Vital signs: nonsmokers' exposure to secondhand smoke --- United States, 1999-2008.

BACKGROUND: Secondhand exposure to tobacco smoke causes heart disease and lung cancer in nonsmoking adults and sudden infant death syndrome, acute respiratory infections, middle ear disease, exacerbated asthma, respiratory symptoms, and decreased lung function in children. METHODS: National Health and Nutrition Examination Survey data from 1999-2008 were analyzed to determine the proportion of the nonsmoking population with serum cotinine (the primary nicotine metabolite) levels ≥0.05 ng/mL, by age, sex, race/ethnicity, household income level, and to determine whether the household included a

Outcomes of trauma patients with no vital signs on hospital admission.

In 5 years, 267 patients with cardiopulmonary arrest after trauma were treated at our institution. The long-term survival rate was 2.6%. Only 1.5% of the 267 patients were functional individuals. Overall, neither the mechanism of injury nor routine emergency thoracotomy influenced the salvage rate. Our results in the management of trauma victims without vital signs indicate that: 1) among blunt trauma patients, those with isolated head injury have the highest survival rate; 2) patients with blunt multisystem injuries involving the chest, abdomen, or truncal orthopedic structures are unsalvagea

Vital Signs: Missed Opportunities for Preventing Congenital Syphilis - United States, 2022.

INTRODUCTION: Congenital syphilis cases in the United States increased 755% during 2012-2021. Syphilis during pregnancy can lead to stillbirth, miscarriage, infant death, and maternal and infant morbidity; these outcomes can be prevented through appropriate screening and treatment. METHODS: A cascading framework was used to identify and classify missed opportunities to prevent congenital syphilis among cases reported to CDC in 2022 through the National Notifiable Diseases Surveillance System. Data on testing and treatment during pregnancy and clinical manifestations present in the newborn wer

Do Orthostatic Vital Signs Have Utility in the Evaluation of Syncope?

BACKGROUND: Syncope is a common presentation in the emergency department (ED). The differential diagnosis is long and includes benign conditions as well as acute life threats, such as dysrhythmias or pulmonary embolism. OBJECTIVE: The specific goals of this review are twofold: 1) to define the diagnostic utility of orthostatic vital signs (OVS) as a test for orthostatic syncope, and 2) to determine whether OVS help diagnose or exclude life-threatening causes of syncope in ED patients. METHODS: Three prospective cohort studies plus 2017 national guidelines for syncope management were identifi

Neuropsychiatric outcomes in sepsis: A vital sign worth monitoring.

Septic shock is a state of physiological disarray that disrupts perfusion, overwhelms inflammatory control, and, as evidence now shows, compromises the structural and functional integrity of the brain. In their recent study, Li et al explored how specific hemodynamic derangements, such as reduced cardiac index, low mean arterial pressure, and elevated lactate, correlate with neuropsychiatric outcomes including delirium, depression, anxiety, and post-traumatic stress disorder. The findings reframe psychiatric symptoms not only as downstream consequences, but as measurable, clinically relevant e

Streamlining pediatric vital sign assessment: innovations and insights.

Accurate assessment of pediatric vital signs is critical for detecting abnormalities and guiding medical interventions, but interpretation is challenging due to age-dependent physiological variations. Therefore, this study aimed to develop age-specific centile curves for blood pressure, heart rate, and respiratory rate in pediatric patients and create a user-friendly web-based application for easy access to these data. We conducted a retrospective cross-sectional observational study analyzing 3,779,482 records from the National Emergency Department Information System of Korea, focusing on pati

Clinical Utility and Validity of Exercise Vital Sign in Children.

We reviewed charts of 14,446 sports medicine patients, children aged 5 to 18 years, over a 3-year period to determine the discriminant validity of Exercise Vital Sign (EVS) questions. A logistic regression analyzed factors related to any moderate to vigorous physical activity (MVPA). A linear regression analyzed factors related to amount of MVPA for those who participated in any weekly MVPA. Overall, 48% of children reported meeting physical activity guidelines for 420 min·wk-1. Overall, children reported 400.36 ± 280.04 min·wk-1 of MVPA. Those with depression had significantly less MVPA than

The effect of cocaine and amphetamines on vital signs in trauma patients.

The object of this study was to determine if the presence of sympathomimetics (cocaine, benzoylecgonine, or amphetamine), detected by routine urine toxicology screen, affect the initial presenting pulse, blood pressure, respiratory rate, or level of consciousness in trauma patients. A retrospective chart review was performed on 1,679 patients enrolled in an urban level 1 trauma registry between October 1987, and July 1992, for whom complete toxicology data were available. There were no clinically significant differences in the vital signs of patients with positive toxicology screens and those

Wearable Sensor System for Diver Vital Sign Monitoring.

INTRODUCTION: Military divers operate at the limits of human physiology, where inadequate oxygen delivery to the brain-caused by hypoxemia or reduced cardiac output-can rapidly lead to loss of consciousness and death. Existing dive safety systems provide limited means to monitor vital signs underwater. Pulse oximetry (PO) and photoplethysmography (PPG) can yield critical indicators for anticipating diving casualties, but conventional devices fail under motion, immersion, and communication constraints. To address this gap, we developed the HYbrid Diver Remote Observation System (HYDROS), a wear

Vital signs and other observations used to detect deterioration in pregnant women: an analysis of vital sign charts in consultant-led UK maternity units.

BACKGROUND: Obstetric early warning systems are recommended for monitoring hospitalised pregnant and postnatal women. We decided to compare: (i) vital sign values used to define physiological normality; (ii) symptoms and signs used to escalate care; (iii) type of chart used; and (iv) presence of explicit instructions for escalating care. METHODS: One-hundred-and-twenty obstetric early warning charts and escalation protocols were obtained from consultant-led maternity units in the UK and Channel Islands. These data were extracted: values used to determine normality for each maternal vital sign

Delayed EHR availability of vital signs is associated with nursing shift structure.

Delayed EHR availability of vital signs may jeopardize patient safety and data integrity. In 11.4 million measurements from 26,479 encounters, median measurement-to-validation lag ranged 12-25 minutes across locations; one-third of abnormal values were delayed over an hour. Lag peaked before nursing shift changes, including in settings with device-EHR integration. These findings implicate workforce-driven workflow, rather than technical failure, as a primary barrier to real-time surveillance and health system responsiveness.

Vital signs during complication-free recovery after major surgery: a retrospective analysis.

Continuous vital sign monitoring (CVSM) is superior to intermittent manual vital sign assessment in detecting deviations, but alert thresholds have limited evidence, as a strong definition of physiologically normal variations are lacking. Consequences of too few alerts are missed detections of oncoming complications, and too many alerts result in over-alerting and over-treatment. This study aimed to describe duration and deviation of vital signs in postoperative patients without complications when monitored continuously. Adult patients admitted to the general ward after major surgery were incl

Promoting Sleep for Neurology Inpatients: The Value of Routine Overnight Vital Signs.

Background and objectivesSleep is a critical component of health and recovery. Awakening sleeping patients for routine vital sign monitoring overnight is a common inpatient practice, but its benefits are uncertain. This study asked 3 questions: (1) How often are routine overnight vital signs abnormal? (2) How often are those abnormal vital signs clinically actionable? (3) Are there patients who may not need routine overnight vital sign monitoring?MethodsWe analyzed observational retrospective data extracted from electronic health records of patients admitted to inpatient neurology floors at ou

Vital signs preceding in-hospital cardiac arrest: a matched case-control study.

BACKGROUND: Hospitalised patients undergo routine vital sign measurements to detect clinical deterioration. Research into vital signs prior to in-hospital cardiac arrest is crucial, as early detection of deterioration can enable timely intervention and potentially prevent cardiac arrest. This study aimed to investigate the association between vital signs and the risk of in-hospital cardiac arrest. METHODS: We conducted a registry-based, matched case-control study, including cases with in-hospital cardiac arrest and hospitalised patients as controls, in the Central Denmark Region from 2017 to

Establishing outcome-driven vital signs ranges for children in the prehospital setting.

BACKGROUND: Vital signs are frequently used in pediatric prehospital assessments and guide protocol utilization. Common pediatric vital sign classification criteria identify >80% of children in the prehospital setting as having abnormal vital signs, though few receive lifesaving interventions (LSIs). We sought to identify data-driven thresholds for abnormal vital signs by evaluating their association with prehospital LSIs. METHODS: We evaluated prehospital care records for children (<18 years) transported to the hospital during 2022 from a large, national repository of emergency medical servi

Development and validation of novel sepsis subphenotypes using trajectories of vital signs.

PURPOSE: Sepsis is a heterogeneous syndrome and identification of sub-phenotypes is essential. This study used trajectories of vital signs to develop and validate sub-phenotypes and investigated the interaction of sub-phenotypes with treatment using randomized controlled trial data. METHODS: All patients with suspected infection admitted to four academic hospitals in Emory Healthcare between 2014-2017 (training cohort) and 2018-2019 (validation cohort) were included. Group-based trajectory modeling was applied to vital signs from the first 8 h of hospitalization to develop and validate vitals
