Bradykinin is involved in hyperalgesia induced by Bothrops jararaca venom.

Bradykinin is involved in hyperalgesia (pain hypersensitivity) induced by Bothrops jararaca venom-intraplantar injection of B. jararaca venom (5microg/paw) in rats caused hyperalgesia, which peaked 1h after venom injection. This phenomenon was not modified by promethazine (H(1) receptor antagonist), methysergide (5-HT receptor antagonist), guanethidine (sympathetic function inhibitor), anti-TNF-alpha or anti-interleukin-1 antibodies or by the chelating agent CaNa(2)EDTA. Venom-induced hyperalgesia was blocked by the bradykinin B(2) receptor antagonist HOE 140. On the other hand, des-Arg(9), [L

Effect of chloroquine on toxicity in mice of the venom and neurotoxins from the snake Bungarus multicinctus.

Antivenoms are the currently available agents for treatment of snake venom intoxication. The development of therapeutic strategies employing more generally available drugs could improve treatment of invenomation by reducing hypersensitivity reactions, cost and storage requirements. I report that chloroquine reduced the toxicity in mice of the venom from the Formosan krait, Bungarus multicinctus, when both venom and drug were administered i.p. When chloroquine was injected immediately after the injection of venom, the median lethal dose (LD50) of the venom was increased 5-fold from 19 to 95 mic

Adverse reactions to wine: think outside the bottle.

PURPOSE OF REVIEW: Wine contains chemical and biological contaminants. Symptoms such as facial flushing, asthma and oral allergic swelling and burning (oral syndrome) have been attributed to these contaminants and food additives. Their clinical implications should be known. RECENT FINDINGS: Recent studies have reported a high prevalence of hypersensitivity symptoms after intake of alcoholic drinks in the general population. Red wine was the most common beverage implicated. Wine contains many contaminants. Some of them come from Hymenoptera insects that fall into the wine when grapes are colle

[A case of unusual occupational allergy to bee venom].

The authors discuss an exceptional case of allergy to bee venom in a zoo-technician working on a morphological analysis of bees. During her work, the zoo-technician was repeatedly stung, initially with only small local reaction. After six-year period of work, the symptoms that followed stinging had all the characteristics of general oedema. Next years of exposure to bee venom resulted in collapse in the patient. Allergological examinations indicated work-related hypersensitivity to bee venom. The case history was supplemented with prophylactic recommendations. General principles of controlling

The use of the radioallergosorbent test in the diagnosis of Hymenoptera anaphylaxis.

IgE antibody levels to Hymenoptera (honey bee, hornet, wasp or yellow jacket) venom or venom sac were measured in the serum of ninety-six patients with a history of immediate hypersensitivity reactions to these insect stings. Normal levels of IgE antibody were found in fourty-four of these patients tested. The severity of the systemic reaction was similar in those patients with normal and elevated IgE antibody levels, and the interval from the systemic reaction to antibody determination was also similar. In twelve patients with a definite reaction to honey bee venom, the IgE antibody was eleva

Venomous Bites, Stings, and Poisoning: An Update.

This article discusses the epidemiology, prevention, clinical features, and treatment of venomous bites by snakes, lizards, and spiders; stings by fish, jellyfish, echinoderms, insects, and scorpions; and poisoning by ingestion of fish, turtles, and shellfish. Invertebrate stings cause fatalities by anaphylaxis, secondary to acquired hypersensitivity (Hymenoptera, such as bees, wasps, and ants; and jellyfish), and by direct envenoming (scorpions, spiders, jellyfish, and echinoderms). Simple preventive techniques, such as wearing protective clothing, using a flashlight at night, and excluding v

Systemic Reaction After Centipede Bite in a Child: Diagnostic Challenges Between Toxic and Hypersensitivity Mechanisms.

BACKGROUND Centipede (Scolopendra subspinipes) envenomation typically causes localized pain and swelling and is generally considered a self-limited condition. However, systemic reactions resembling hypersensitivity responses, such as generalized urticaria and respiratory discomfort, have occasionally been reported. Differentiation of toxic venom effects from immunologically mediated reactions remains challenging, particularly in pediatric patients, because standardized diagnostic testing for centipede venom allergy is unavailable. CASE REPORT A previously healthy 4-year-old girl developed gene

Epileptic attacks associated with wasp sting-induced anaphylaxis.

We report the case of a patient who suffered two epileptic attacks, each one associated with an anaphylactic shock due to wasp venom. He had no history of atopy nor any known diseases. Previous wasp stings had been well tolerated. An IgE-mediated hypersensitivity to the venom was shown both by cutaneous and immunologic tests. Specific IgE against Polistes spp. venom was 0.86 kU/I (RAST method), and intradermal tests with Polistes spp. venom were positive at a concentration of 1 microgram/ml. Cutaneous tests and serum specific IgE against Vespula spp. and Apis mellifera venoms were negative. In

Insect venom phospholipases A1 and A2: Roles in the envenoming process and allergy.

Insect venom phospholipases have been identified in nearly all clinically relevant social Hymenoptera, including bees, wasps and ants. Among other biological roles, during the envenoming process these enzymes cause the disruption of cellular membranes and induce hypersensitive reactions, including life threatening anaphylaxis. While phospholipase A2 (PLA2) is a predominant component of bee venoms, phospholipase A1 (PLA1) is highly abundant in wasps and ants. The pronounced prevalence of IgE-mediated reactivity to these allergens in sensitized patients emphasizes their important role as major e

Effect of chlorpromazine and quinacrine on the lethality in mice of the venoms and neurotoxins from several snakes.

Antivenoms are the currently available agents for the treatment of intoxication by snake venoms. Therapeutic approaches using more generally available drugs could improve treatment of envenomation by reducing the cost of the therapeutic agent, eliminating hypersensitivity reactions to antivenoms, and reducing storage costs. I investigated the efficacies of chlorpromazine and quinacrine with respect to reducing the lethality in mice of Bungarus caeruleus venom, Bungarus multicinctus venom and its neurotoxic components alpha- and beta-bungarotoxin, Crotalus durissus terrificus venom and its neur

Characteristics of Patients with Anaphylaxis to European Hornet (Vespa crabro) Venom Compared to Anaphylaxis to Wasp (Vespula spp.) Venom in Southern Germany.

INTRODUCTION: Systemic hypersensitivity to hymenoptera species venom is the most common cause for anaphylaxis in adults. Due to similarities between hornet (Vespa crabro) and wasp (Vespula spp.) venom, patients with hornet venom anaphylaxis are often treated similarly to wasp-allergic patients. However, comparative data are scarce. This study aimed to analyze differences and similarities between these two groups. METHODS: A retrospective analysis with 42 patients with certain anaphylaxis to hornet venom was conducted. These were matched for age, gender, and CAP class with 42 patients with was

Diagnosis and treatment of allergy to hymenoptera venoms.

PURPOSE OF REVIEW: This review overviews select advances reported in the literature from mid-2008 to the present in the diagnosis and management of individuals with immediate-type hypersensitivity to hymenoptera venom(s). RECENT FINDINGS: Skin testing remains the principal confirmatory test for sensitization to hymenoptera venoms. Use of dialyzed extracts in North America could enhance the sensitivity of intradermal skin testing. Hymenoptera venom components are overviewed with focus on their use in IgE antibody microarray assays to distinguish true dual sensitization to yellow jacket venom (

An economic evaluation of prophylactic self-injectable epinephrine to prevent fatalities in children with mild venom anaphylaxis.

BACKGROUND: Mild (cutaneous) venom anaphylaxis is the most common presentation of systemic venom hypersensitivity during childhood. Guidelines recommend prophylactic self-injectable epinephrine for children with mild venom anaphylaxis. However, progressive venom-associated reactions are uncommon in this population. OBJECTIVE: To characterize the cost-effectiveness of prophylactic self-injectable epinephrine in mild childhood venom anaphylaxis from a societal perspective. METHODS: Cohort simulations were used, and the base case was represented by a 6-year-old child with a history of mild veno

[Side effects of specific immunotherapy in patients with hymenoptera venom allergy].

The aim of this study was to estimate safety of specific immunotherapy to insects' venom. The frequency of side effects was compared in two groups of patients: subjects with hypersensitivity to insects' venom only and patients with allergy to other allergens. In the examined group of 32 patients with Hymenoptera venoms allergy skin prick tests with common aeroallergens were performed in 30 subjects. In 18 patients (60%) at least one test was positive. In 23 patients (71.9%) immunotherapy with Hymenoptera venom was connected with side effects observed just after administration of vaccine. They

Hymenoptera venom allergy in children.

From a taxonomic point of view, Hymenoptera are subclassified into families: Apidae, including honeybees (Apis mellifera) and bumblebees (Bombus), and Vespidae, which, in turn, are divided into the subfamilies of Vespinae (wasps, including hornets, vespules, dolichovespules) and Polistinae (paper wasp). Hypersensitivity to Hymenoptera venom can be linked to immunological (IgE-mediated or non-IgE-mediated) and non-immunological mechanisms. Reactions are classified into local reactions, large local reactions, systemic reactions, toxic reactions, and unusual reactions. In general, children sensit

Ant allergens and hypersensitivity reactions in response to ant stings.

Hypersensitivity reactions caused by ant stings are increasingly recognized as an important cause of death by anaphylaxis. Only some species of ants ( e.g. Solenopsis spp., Myrmecia spp., and Pachycondyla spp.) cause allergic reactions. Ant species are identified by evaluating the morphologic structures of worker ants or by molecular techniques. Ant venom contains substances, including acids and alkaloids, that cause toxic reactions, and those from Solenopsis invicta or the imported fire ant have been widely studied. Piperidine alkaloids and low protein contents can cause local reactions (ster

Precision Medicine in Hymenoptera Venom Allergy: Diagnostics, Biomarkers, and Therapy of Different Endotypes and Phenotypes.

Allergic reactions to stings of Hymenoptera species may be severe and are potentially fatal deviations of the immunological response observed in healthy individuals. However, venom-specific immunotherapy (VIT) is an immunomodulatory approach able to cure venom allergy in the majority of affected patients. An appropriate therapeutic intervention and the efficacy of VIT not only depend on a conclusive diagnosis, but might also be influenced by the patient-specific manifestation of the disease. As with other diseases, it should be borne in mind that there are different endotypes and phenotypes of

Anti-snake Venom-Induced Kounis Syndrome: A Unique Case in the Emergency Department.

Kounis syndrome is the concurrence of acute coronary syndromes associated with allergic or hypersensitivity and anaphylactic or anaphylactoid triggers. Although it is not a rare diagnosis, the different presentations and afflictions of all ages, sex, and racial groups make it a diagnostic challenge. Various triggers include food types, environmental exposure, and drugs. Cases triggered by serum sickness, tetanus antitoxin, and snake bites have been documented in the literature. However, to the best of our knowledge, no case triggered by anti-snake venom (ASV) has been reported yet, as seen in
