Natural history and treatment of varicella-zoster in high-risk populations.

Rigorous clinical trials have established that both acyclovir and vidarabine favourably alter the clinical course of herpes zoster and chicken-pox in immunocompromised patients. In one comparative study, acyclovir was shown to be superior to vidarabine for zoster in bone marrow transplant recipients. These data, plus the fact that acyclovir is easier to administer than vidarabine, and perhaps less toxic, have made intravenous acyclovir the recognized drug of choice for treatment of herpes zoster in immunocompromised patients. Acyclovir sodium sterile powder received Federal Drug Administration

Kaposi's varicelliform eruption in mycosis fungoides.

Kaposi's varicelliform eruption, caused by herpes simplex type 1, developed in a patient with mycosis fungoides four days after initiation of photochemotherapy with methoxsalen plus long-wave ultraviolet light (PUVA). He had a history of recurrent localized herpes infection. Vidarabine (adenine arabinoside) was used as treatment. Rechallenge with PUVA did not provoke another herpes infection. Kaposi's varicelliform eruption is rare in mycosis fungoides. Vidarabine may prove to be useful in the treatment of widespread herpes simplex.

Comparative use of adenine arabinoside and adenine arabinoside monophosphate in varicella and disseminated zoster in immunosuppressed patients.

Adenine arabinoside monophosphate (ARA AMP) was used in ten immunosuppressed patients suffering from varicella or disseminated zoster, and compared to adenine arabinoside (vidarabine) in ten other similar patients. Good results were obtained with both drugs: the fever stopped suddenly in three or four days and no new vesicles were observed after the third day. ARA AMP is more soluble than vidarabine and is thus easier to handle; it could be an effective treatment of severe varicella-zoster virus infections in immunosuppressed patients.

[Periarteritis nodosa related to hepatitis B virus. Determination of a new therapeutic strategy: 13 cases].

The treatment and prognosis of periarteritis nodosa associated with hepatitis B virus were reconsidered from a series of 13 patients representing 32.5 per cent of the 40 patients with periarteritis nodosa admitted during the same period. HBs and HBe antigens were present in every case, and hepatitis B virus replication was demonstrated by the finding of viral DNA in serum. One patient had anti-HBc IgM's. Five patients were treated with corticosteroids, cyclophosphamide and occasional plasma exchanges. All were cured or achieved complete remission. Eight patients were treated with plasma exchan

Trifluridine: a review of its antiviral activity and therapeutic use in the topical treatment of viral eye infections.

Trifluridine (trifluorothymidine) is an antiviral agent for topical use in the eye, and is structurally related to idoxuridine. In vitro studies have shown that it effectively inhibits the replication of herpes simplex virus type 1, which causes primary keratoconjunctivitis and recurrent epithelial keratitis in man. In masked comparative studies, predominantly in patients with dendritic ulcers, trifluridine 1% solution was effective in over 90% of patients; in such studies it was comparable with vidarabine in treating dendritic ulcers, and was at least as effective as, and in some studies more

Acylovir in oral and ganglionic herpes simplex virus infections.

The local and trigeminal ganglionic therapeutic efficacy of two topical and systemic antiviral drugs was studied in mouse lips inoculated with herpes simplex virus type 1 after thermal injury. Application of topical 3% acylovir (acycloguanosine) ointment three times daily for four days completely blocked the replication of virus in the lips, and the healing process was greatly accelerated compared with that in placebo-treated infected controls. However, neither the healing process nor the viral replication was influenced by similar therapy with 3% vidarabine ointment. When given systemically f

Antiviral drugs 1983.

A number of antiviral compounds are currently available, and several others are of great interest. Trifluridine, idoxuridine, and vidarabine are effective topically in herpes simplex virus keratoconjunctivitis infection. Vidarabine (and presumably acyclovir) is effective in herpes simplex virus encephalitis and in herpes zoster infections in the immunocompromised host. Acyclovir is effective topically, orally, and intravenously in primary herpes genitalis, and the oral and intravenous forms are effective in recurrent herpes genitalis as well. Amantadine and rimantadine are effective prophylact

Polyarteritis nodosa related to hepatitis B virus. A retrospective study of 66 patients.

In an attempt to establish the characteristics, circumstances leading to infection and development of polyarteritis nodosa (PN) related to hepatitis B virus (HBV), prognostic factors and outcome, and to define the most effective treatment, 66 patients observed between 1972 and 1989 were analyzed. Hepatitis was clinically present in 19/66 patients before PN. In most cases, PN occurred less than 6 months after infection. Clinical manifestations of PN were comparable to those observed in patients without HBV infection except for orchitis which was present in 13.6% and for pulmonary signs which we

CNS diseases associated with varicella zoster virus and herpes simplex virus infection. Pathogenesis and current therapy.

In recent years, herpes simplex virus has been recognized as an important CNS pathogen in neonates and adults. The recent development of effective antiviral therapy has substantially reduced the excessive morbidity and mortality associated with these infections. For neonatal herpes simplex infections, the current drug of choice is vidarabine. The results of ongoing clinical trials comparing vidarabine with acyclovir in neonatal herpes may lead to a change in the recommended therapy. In the adult, the therapy of choice for herpes simplex encephalitis is acyclovir. Although effective, the presen

Acyclovir therapy of varicella-zoster virus infections in immunocompromised patients.

In a randomized, placebo-controlled, double-blind trial of intravenous acyclovir in the treatment of varicella zoster virus (VZV) infections, 8 of 20 immunocompromised children with varicella received acyclovir (500 mg/m2/dose three times daily for 7 days). There was no significant difference in skin healing between the acyclovir and placebo groups although there was a significant reduction in the incidence of development of pulmonary involvement during acyclovir treatment. Nineteen out of 34 patients received vidarabine (10 mg/kg/day for 5 days). Vidarabine significantly shortened the duratio

Syndrome of inappropriate antidiuretic hormone secretion and herpes zoster infection: 1. Report of this association in a patient suffering from AIDS.

The syndrome of inappropriate secretion of antidiuretic hormone is a common consequence of neurologic and pulmonary infections as well as drug intake and many other clinical situations. Its association with herpes varicella-zoster virus infections is scarcely reported in the literature. It generally appears in immunosuppressed patients suffering from serious underlying diseases. There are also a few cases of syndrome of inappropriate secretion of antidiuretic hormone related to vidarabine use. We report the case of a man infected by human immunodeficiency virus who developed a disseminated her

[Prognosis and treatment of chronic viral hepatitis].

Prognosis of chronic viral hepatitis can be severe. One have to keep in mind the possibility of cirrhosis and hepatocellular carcinoma. Thanks to anatomopathological and immunological tests, it is possible nowadays to evaluate the extent of chronic viral hepatitis and its prognosis. The most efficient therapies are actually the antiviral drugs (vidarabine, interferon): against chronic viral hepatitis B: vidarabine or interferon preceded by a cure of corticoids lead to about 35% of H Be seroconversion--against chronic viral hepatitis B-Delta: interferon Has a suppressive effect in about 50% of

Antiviral agents for treatment of herpes simplex virus infection in neonates.

BACKGROUND: Herpes simplex virus (HSV) is a rare but serious neonatal pathogen. Prior to the availability of antiviral drugs the mortality associated with all but localised neonatal infection was high, with 85% of infants with disseminated HSV infection and 50% of infants with encephalitis dying by one year of age. The morbidity in the survivors of multiorgan infection was also high, with up to 50% experiencing long-term neurological sequelae. OBJECTIVES: To determine the effect of antiviral agents in the treatment of neonatal HSV infections on mortality, progression of disease and neurodevel

Evaluation of the antiherpetic activity of acyclovir in rabbits.

Acyclovir (3 percent ointment) used topically one to five times a day on acute ocular herpes simplex virus (HSV) infection gave beneficial results as measured by a reduction in corneal involvement, conjunctivitis, iritis, and corneal clouding. Early intensive acyclovir topical treatment every two hours beginning 24 hours after inoculation prevented all but conjunctivitis. Topical treatments did not prevent the establishment of latent HSV infection. Effects of topical acyclovir treatment on rabbit eyes infected with parent McKrae, idoxuridine-resistant, and vidarabine-resistant strains of HSV-1

Hepatitis C virus in patients with polyarteritis nodosa. Prevalence in 38 patients.

In order to assess the prevalence of hepatitis C virus (HCV) in polyarteritis nodosa (PN), 38 patients with systemic necrotizing angiitis were retrospectively tested for the presence of anti-HCV antibodies (Ab). Twenty-one patients were hepatitis B virus (HBV) positive, comprising group A, and 17 were HBV negative, comprising group B. Two patients from group A had anti-HCV Ab (2/21: 9.5%). One was treated unsuccessfully with corticosteroids, then with vidarabine and plasma exchanges; HBe/anti-HBe seroconversion was not observed and anti-HCV Ab disappeared 8 months after the onset of PN. A novel corneal epithelial cell culture assay for assessing topical antivirals against herpes simplex virus keratitis.

Various in vitro models have been used to study HSV infection, but corneal epithelial-based assay systems for evaluating antiviral activity remain limited. In this study, we established a in vitro assay of HSV keratitis using the CRL-11516, an immortalized human corneal epithelial (HCE) cell line. HSV readily infected HCE cells and exerted cytopathic effects. These effects were quantified via formazan-based colorimetric assay, which indicated the suitability of our system for antiviral evaluation. Most clinically available antivirals exhibited sufficient activity in our model. Specifically, ac

Antiviral treatment in chickenpox and herpes zoster.

Intravenous acyclovir is effective for varicella in adults and immunocompromised children, causing more rapid resolution of the illness and fewer complications. Intravenous acyclovir in immunocompromised patients with herpes zoster decreases new lesion formation, decreases acute pain, halts dissemination of the virus, and lessens visceral complications. Intravenous acyclovir may also be effective in zoster encephalitis. Intravenous vidarabine also has a favorable affect on chickenpox and herpes zoster. Topical acyclovir may benefit herpes zoster in immunosuppressed patients by accelerating cut

Targeting hepatitis B therapy to the liver. Clinical pharmacokinetic considerations.

The hepatitis B virus (HBV) is the world's most important chronic virus infection. The immunomodulator interferon-alpha (IFN alpha) is the only clinically applied drug available, despite its low response rate (approximately 30%) even in highly selected chronic carriers. Antiviral nucleoside analogues have proven to be potent inhibitors of viral replication in vitro, but their significant adverse effects which are, at least partially, due to their nonspecific body distribution, have forced the cessation of their clinical development in the past. For example, vidarabine causes severe neuromuscul

Chickenpox pneumonia: experience with antiviral treatment.

Of 13 patients with chickenpox pneumonia (12 of them adults) treated during 1979-87, 10 received antiviral drugs--nine acyclovir and one vidarabine. Three died despite intensive treatment. Serious secondary infections occurred in six cases. There were no clear indications that antiviral treatment altered the natural history of the condition. Acyclovir may at present be used too late in the course of chickenpox pneumonia to alter its outcome.
