Vestibular nerve findings in 150 neurectomized patients.

Vestibular nerve specimens obtained at neurectomy were studied by light and electron microscopy in 150 patients with various neuro-otological disorders. The vestibular nerve was considered morphologically normal in only 20% of the cases but the changes observed in most cases were interpreted as artifacts or physiological changes of aging. In nearly half of the cases there was excessive fibrosis of the perineurium or endoneurium of the vestibular nerve, or occurrence of glial tissue within or near Scarpa's ganglion. When these changes are of moderate or severe degree, they may be of pathologica

Cochlear implantation after selective vestibular nerve section.

INTRODUCTION: Vestibular nerve section is a highly effective procedure for the control of vertigo in patients with Ménière's disease. However, hearing loss is a possible complication. If hearing loss occurs after vestibular nerve section, magnetic resonance imaging should make it possible to establish the presence or absence of an intact cochlear nerve. METHOD: Case report and review of the world literature concerning cochlear implantation after vestibular nerve section. CASE REPORT: We present a patient who developed subtotal hearing loss after vestibular nerve section. Magnetic resonance i

Facial nerve function after vestibular schwannoma surgery.

OBJECT: The object of this study was to evaluate long-term postoperative facial nerve (FN) function in patients undergoing vestibular schwannoma (VS) surgery. METHODS: The authors retrospectively reviewed the clinical course of patients affected by isolated VSs with normal preoperative FN function, with no previous surgical or radiotherapeutic treatment, and who underwent surgery between 1987 and 2007. Facial nerve function was clinically evaluated according to the House-Brackmann (HB) scale. The minimum postoperative follow-up was 12 months. RESULTS: Among the 1550 patients surgically treat

Long-term results of vestibular nerve section.

Records are available on 27 patients who had vestibular nerve section between 1975 and 1987 giving a follow-up time of 10 to 22 years with a mean of 16 years. Episodic vertigo was fully controlled in 26 patients, one needing a labyrinthectomy 18 months later. In addition to the patient who had labyrinthectomy, one patient had immediate post-operative profound hearing loss so that long-term follow-up of hearing was possible in 25 patients. After six months there was an overall average improvement in hearing of 1 dB. Thereafter there were average deterioration of 7 dB at two years, 15 dB at 10 y

Retrolabyrinthine vestibular nerve section: a current appraisal.

We report our experience (1987-1993) with Meniere's disease patients treated with a retrolabyrinthine vestibular neurectomy. The current literature was reviewed and our results have been compared with those of previous reports. The overall success rate for vertigo relief was 96.7%, with no serious or permanent complications resulting from the procedure. The technical elements of the operation, as they apply to our approach and those of others, have been analyzed, with special attention given to the anatomical features of the region and their influence on success or failure. We conclude that th

Headache prevention in retrosigmoid vestibular nerve section.

Most disorders of the inner ear resulting in vertigo can be managed medically with good results. Meniere's disease, one of the more common causes of recurring vertigo, does not respond adequately to medical management in 5% of the cases. These patients become candidates for surgical management. Selective vestibular nerve section has become the procedure of choice when one desires to preserve residual hearing. This paper reviews 22 cases of vestibular nerve section via a retrosigmoid approach. The disease entities treated, treatment results, operative morbidity, and complications are tabulated.

VIIIth nerve vascular compression syndrome: vestibular paroxysmia.

Neurovascular cross-compression of the root entry zone of the Vth, VIIth and IXth cranial nerves causes symptoms of trigeminal neuralgia, hemifacial spasm and glossopharyngeal neuralgia. It is reasonable to search for a group of patients presenting with typical paroxysmal vestibular and/or cochlear symptoms, analogously caused by neurovascular compression of the VIIIth cranial nerve. Since no pathognomonic sign or test has yet been established, the diagnosis of 'vestibular paroxysmia' secondary to neurovascular cross-compression is based on four characteristic features: (1) short attacks of ro

Retrolabyrinthine vestibular nerve section: analysis of results.

The retrolabyrinthine section of the vestibular nerve is an effective procedure for the ablation of unilateral vestibular function while preserving hearing. Although this procedure has been most frequently used for the control of unilateral Meniere's disease, it may also be effective in the control of unilateral vestibular dysfunction of other etiologies. A 2-year experience with the indications for and the surgical results in a series of patients who underwent this procedure is presented. Brief case histories of a variety of etiologies of unilateral vestibular dysfunction are discussed.

Effects of unilateral vestibular ganglionectomy on glutaminase activity in the vestibular nerve root and vestibular nuclear complex of the rat.

The metabolism of glutamate, the most likely neurotransmitter of vestibular ganglion cells, includes synthesis from glutamine by the enzyme glutaminase. We used microdissection combined with a fluorometric assay to measure glutaminase activity in the vestibular nerve root and nuclei of rats with unilateral vestibular ganglionectomy. Glutaminase activity in the lesioned-side vestibular nerve root decreased by 62% at 4 days after ganglionectomy and remained at similar values through 30 days. No change occurred in the contralateral vestibular nerve root. Glutaminase activity changes in the vestib

[Imaging of the labyrinth and vestibular nerve. Clinical significance for differential diagnosis of vestibular diseases].

High resolution magnetic resonance imaging (HR-MRI) and computed tomography (HR-CT) of the inner ear are becoming more important for the diagnosis of peripheral vestibular lesions. Modern HR-MRI techniques allow visualization of detailed anatomic features of the vestibulo-cochlear regions as well as pathologic findings in the inner ear such as, neoplastic lesions (e.g., small intracanalicular acoustic neuromas), anomalies causing vertigo and hearing loss (e.g. Mondini's-malformation, perilymph fistula, vestibular paroxysmia), and inflammatory diseases (e.g., Cogan's syndrome, labyrinthitis, zo

Growth of postoperative remnants of unilateral vestibular nerve schwannoma: role of the vestibular ganglion.

Histopathological examination of seven temporal bones from patients who underwent a removal of vestibular nerve schwannomas by the translabyrithine or middle fossa approaches has demonstrated small tumor remnants that failed to grow as long as 25 years after surgery. In spite of the high incidence of residual tumors, the clinical recurrence rate of tumors operated at our institution by the translabyrinthine or middle fossa approaches is low (0.3%). Immunohistochemical labeling of dividing cells demonstrated that segments of tumor adjacent to the vestibular nerve and ganglion contained more div

The morphology of the vestibular nerve in a patient with normal vestibular function and in patients with Ménière's disease.

The morphology of the vestibular nerve was studied in three fresh nerve specimens obtained by surgery from two patients with Ménière's disease and one patient with tinnitus after an attack of sudden deafness. The number of nerve fibres appeared light microscopically normal. The vestibular ganglion cells of the Ménière-specimens showed, as a characteristic finding advanced vesiculation of the cytoplasm. This change was confirmed by electron microscopy and the vacuoles appeared to consist of widened endoplasmic reticulum cisternae. Numerous primary lysosomes and lipofuscin granules were observed

Preservation of tap vestibular evoked myogenic potentials despite resection of the inferior vestibular nerve.

Sound and skull-tap induced vestibular evoked myogenic potentials (VEMP) were studied in a 43-year-old man following inferior vestibular neurectomy. Surgery was performed because of a small acoustic neuroma. Postoperative caloric testing suggested sparing of superior vestibular nerve function on the operated side. In response to sound stimulation there were no VEMP on the operated side, irrespective of whether sounds were presented by air- or bone-conduction. This suggests sound-induced VEMP to be critically dependent on inferior vestibular nerve function and this is in agreement with present

Comparison of postoperative headache after retrosigmoid approach: vestibular nerve section versus vestibular schwannoma resection.

OBJECTIVE: To evaluate intradural drilling as a mechanism for the development of postoperative headache after retrosigmoid craniectomy. STUDY DESIGN: A retrospective review of charts was performed on 565 retrosigmoid approaches to the cerebellopontine angle performed between January 1980 and January 1998. Patients treated with retrosigmoid vestibular nerve section without intradural drilling were compared with patients who underwent retrosigmoid removal of vestibular schwannomas in which intradural drilling was performed for exposure of the internal auditory canal. SETTING: Private practice

Time course of vestibular function in patients with menière's disease following vestibular nerve section.

Harmonic acceleration rotational test and vestibular autorotation test (VAT) were performed on 8 patients with Menière's disease before and after vestibular nerve section. Sinusoidal rotations of 0.01-0.64 Hz were used in the HA rotational test. VAT was performed at frequencies between 0.5 and 6 Hz. Two weeks after operation, almost all the patients consistently showed response asymmetry toward the healthy ear at all stimulus frequencies and a reduction in sensitivity to rotational stimuli at 0.01 Hz in the HA rotational test. Prior to surgery, the time constant of the vestibulo-ocular reflex

Cochleovestibular nerve compression syndrome. II. Vestibular nerve histopathology and theory of pathophysiology.

The present study was undertaken to systematically examine and characterize pathological changes in vestibular nerve specimens obtained at surgery in patients with symptomatic cochleovestibular nerve compression syndrome (CNCS). Vestibular nerves were obtained in six cases of CNCS and were intermingled with vestibular nerves obtained in cases of Meniere's disease. All of the nerve specimens were coded and reviewed microscopically in a blind-study fashion by the neuropathologist. The vestibular nerves obtained from CNCS cases showed significant endoneurial fibrosis, compared to controls (specim
