Intracranial vasospasm with subsequent stroke after traumatic subarachnoid hemorrhage in a 22-month-old child.

Clinical and radiographic evidence of subarachnoid hemorrhage (SAH)-related vasospasm is rare in children and has not been reported in infants. In this report the authors present the case of a 22-month-old child who developed clinically symptomatic, radiographically identifiable vasospasm after traumatic SAH. To the authors' knowledge, this is the first report of vasospasm associated with SAH in a child this young. This 22-month-old boy fell and had a dense SAH. He had a history of surgically corrected craniosynostosis and nonsymptomatic ventriculomegaly. The boy was evaluated for occult vascu

[Preventive effect on intracranial arterial vasospasm using combined ventriculo-cisternal and cisternal drainage (author's transl)].

Cerebral vasospasm following subarachnoid hemorrhage (SAH) is one of the most important problems in the treatment of ruptured intracranial aneurysms. Several reports have recently emphasized that oxyHb from erythrocytes might be responsible for vasospasm. In this study, the hem-oxygenase which converts oxyHb to bilirubin was analyzed and the increase of hem-oxygenase in the CSF after SAH was reconfirmed. In 23 cases of ruptured intracranial aneurysms, combined ventriculo-cisternal drainage and cisternal drainage designed by us was performed after clipping of aneurysms in early stage after rupt

Incidence and Outcome of Peri-interventional Vasospasm During Endovascular or Microsurgical Treatment of Unruptured Intracranial Aneurysms.

BACKGROUND: Peri-interventional vasospasm (PIVS) is associated with high risk of delayed cerebral vasospasm (DCVS), delayed cerebral ischemia, and poor outcome after aneurysmal subarachnoid hemorrhage. However, the incidence rate associated with treatment of unruptured intracranial aneurysm (UIA) remains unclear. OBJECTIVE: To define the incidence and clinical significance of PIVS in UIA repair based on intraoperative/peri-interventional digital subtraction angiography. METHODS: A consecutive series of 205 patients who underwent UIA treatment by means of microsurgical clipping (n = 109) or e

Vasospasm and delayed cerebral ischemia after uneventful clipping of an unruptured intracranial aneurysm - a case report.

BACKGROUND: Due to improvements in both the quality and availability of intracranial imaging as well as the evolution of surgical and endovascular techniques during the last decade, the number of treatments of unruptured intracranial aneurysms (UIA) has increased steadily. However, it is not generally known that vasospasm can arise after an uneventful clipping. CASE PRESENTATION: We present a case of a 69-year-old woman who suffered from vasospasm and delayed cerebral ischemia that occurred after an uneventful clipping of a UIA. The aneurysm of the right middle cerebral artery was found incid

Cystic Encephalomalacia following Vasculopathy and Vasospasm of Proximal Intracranial Arteries Due to Pneumococcal Meningitis in a Infant.

Despite the availability of modern antibiotics, pneumococcal meningitis in both children and adults remains a severe disease-one known to frequently cause grave complications and residual disability. Although the appearance of arterial vasospasms in bacterial meningitis systematically has been investigated and reported on for adult patients, such research is lacking when it comes to infants. We report on a 4-week-old infant who, 6 days after onset of pneumococcal meningitis, suffered severe neurological deterioration with treatment-resistant seizures and coma. Generalized cortical and subcorti

[HES 130/0,4/9:1 effect on intracranial pressure, cerebral blood flow, oxygenation and cerebral vasospasm].

The paper contains results of clinical investigation of influence of intravenous infusion of 6% Hydroxyethyl starch 130/0.4/9:1 (Voluven, Fresenius Kabi) on cerebral blood flow, cerebral metabolism and ICP in 7 patients with cerebral angiospasm after subarachnoid hemorrhage. It's shown that such infusion in patients with cerebral vasospasm doesn't lead to ICP elevation but can cause regional volumetric blood flow rising and improving of cerebral metabolism.

Symptomatic Cerebral Vasospasm After Clipping of an Unruptured Intracranial Aneurysm: A Case Report and Literature Review.

Symptomatic cerebral vasospasm after clipping of unruptured intracranial aneurysms (UIA) is extremely rare. Furthermore, its pathophysiology and risk factors are poorly understood. Delayed diagnosis and treatment can lead to severe neurological deficits. We report the case of a 53-year-old woman who developed delayed symptomatic cerebral vasospasm seven days after clipping of an unruptured left middle cerebral artery (MCA) aneurysm. The patient presented with right-sided hemiparesis and motor aphasia. Digital subtraction angiography revealed vasospasms from the M1 to M2 segments. Treatment wit

The Role of Computed Tomographic Angiography in Predicting the Development of Vasospasm Following Ruptured Intracranial Aneurysm Microsurgery.

Introduction Following subarachnoid hemorrhage, cerebral vasospasm is the primary cause of morbidity and death. The aim of this study is to predict the development of vasospasm by detecting changes in vessel diameter after surgery using computed tomography angiography. Methods We retrospectively evaluated the patients who underwent aneurysm clipping due to a bleeding aneurysm between 2019-2022. Age, gender, location, subarachnoid hemorrhage grades, development of perioperative rupture, and temporary clip use were examined. Preoperative and postoperative diameters of the internal carotid artery

How to differentiate intracranial atherosclerotic disease or vasospasms after mechanical thrombectomy. Be patient or vasodilator is the secret?

Here we describe a successful mechanical thrombectomy (MT) for acute large vessel occlusion in stroke treatment with one passage (thrombolysis in cerebral infarction, TICI 3). Immediately after the withdrawing of the stent retriever, a narrowing of the middle cerebral artery was diagnosed. The rate of vasospasms during this procedure can be as higher as 41% (range from 6-41%). Here we describe our protocol when a narrowing of the artery is visualized after a stent retriever is withdrawn. A patient presented in our emergency room with National Institute of Health Stroke Scale (NIHSS) of 21, Alb

Risk factors in intracranial saccular aneurysms. Aspects on the formation and rupture of aneurysms, and development of cerebral vasospasm.

Intracranial saccular aneurysms have been a well-known clinical and pathological entity for over two centuries. The pathophysiological events that lead to aneurysm formation and rupture are, however, poorly understood. Besides an HLA-associated genetic factor, the most widely accepted risk factors are arterial hypertension, female gender, and increasing age. Some aneurysm patients have a deficient formation of Type III collagen. This seems to interfere with the mechanical integrity of the cerebral arterial wall encouraging aneurysm formation. While some of the risk factors may be involved in t

Machine learning models for predicting vasospasm following ruptured intracranial aneurysms: a systematic review and meta-analysis.

BACKGROUND: Cerebral vasospasm remains a leading cause of delayed cerebral ischemia following aneurysmal subarachnoid hemorrhage (aSAH). Despite advances in critical care, current monitoring strategies are reactive and non-personalized. Machine learning (ML) has emerged as a promising tool to anticipate vasospasm risk. METHODS: A systematic review and meta-analysis were performed following PRISMA 2020 guidelines. PubMed and Embase databases were searched for studies applying ML algorithms to predict clinical or radiological vasospasm. Data were pooled using bivariate and proportional meta-ana

Management of radiological cerebral vasospasm after coil embolization for ruptured intracranial aneurysms: A noninferiority bicentric cohort study.

OBJECTIVE: Whether radiological cerebral vasospasm (CVS) should be proactively treated following subarachnoid hemorrhage to reduce the development of delayed cerebral ischemia (DCI) and brain infarction remains unclear. In this bicentric retrospective study, we sought to determine if a first-line intravenous therapy (IVT) to early detect and treat CVS could be noninferior on neurological outcomes to a first-line endovascular therapy (ET) requiring intra-arterial procedures for confirmed CVS as the reference approach. METHODS: Two cohorts of unselected patients exposed to the risk of CVS were

Gut microbiome in intracranial aneurysm growth, subarachnoid hemorrhage, and cerebral vasospasm: a systematic review with a narrative synthesis.

Intracranial aneurysms (IA) are the most common cerebral vascular pathologies. Their rupture leads to the most dangerous subtype of stroke-aneurysmal subarachnoid hemorrhage (aSAH), which may be followed by cerebral vasospasm and ischemic sequelae. Recently, an imbalance within the intestinal microbiota, referred to as dysbiosis, was suggested to play a role in the formation, progression, and rupture of IA. As no systematic review on this topic exists, considering the significance of this matter and a lack of effective prophylaxis against IA or cerebral vasospasm, we aim to sum up the current

Cerebral angioplasty using the Scepter XC dual lumen balloon for the treatment of vasospasm following intracranial aneurysm rupture.

BACKGROUND: Cerebral vasospasm following subarachnoid hemorrhage (SAH) results in significant morbidity and mortality. Intra-arterial administration of calcium channel blockers or intracranial angioplasty may be performed when non-invasive medical management fails to prevent neurologic deterioration. Technical improvements in balloon catheters are expected to improve the success and safety of cerebral angioplasty. OBJECTIVE: To describe our initial experience with the new Scepter XC balloon catheter in cerebral vasospasm treatment following SAH. DESIGN: All patients who underwent cerebral an

Inhibition of cerebral vasospasm by intracranial delivery of ibuprofen from a controlled-release polymer in a rabbit model of subarachnoid hemorrhage.

OBJECT: Leukocyte-endothelial cell interactions may play a role in the development of cerebral vasospasm after aneurysmal subarachnoid hemorrhage (SAH) because the extravasation of circulating leukocytes into the periadventitial space within 24 hours after the hemorrhage appears to be a critical event in this process. Ibuprofen is an antiinflammatory agent that inhibits the expression of specific cell adhesion molecules and, consequently, disrupts leukocyte-endothelial cell interactions. The authors investigated the efficacy of ibuprofen delivered locally from controlled-release polymers in th

Treatment of unusually located traumatic intracranial aneurysms and severe vasospasm following a gunshot wound to the head: A case report.

BACKGROUND: Traumatic intracranial aneurysms (TICAs) represent up to 1% of all intracranial aneurysms. They can be the result of non-penetrating and penetrating brain injury (PBI). Approximately 20% of TICA are caused by PBI. Endovascular treatments as well as surgical clipping are reported in the literature. Other vascular complications of PBI include vasospasm although the literature is lacking on this topic. CASE DESCRIPTION: The authors present a unique case of multiple TICAs after a PBI in a 15-year-old patient who sustained a gunshot wound to the head. The patient sustained injury throu

Molecular Basis of Cerebral Vasospasm: What Can We Learn from Transcriptome and Temporal Gene Expression Profiling in Intracranial Aneurysm?

Cerebral vasospasm (CV) is a significant complication following aneurysmal subarachnoid hemorrhage (aSAH), and lacks a comprehensive molecular understanding. Given the temporal trajectory of intracranial aneurysm (IA) formation, its rupture, and development of CV, altered gene expression might be a molecular substrate that runs through these clinical events, influencing both disease inception and progression. Utilizing RNA-Seq, we analyzed tissue samples from ruptured IAs with and without vasospasm to identify the dysregulated genes. In addition, temporal gene expression analysis was conducted

Delayed intracranial delivery of a nitric oxide donor from a controlled-release polymer prevents experimental cerebral vasospasm in rabbits.

OBJECTIVE: Decreased local availability of nitric oxide (NO) may mediate chronic vasospasm after aneurysmal subarachnoid hemorrhage (SAH). Previous reports have shown that early treatment with NO prevents vasospasm in animals. We evaluated the efficacy of controlled-release polymers that contain the NO donor diethylenetriamine (DETA-NO) for the delayed treatment of vasospasm in a rabbit model of SAH. METHODS: DETA-NO 20% (wt/wt) was incorporated into ethylene-vinyl acetate (EVAc) polymers. Animals (n = 52) were randomized to two experimental groups. In the first group (n = 32), animals receiv
