We report a 79-year-old woman with T-cell prolymphocytic leukemia (T-PLL) who was successfully treated with fludarabine monophosphate. She was admitted to our hospital because of dyspnea on effort. On admission, anemia and hepatosplenomegaly were apparent but lymphadenopathy was absent. Peripheral blood examination showed anemia and leukocytosis with 29.5% abnormal lymphocytes. The bone marrow was infiltrated with 84.1% abnormal lymphocytes. The nucleolus was visible in some of these abnormal cells. Eight patients with chronic (mean 52 months) type B hepatitis (chronic active hepatitis and active cirrhosis) and markers of active viral replication (presence of HBeAg and HBV-DNA in serum) were treated with Ara-AMP for 7-12 weeks. In order to reduce the extrahepatic side effects of antiviral nucleoside analogues in the treatment of chronic viral hepatitis, these drugs are conjugated with galactosyl-terminating macromolecules. Fludarabine phosphate is a derivative of adenosine arabinoside. The compound is an antimetabolite which resists deamination by the addition of a phosphate moiety. A phase I trial was conducted and showed the safe dose to good-risk patients to be 20 mg/m2 in 125 ml of 5% dextrose given over 30 min every 12 hr for 6 doses. The cycle can be repeated every 21 days. The major side-effect is myelosuppression, which can be severe at higher doses.
