Chronic pain treatment and health service utilization of veterans with hepatitis C virus infection.

OBJECTIVES: Hepatitis C virus (HCV) infection is estimated to affect 2% of the general U.S. population and chronic pain is a common comorbidity among persons with HCV. The primary purpose of this study was to compare health service utilization of U.S. military veterans with HCV with and without the presence of comorbid chronic pain. DESIGN: Cross-sectional study with retrospective review of patient medical records. PATIENTS: One hundred seventy-one U.S. military veterans with confirmed HCV, recruited through a single U.S. Veterans Administration hospital. OUTCOME MEASURES: Medical service u

The association of disordered eating and sexual health with relationship satisfaction in female service members/veterans.

Low relationship satisfaction is associated with mental health disorders in service members/veterans (SM/Vs), yet eating disordered behavior (EDB) and sexual function and satisfaction in SM/Vs are understudied. Those with EDB may experience bodily discomfort that may be associated with low relationship satisfaction because of avoidance of physical contact and intimacy, suggesting that sexual satisfaction and function may modify the association of EDB and relationship satisfaction. As the majority of female SM/Vs are partnered, it is imperative to study the association of sexual function and sa

Motor neurone disease and military service: evidence from the Scottish Veterans Health Study.

OBJECTIVES: In 2003, it was reported that motor neurone disease was linked to military service in the 1990-1991 Gulf War. A large study in the US confirmed an association with military service but found no association with specific conflicts or length of service. Non-veteran studies have suggested an association with physical activity, smoking and other risk factors. We used data from the Scottish Veterans Health Study to investigate the association between motor neurone disease and military service in UK veterans. METHODS: Retrospective cohort study of 57,000 veterans born 1945-1985, and 173

Unmet Need for Outpatient Occupational Therapy Services Among Veterans With Mild Traumatic Brain Injury in the Veterans Health Administration: The Role of Facility Characteristics.

OBJECTIVE: To investigate whether participation restrictions, an indicator of need for occupational therapy (OT), was associated with outpatient OT utilization in the Veterans Health Administration (VHA) among Veterans with mild traumatic brain injury (mTBI), and whether this relation differs by facility characteristics. DESIGN: In a secondary analysis of national VHA data, we used modified Poisson regression to model OT utilization (yes/no) as a function of participation restrictions (Mayo-Portland Adaptability Inventory Participation Index [M2PI]), facility characteristics, and covariates.

Inclusion of Veterans Health Administration hospitals in Centers for Medicare & Medicaid Services Overall Hospital Quality Star Ratings.

BACKGROUND/OBJECTIVE: The Centers for Medicare & Medicaid Services (CMS) Overall Hospital Quality Star Rating, established in 2016, is a summary of publicly available quality information for acute care hospitals. In July 2023, Veterans Health Administration (VHA) hospitals became eligible to receive a CMS Overall Hospital Quality Star Rating for the first time. Our objective was to compare performance in quality ratings among VHA and non-VHA hospitals. METHODS: We used the hospital quality measure scores posted to Care Compare on Medicare.gov as of January 2023 as our primary data set. We con

The effect of military sexual trauma on competitive employment status following veterans health administration supported employment services.

Military sexual trauma (MST) is prevalent, and some initial evidence suggests it is associated with impaired employment outcomes. The Veterans Health Administration (VHA) offers supported employment (SE) and Community-Based Employment Services (CBES), which consist of individualized employment support integrated with mental health treatment. The objective of this study was to examine the prevalence and effect of MST on competitive employment outcomes following participation in SE or CBES. Participants were a national set of Veterans who participated in VHA SE or CBES services between 2015 and

Tobacco and nicotine cessation in military service members and veterans: implications for area health education centers.

Area Health Education Centers (AHECs) have played a longstanding role in strengthening the United States (U.S.) health care workforce through education and training and expanding access to care services in rural and underserved communities. As tobacco and nicotine use emerged as a major preventable cause of morbidity and mortality decades ago, AHECs played a key role in the integration of evidence-based cessation education into health professions training and community-based programming. Tobacco and nicotine use remains disproportionately high among U.S. military service members and veterans,

Predictors of High-Intensity Psychiatric Service Utilization by Veterans Health Administration Patients With Early Psychosis.

OBJECTIVE: People with early episode psychosis (EEP) have more negative care outcomes than do people with later episode psychosis (LEP), including higher levels of high-intensity psychiatric service use. It is unclear whether these differences are best explained by clinical differences between these two groups or whether people with EEP have specific treatment needs. An assessment of the treatment needs of patients with EEP can help inform the implementation of national treatment programming designed to provide better care to this group. METHODS: Administrative data were used to compare chara

Leveraging Natural Language Processing to Identify Veterans Who Inject Drugs to Assess Preexposure Prophylaxis and Sexually Transmitted Infection Testing Services at the Veterans Health Administration.

BACKGROUND: People who inject drugs (PWID) face disproportionate risks for infectious diseases yet remain difficult to identify within health care systems. Natural language processing (NLP) offers potential solutions for identifying PWID to improve access to harm reduction services. METHODS: We evaluated an NLP dashboard designed to identify Veterans with evidence of injection drug use across 6 Veterans Health Administration facilities between August and October 2024. Four independent reviewers assessed electronic health records to confirm recent injection drug use and evaluated preventive ca

Ten Years of Outcomes in a National Mental Health Network for Veterans, Service Members, and Military Families.

Military-affiliated adults including veterans, active-duty service members, and their families experience elevated rates of mental health conditions such as PTSD, depression, and anxiety. Addressing these needs requires accessible, high-quality, and evidence-based care delivered at scale. Cohen Veterans Network (CVN), a national network of community-based military family clinics, was established in 2015 to provide rapid access to mental healthcare, emphasizing evidence-based psychotherapies, measurement-based care, and standardized clinical and operational practices. This retrospective study e

Cultural and service factors related to mental health beliefs among post-9/11 veterans.

BACKGROUND AND AIMS: The primary purpose of this study was to examine cultural and demographic predictors of dimensions of anticipated stigma and negative mental health beliefs, known barriers to mental health care utilization, among post-9/11 veterans. METHODS: A cross-sectional survey methodology was used to assess 132 post-9/11 veterans. Bivariate correlations were used to examine associations between military cultural values (self-reliance and emotional control), demographic variables (gender and service characteristics) and dimensions of anticipated stigma and negative beliefs about ment

Long-term effects of military service on mental health among veterans of the Vietnam War era.

Comparing outcomes of veterans who served in Vietnam and those who served elsewhere, we examined treatment of post-traumatic stress disorder, treatment of other mental health conditions, psychiatric treatment location, and six mental health well-being measures. The analytic sample consisted of nationally representative data from the 2001 National Survey of Veterans. Analyses included multivariate logistic regression that controlled for sociodemographic characteristics. Of Vietnam War-era veterans in the National Survey of Veterans (N = 7,914), 3,937 served in Vietnam and 3,977 served elsewhere

Comparing the Utilization and Cost of Health Services between Veterans Experiencing Brief and Ongoing Episodes of Housing Instability.

Housing instability is associated with costly patterns of health and behavioral health service use. However, little prior research has examined patterns of service use associated with higher costs among those experiencing ongoing housing instability. To address this gap, we compared inpatient and outpatient medical and behavioral health service utilization and costs between veterans experiencing brief and ongoing episodes of housing instability. We used data from a brief screening instrument for homelessness and housing instability that has been implemented throughout the US Department of Vete

The Role of Military Sexual Trauma History and Frequency of Use of Veterans Health Administration Homeless and Justice Program Services Among Male and Female Homeless Veterans.

Military sexual trauma (MST) is a prevalent experience associated with deleterious chronic health and psychiatric conditions. The Department of Veterans Affairs (VA) offers MST-related care free of charge; however, MST history can impact perceptions of trust and safety, affecting willingness to access such care. MST is also more prevalent among Veterans with a history of homelessness and criminal legal involvement. These Veterans are often clinically complex and may benefit from higher rates of VA service use to meet their health and social care needs. No research to date has examined the pote

Association Between Use of Services To Address Adverse Social Determinants of Health and Documented Suicide Attempt Among Patients in the Veterans Health Administration.

Suicide prevention is a top priority for the US Department of Veterans Affairs (VA), and suicide is often associated with adverse social factors (e.g., financial, legal, and housing problems). The VA provides social services integrated with healthcare services, which may increase the opportunities to detect and document suicide attempt in EHR records. Using VA administrative data, we examined three cohorts of all patients from 2014 to 2018 who had housing instability (n = 659,987), justice involvement (n = 200,487), and unemployment (n = 346,556).
