OBJECTIVE: To determine if cervical vestibular evoked myogenic potentials (cVEMPS) differ in patients with migraine without aura (MoA), vestibular migraine (VM) and tension type headache (TTH).
METHODS: Twenty patients with MoA, 24 patients with VM and 20 patients with TTH were included in the study. Thirty healthy volunteers of comparable age and gender were taken as the control group. The latencies of peaks p13 and n23, peak-to-peak amplitude of p13-n23 divided by a mean prestimulus EMG recorded during cVEMP testing were measured. The amplitude asymmetry between right and left sides was als…

BACKGROUND: A functional vestibular system is a prerequisite for motion sickness (MS) to occur. The otolithic organs (saccule and utricle) are currently considered to be important in the pathogenesis of MS. Recent investigations have demonstrated that the ocular vestibular evoked myogenic potentials (oVEMP) test is an objective evaluation of utricular function. To date, there is no functional guide that can serve as a diagnostic tool for individual susceptibility to MS. Therefore, we investigated the link between oVEMP and MS susceptibility.
METHOD: The oVEMP test was performed on 31 MS susce…

Vestibular evoked myogenic potentials (VEMPs) were measured in 22 unilateral Menière patients with monaural and binaural stimulation with 250 and 500 Hz tone bursts. For all measurement situations significantly lower VEMP amplitudes were on average measured at the affected side compared to the unaffected side. Unilateral Menière patients have, in contrast to normal subjects, asymmetric VEMPs, indicating a permanently affected vestibular (most likely otolith) system at the side of hearing loss. The diagnostic value of VEMP amplitude asymmetry measurement in individual patients is low, because o…

Vestibular dysfunction has been reported in patients with cervical dystonia (CD), but it is still unclear whether the abnormalities occur as part of the CD syndrome or whether they arise from the abnormal posture and movement of the head. We compared vestibular-evoked myogenic potentials (VEMPs) recorded from the affected neck muscles (i.e., cervical VEMPs) with those recorded from muscles unaffected by the dystonia, the extraocular muscles (i.e., ocular VEMPs). We compared 21 patients and age-matched normal controls to investigate whether these short-latency reflexes are altered in CD. We als

Testing vestibular evoked myogenic potentials (VEMPs) may be the most important new clinical test for evaluation of vestibular function developed during the past 100 years since the introduction of the caloric test. VEMPs are easily recordable and therefore suitable for everyday testing in clinical neurotology. VEMPs in response to air-conducted sound stimulation using surface electrodes over the sternocleidomastoid muscles reveal saccular function, inferior vestibular nerve function, and vestibulocollic connections. At present, VEMPs are of clinical importance for estimating the severity of p…

OBJECTIVES: To clarify if p13-n23 of vestibular evoked myogenic potentials (VEMPs) is independent of cochlear afferents.
METHODS: Twenty normal volunteers and 10 patients with hemifacial palsy enrolled into this study. VEMP and auditory brainstem responses (ABRs) were recorded with or without white noise (WN, 75 dBnHL or 95 dBnHL) ipsilaterally or contralaterally to the stimulated ear. Short tone bursts (STBs) of 0.5 kHz (95 dBnHL, rise/fall time=1 ms, plateau time=2 ms) were presented though headphones. For recording VEMPs surface electromyographic activity was recorded in the supine subject…

The aim of this study is to investigate the feasibility of recording vestibular evoked myogenic potential (VEMP) using simultaneous binaural acoustic stimulation (B-VEMP), and compare it with that using monaural acoustic stimulation (M-VEMP). Seven healthy volunteers were evoked by initial B-VEMP test and subsequent M-VEMP test, whereas vice versa in another 7 volunteers. All 14 subjects demonstrated both B-VEMPs and M-VEMPs, without significant difference in the latencies of p13 and n23. When using interaural amplitude difference (IAD) ratio for interpreting amplitude, B-VEMPs did not differ

OBJECTIVES: to show that abnormal vestibular evoked myogenic potentials on the sternocleidomastoid muscle (SCM) in patients with unilateral Meniere's disease are caused by endolymphatic hydrops.
SUBJECTS: six normal volunteers and 17 patients with unilateral Meniere's disease were examined.
METHODS: click-evoked myogenic potentials were recorded with surface electrodes over each SCM. Responses evoked by clicks recorded after oral administration of glycerol (1.3 g/kg body weight) were compared with those recorded before administration.
RESULTS: the change rate of the p13-n23 amplitude was ca…

We report a case of vestibular schwannoma with absent vestibular evoked myogenic potentials (VEMPs) to clicks but normal auditory brainstem responses, caloric responses, and vestibular evoked myogenic potentials to 500 Hz tone bursts. This patient visited our clinic with complaint of sudden right hearing loss. This was the third episode of hearing loss in his right ear. Due to atypical medical history, he underwent further neuro-otological and neuroradiological examinations. Among the neurophysiological tests, only VEMPs to clicks showed abnormal findings (absent responses on the right). MRI r…

Intravestibular intralabyrinthine schwannomas (ILSs) are uncommon benign tumors that arise from the saccular, utricular, and lateral and superior ampullary nerves. According to the literature, there is an average delay of 8 years between the onset of symptoms and diagnosis. The diagnosis is based on an audiovestibular examination and magnetic resonance imaging (MRI). We describe a case of intravestibular ILS in which we included the ocular vestibular evoked myogenic potentials (oVEMPs) test in the diagnostic workup. The oVEMPs test is a relatively new neurophysiologic diagnostic modality that …

Ankylosing spondylitis (AS) is a chronic systemic inflammatory disease with unknown etiology. Although sacroiliac joint involvement is the classic sign along with the formed immune mediators, it may result in immune-mediated inner ear disease and may cause damage to the audiovestibular system. Vestibular evoked myogenic potentials (VEMP) is a clinical reflex test used in the diagnosis of vestibular diseases and is performed by recording and evaluating the muscle potentials resulting from the stimulation of the vestibular system with different stimuli. The aim of this study is to evaluate the c…

Air conducted vestibular evoked myogenic potentials (VEMP) can be elicited by various low frequency and intense sound stimuli, mainly clicks or short tone bursts (STB). Chirp stimuli are increasingly used in diagnostic audiological evaluations as an effective means to obtain acoustically evoked responses in narrowed or extended frequency ranges. We hypothesized in this study that band limited chirp stimulation, which covers the main sensitivity range of sound sensitive otolithic afferents (around 500 Hz), might be useful for application in cervical and ocular VEMP to air conduction. For this p…

Over the last years, vestibular-evoked myogenic potentials (VEMPs) have been established as clinical tests of otolith function. Complementary to the cervical VEMPs, which assess mainly saccular function, ocular VEMPs (oVEMPs) test predominantly utricular otolith function. oVEMPs are elicited either with air-conducted (AC) sound or bone-conducted (BC) skull vibration and are recorded from beneath the eyes during up-gaze. They assess the vestibulo-ocular reflex and are a crossed excitatory response originating from the inferior oblique eye muscle. Enlarged oVEMPs have proven to be sensitive for

INTRODUCTION: Otolith function, which is dependent on linear velocity and acceleration, may be expected to change in underwater divers, who are submerged in a medium that is denser than air. The purpose of the present study was to examine possible changes in the sacculocollic reflex of professional divers and to investigate whether there might be diving-induced adaptation of the saccular response.
METHODS: We used the vestibular evoked myogenic potential (VEMP) response to evaluate saccular function in 12 professional divers shortly after a dive and after an interval of at least 24 h. The con

By easily stimulating the ear with loud sound and recording on tonically contracted neck muscles, vestibular evoked myogenic potential (VEMP) test can reflect inner ear function other than the cochlea and semicircular canal. This expands the test battery for clinicians to explore saccular disease, adding a potential usefulness to the sacculo-collic reflex. The ideal stimulation mode for VEMPs is as follows: 95 dB tone bursts, frequency 500 Hz, stimulation repetition rate 5 Hz, rise/fall time 1 ms, plateau 2 ms, binaural stimulation with bilateral recordings. Animal model using guinea pigs has
