BACKGROUND: Breech presentation occurs in approximately 4% of term pregnancies. Recently the American College of Obstetrics and Gynecology has suggested that cesarean section is the safest option if the fetus remains in breech position. As an alternative to cesarean section, external cephalic version has been used prior to labor and even recently in the patient with rupture of membranes not in labor. We present two cases found at our institution from 1990 through 2001, who at term presented in early labor with spontaneous rupture of membranes and underwent successful external cephalic version.

The purpose of this study was to examine the efficacy of remifentanil on external cephalic version (ECV) in breech presentation. An extensive search was conducted using PubMed, Cochrane Library, and other grey literature. Only randomized controlled trials using remifentanil for ECV were included. Risk ratio (RR) and mean difference (MD) were used to estimate outcomes and quality of evidence was assessed using the Risk of Bias and GRADE system. We report a novel application of intrapartum sonography, herein used to assist the internal podalic version and the vaginal delivery of a transverse-lying second twin. Following the vaginal delivery of the first cephalic twin, the internal podalic version was performed under continuous ultrasound vision, leading to the uncomplicated breech delivery of a healthy neonate.

A 36-year-old multiparous woman with fetus in the breech position applied ice to the fundus of the uterus and achieved successful cephalic version. No other reports of using ice to induce cephalic version are found with MEDLINE search; however, it has been used as a folk remedy. The effectiveness of external cephalic version with tocolysis when routinely used in the 37th week of gestation is reported. The procedure had earlier been ruled out in our department, was found effective in a prospective study, and afterwards settled as a routine. Among 1038 women with single breech presentation, 882 could be offered an attempt of external cephalic version during the period 1982-1988. Attempt of version was carried out in 316 women resulting in a vertex presentation at delivery in 100. The success rate on average was 35%. The estimated reduction of breech deliveries was 100/1
