Elevation of vitreous leptin in diabetic retinopathy and retinal detachment.

PURPOSE: Leptin is a cytokine that regulates energy metabolism and is linked to diabetes mellitus through its metabolic actions. Leptin is angiogenic and promotes wound healing, and therefore this investigation was conducted to determine whether leptin is associated with neovascular and fibrotic complications of diabetes and other retinopathies. METHODS: Serum and vitreous samples were collected from patients classified by the presence and type of diabetic retinopathy or other ocular diseases. Leptin was measured in serum and vitreous by radioimmunoassay, and leptin and leptin receptor were l

Induction of vitreolysis and vitreous detachment with hyaluronidase and perfluoropropane gas.

To simulate the posterior vitreous detachment (PVD) in the rabbit, 1 IU hyaluronidase and/or 0.2 ml of perfluoropropane gas was intravitreally injected. Ophthalmoscopic, light microscopic examination prepared by cryotechnique, electron microscopic examination, and electroretinogram were done on the 3rd and 28th postoperative days. As a result, the eyes undergone simultaneous intravitreal injection of 1 IU hyaluronidase and 0.2 ml perfluoropropane gas showed membranous structure split from the internal limiting membrane of the superior retina in 3 days after injection. The eyes also demonstrate

Early posterior vitreous detachment is associated with LAMA5 dominant mutation.

BACKGROUND: Extracellular matrix molecular components, previously linked to multisystem syndromes include collagens, fibrillins and laminins. Recently, we described a novel multisystem syndrome caused by the c.9418G>A p.(V3140M) mutation in the laminin alpha-5 (LAMA5) gene, which affects connective tissues of all organs and apparatus in a three generation family. In the same family, we have also reported a myopic trait, which, however, was linked to the Prolyl 4-hydroxylase subunit alpha-2 (P4HA2) gene. Results of investigation on vitreous changes and their pathogenesis are reported in the pre

Vitreous Microparticle Shedding in Retinal Detachment: A Prospective Comparative Study.

PURPOSE: Microparticles (MPs) are membrane-derived vesicles measuring less than 1 μm in diameter. They are shed from nearly every activated or preapoptotic cell and may exhibit biologic activities in inflammation or apoptosis settings. The main purpose of this study was to determine whether MP shedding was higher in the vitreous of patients with retinal detachment (RD). METHODS: This was a prospective, comparative study. Levels of vitreous MPs (including phosphatidylserine [PS]-expressing MPs, photoreceptor cell-derived MPs, and photoreceptor cell-derived MPs expressing PS) and soluble proinf

Posterior vitreous detachment: evolution and complications of its early stages.

PURPOSE: To summarize emerging concepts regarding the onset and progression, traction effects, and complications of the early stages of age-related posterior vitreous detachment (PVD). DESIGN: Interpretive essay. METHODS: Review and synthesis of selected literature, with clinical illustrations, interpretation, and perspective. RESULTS: Imaging of the vitreoretinal interface with optical coherence tomography has shown that PVD begins in the perifoveal macula. Recent longitudinal studies have demonstrated conclusively that early PVD stages persist chronically and progress slowly over months t

Posterior Vitreous Detachment as Observed by Wide-Angle OCT Imaging.

PURPOSE: Posterior vitreous detachment (PVD) plays an important role in vitreoretinal interface disorders. Historically, observations of PVD using OCT have been limited to the macular region. The purpose of this study is to image the wide-angle vitreoretinal interface after PVD in normal subjects using montaged OCT images. DESIGN: An observational cross-sectional study. PARTICIPANTS: A total of 144 healthy eyes of 98 normal subjects aged 21 to 95 years (51.4±22.0 [mean ± standard deviation]). METHODS: Montaged images of horizontal and vertical OCT scans through the fovea were obtained in ea

Posterior vitreous detachment and macular hole formation in myopic foveoschisis.

PURPOSE: To report two cases of macular hole (MH) associated with myopic foveoschisis (MF). DESIGN: Interventional case report. METHODS: Two women presented with metamorphopsia. Preoperative optical coherence tomography (OCT) showed an MH and MF. We performed vitrectomy, internal limiting membrane (ILM) peeling and gas tamponade. RESULTS: Preoperative OCT examination and intraoperative microscopic observation revealed partial posterior vitreous detachment (PVD) at the posterior retina and vitreous strands adhering to the edge of the MHs. The MF resolved in both patients, but the MHs remaine

Treatment of idiopathic macular holes by induced posterior vitreous detachment.

PURPOSE: To determine if an expansile gas bubble can relieve vitreofoveal traction without vitrectomy by inducing a posterior vitreous detachment (PVD) in eyes with an idiopathic impending or full-thickness macular hole (stages 1-3). The status of the impending and full-thickness macular holes after gas injection and tamponade also was studied secondarily. METHODS: Eighteen patients participated in this pilot study. Eleven patients with an impending macular hole (stages 1A and 1B) and seven patients (8 eyes) with a full-thickness macular hole (stages 2 and 3) received gas injections and prosp

Idiopathic progressive unilateral vitreous fibrosis and secondary traction retinal detachment.

Two eyes with idiopathic progressive avascular fibrocellular proliferation involving the posterior vitreous surface and causing visual loss due to traction on the optic nervehead, peripapillary retina, and macula were studied. This occurred in one eye of each of two elderly patients. There were no other apparent abnormalities. Vitreous surgery was used to remove the posterior vitreous surface and most of the abnormal tissue. This reduced the traction on the retina, although partial visual loss persisted because of chronic macular edema in each case. Ultrastructural examination of the excised t

Spontaneous resolution of foveal detachment in traction maculopathy in high myopia unrelated to posterior vitreous detachment.

BACKGROUND: Foveal detachment associated with foveoschisis usually takes a progressive course, and is associated with a poor visual outcome. The purpose of this study was to report the spontaneous resolution of foveal detachment in patients with myopic traction maculopathy without posterior vitreous detachment. METHODS: A retrospective study involving eight cases of high myopia with foveoschisis and foveal detachment in which the subfoveal fluid had spontaneously resolved. The clinical characteristics and optical coherence tomography (OCT) findings were described. RESULTS: All cases involved

Hemorrhagic posterior vitreous detachment: A comprehensive review of the risks of retinal breaks, detachment, and management options.

Patients with hemorrhagic posterior vitreous detachment (hPVD), defined as concomitant PVD and vitreous hemorrhage, are at heightened risk of retinal breaks (RBs) and detachment (RD). Despite this, patients are routinely given the option between conservative management and early intervention with vitrectomy, as no consensus has been reached regarding the superiority of either strategy. Herein, we provide a scoping review of the current evidence regarding the risk of RBs and RD and management of patients with hPVD. Findings suggest that 30-60% of patients with hPVD may suffer from retinal break

A Case of Self-Detachment of a Stage 3 Idiopathic Epiretinal Membrane Due to Posterior Vitreous Detachment.

We report our findings in a case in which the retinal traction caused by an epiretinal membrane (ERM) was relieved by a spontaneous posterior vitreous detachment (PVD). A 53-year-old woman complained of visual disturbances in both eyes. Her decimal best-corrected visual acuity (BCVA) was 1.2 (about 20/16 in Snellen units) in both eyes with no abnormalities in the anterior chamber, lens, or vitreous. Ophthalmoscopy and optical coherence tomography revealed a Stage 3 ERM in the right eye with a foveal macular thickness (FMT) of 408 μm and an intact posterior hyaloid membrane. The M-chart score w

A New Retinal Detachment Treatment Model for Evaluation of Vitreous Tamponades.

Purpose: To develop a treatment model of rhegmatogenous retinal detachment (RRD) in which the effects of various vitreous tamponades can be explored. Methods: In a primary session, detachment was produced in the right eye of 24 rabbits using vitrectomy, posterior vitreous detachment, retinal break induction, and subretinal injection of viscoelastic solution. The following day, detachments were treated in 16 eyes using SF6 (n = 8) or Healaflow® (HF, a cross-linked hyaluronic acid hydrogel, n = 8) tamponade. Animals were followed for 1 month and thereafter examined macroscopically and morphologi

Posterior vitreous detachment rates post-uncomplicated phacoemulsification surgery: a systematic review.

PURPOSE: It is commonly accepted that phacoemulsification surgery is a risk factor for the development of posterior vitreous detachment (PVD) and may accelerate the process. This is an important consideration particularly in cases involving young patients who pre-operatively have no PVD, given the increased risk of retinal tears and detachments. METHODS: A comprehensive literature search was conducted to identify studies reporting incidence of PVD post-uncomplicated phacoemulsification surgery. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement was used for searc

Comparison of posterior vitreous detachment-related and glaucomatous optic disc hemorrhage.

This study compared the characteristics of posterior vitreous detachment (PVD)-related and glaucomatous optic disc hemorrhage (DH). Fundus photographs of eyes with PVD-related DH (PVD group) and glaucomatous DH (glaucoma group) were reviewed. The shape, type, layer, location (clock-hour sector), and DH/disc area (DH/DA) ratio of DH were investigated. In the PVD group, DH presented as a flame (60.9%), splinter (34.8%), and dot or blot (4.3%) shape. However, most of the glaucomatous DH revealed a splinter shape (92.3%), followed by a flame shape (7.7%, p < 0.001). In the PVD group, the most comm

Relationship Between Posterior Vitreous Detachment and Macular Dimensions in Myopic Eyes.

BACKGROUND: Posterior vitreous detachment (PVD) can result in serious pathologic events at the vitreoretinal interface. This study aims to assess the relationship between PVD, macular thickness (MT), and the foveal avascular zone (FAZ) in myopic eyes. METHODS: This retrospective study evaluated 63 myopic subjects' data who were examined between January 1 and June 30, 2019. The myopes were grouped on a severity scale, namely mild, moderate, high, and very high using their spherical equivalent (SE). The PVD classification was based on OCT images. The status of the PVD and MT were evaluated with
