitic syndromes as well as associated diseases and histopathology are discussed.

Gallbladder vasculitis.

Five cases of gallbladder vasculitis were encountered during a 10-year period in a community hospital with annual surgical cases of about 12,000. All five patients were clinically diagnosed as having cholelithiasis with or suspected to have cholecystitis. Vasculitis in the form of arteritis identical to that seen in polyarteritis nodosa was present microscopically in the cholecystectomy specimens. One patient presented with systemic symptoms, and the diagnosis of polyarteritis nodosa was established after arteritic lesions were identified in the cholecystectomy, liver biopsy, and appendectomy

Retinal vasculitis.

Evidence is now accumulating on both clinical and experimental grounds that the retina is an a priori source of inflammatory activity. Reactive inflammation in the retina may produce many of the clinical signs previously ascribed to uveal inflammation. Autoimmune mechanisms are probably responsible for the majority of cases of retinal vasculitis. Autoimmune retinal vasculitis occurs without other classical signs of inflammatory response in any other parts of the body. When associated systemic manifestations occur they may reflect different underlying immunopathogenic abnormalities. Thus in dis
