The efficacy of vomer flap for closure of hard palate during primary lip repair.

BACKGROUND AND AIM: This study presents the institutional experience of the use of vomer flap for early closure of hard palate during unilateral complete cleft-lip repair. The purpose of this study was to determine the survival rate of the vomer flap and to investigate its effect on the subsequent palatoplasty. PATIENTS AND METHODS: This retrospective analysis includes 101 non-syndromic patients with complete unilateral cleft lip who received a vomer flap for the closure of the hard palate during cleft-lip repair. Patients were aged 6 months to 28 years (median 1 year). Success rates of the v

[The primary treatment of velar, palatal and vomer malformations as a prerequisite for normal speech development].

In this article the embryology, morphology, pathophysiology, and treatment of the cleft malformation of the hard, the soft palate, and the vomer are described. A therapy is suggested, which minimizes the risk that these children are suffering from an impaired development of speech. Therefore, it is necessary: 1. to recognize the full extent of the malformation, 2. to close the cleft before the development of hearing and speech is finished as these 2 abilities are fundamental milestones of a normal psychosocial development. 3. Nearly normal anatomical and functional structures are to be achieve

Three-Dimensional Analysis of Vomer Volume and Deviation in Patients With Unilateral Cleft Lip and Alveolus.

Since almost all patients with an alveolar cleft have a deviated and thickened vomer bone, the purpose of this study was to investigate the relationship between the vomer shape and the size of the alveolar cleft in patients with unilateral cleft lip and alveolus (CLA). A total of 40 adult patients with unilateral CLA were enrolled, and 40 normal adults without CLA were enrolled as controls. All patients underwent preoperative computed tomography scans and analysis by computer-aided engineering software to obtain the vomer volume (VV), deviated septal angle (DSA), and alveolar cleft volume (ACV

Suturing a Vomer Flap to the Hard Palate Through a Hole When the Nasal Lining is Not Available.

The nasal lining by suturing a vomer flap onto the bony ridge of the cleft through a hole was repaired.A 16-month-old girl had an incomplete bilateral posterior cleft palate. While incising the periosteum and elevating the nasal mucosa from the cleft edge of the right side, the mucosa was sheared and a buttonhole was made, and as a result, the nasal lining could not be coapted. A vomer flap to close the nasal lining was applied. On the left side, the nasal mucosa from the cleft margin was sutured to the vomer flap. On the right side, where no nasal lining was available, 2 holes were drilled 5

Follow-up investigation of palatal closure by means of a one-layer cranially-based vomer-flap.

The relatively seldom-used technique of a single-layer cranially based vomer-flap for the primary closure of a hard cleft palate has been demonstrated. 25 unilateral cleft lip and palate cases, carefully selected in the age group 6 to 20 years, were examined. 3 different methods of analysing these cases were used in order to determine the dysgnathia incidence. The results of these 3 analyses were compared and a comparison was also made between the other types of hard palatal closures. The results were in favour of the above-mentioned technique, where relatively small growth disturbances, thus

A patient with congenital defect of nasal cartilaginous septal and vomeral bone reconstructed with costal cartilaginous graft.

We present a 64-year-old woman who had a congenital defect of the nasal septal cartilage and vomeral bone. She noticed her nasal tip was collapsed approximately 2 months before consultation. Computed tomography showed the absence of cartilaginous septum and vomeral bone, and radiograph showed a depression in the anterior nasal floor and hypoplasia of anterior nasal spine. She had no history of maxillofacial trauma, disease, or surgery in the midface. Fiberendoscopic examination of nasal cavity showed normal mucosa without signs of chronic inflammatory diseases such as Wegener's granulomatosis.

Three-dimensional surgical guide for frontal-nasal-ethmoid-vomer disjunction in Le Fort III osteotomy.

The aim of this article was to describe the achievement and the clinical use of a new three-dimensional surgical guide for frontal-nasal-ethmoid-vomer osteotomy. The three-dimensional guide is based on a three-dimensional rapid prototyping model and provides the three-dimensional spatial orientation and depth for the frontonasal osteotome. The method was applied to a 7-year-old patient with Apert syndrome. This technique allowed a critical osteotomy path in Le Fort III surgery to be transferred in a secure, fast, and cost-effective way from the three-dimensional rapid prototyping model to the

Vomer flap for hard palate repair is related to favorable maxillary growth in unilateral cleft lip and palate.

OBJECTIVE: Vomer flap repair is assumed to improve maxillary growth because of reduced scarring in growth-sensitive areas of the palate. Our aim was to evaluate whether facial growth in patients with unilateral cleft lip and palate was significantly affected by the technique of hard palate repair (vomer flap versus two-flap). MATERIALS AND METHODS: For this retrospective longitudinal study, we analyzed 334 cephalometric radiographs from 95 patients with nonsyndromic complete unilateral cleft lip and palate who underwent hard palate repair by two different techniques (vomer flap versus two-fla

Vomer Flaps Support Velopharyngeal Competency, Fistula Free Repair, and 3D Construction of Normal Anatomy in Patients with Cleft Palate.

UNLABELLED: Cleft palate repair comprises the surgical creation of a congenitally nonexistent normal anatomy, to establish physiological function by moving tissues into their normal anatomical positions. In patients with isolated incomplete (IICP) or submucous (SMCP) cleft palate, the vomer is usually not completely attached to the palatal plate in the midline. This condition, which is visible through surgical access radiologically or via endoscope, is often disregarded during hard palate repair. This can lead to "hypernasality" despite a well-functioning velopharyngeal mechanism. The general

Presurgical Alignment of Bilateral Cleft Segments With 3D Simulation Under Special Consideration of the Vomer: A Technical Note.

BACKGROUND: The main goal of presurgical orthopedics (PSO) for patients with bilateral cleft lip and palate is to correct the protruded and/or twisted premaxilla. However, PSO is associated with the risk of uncontrolled development of the vomer, which has received little attention to date. SOLUTION: We present a removable orthodontic device that can be used to keep or align the vomer and the premaxilla in the midline during preoperative molding of cleft segments independently and 3 dimensionally.
