Separation of circadian and wake duration-dependent modulation of EEG activation during wakefulness.

The separate contribution of circadian rhythmicity and elapsed time awake on electroencephalographic (EEG) activity during wakefulness was assessed. Seven men lived in an environmental scheduling facility for 4 weeks and completed fourteen 42.85-h 'days', each consisting of an extended (28.57-h) wake episode and a 14.28-h sleep opportunity. The circadian rhythm of plasma melatonin desynchronized from the 42.85-h day. This allowed quantification of the separate contribution of circadian phase and elapsed time awake to variation in EEG power spectra (1-32 Hz). EEG activity during standardized be

Brainstem control of sleep, wakefulness, alertness, and consciousness.

The brainstem is crucial for maintaining wakefulness, sleep regulation, and consciousness. It orchestrates alertness through the ascending reticular activating system, integrating sensory information and projecting to the cortex. Consciousness emerges from intricate interactions involving the brainstem, though the brainstem alone does not generate consciousness. Clinical syndromes resulting from brainstem lesions highlight distinctions between wakefulness, alertness, and consciousness. Disorders such as sleep paralysis, parasomnias, vegetative state, minimally conscious state, coma, and locked

A Subcircuit in the Suprachiasmatic Nucleus Generates Wakefulness.

The daily cycles of sleep and arousal are among the most prominent biological rhythms under circadian control. While the role of the suprachiasmatic nucleus (SCN) as the master circadian pacemaker is well-established, the molecular and circuit mechanisms by which it regulates the rhythms of sleep and arousal remain poorly understood. It has previously shown that the Drosophila clock-output molecule Wide Awake (WAKE) and its mammalian homolog mWAKE are expressed in fly clock neurons and dorsomedial hypothalamus neurons that promote arousal. Here, it is investigated whether mWAKE labels an arous

Sleep/Wakefulness Detection Using Tracheal Sounds and Movements.

PURPOSE: The current gold standard to detect sleep/wakefulness is based on electroencephalogram, which is inconvenient if included in portable sleep screening devices. Therefore, a challenge in the portable devices is sleeping time estimation. Without sleeping time, sleep parameters such as apnea/hypopnea index (AHI), an index for quantifying sleep apnea severity, can be underestimated. Recent studies have used tracheal sounds and movements for sleep screening and calculating AHI without considering sleeping time. In this study, we investigated the detection of sleep/wakefulness states and est

[Unresponsive wakefulness syndrome: clinical predictors of late recovery.].

Patients affected by the outcomes of serious traumatic and/or non-traumatic brain injury may show progressive recovery characterized by a broad spectrum of clinical conditions, which are often not completely different, but of differing seriousness. The unresponsive wakefulness syndrome (UWS) and minimally conscious state (MCS) are syndromes that may characterize progression from the coma state and may be temporary clinical conditions, or in some cases, the final outcome of acute brain injury. The eventual recovery of consciousness is a dynamic process, in constant progress following the acute

Arousal From Sleep and Sympathetic Excitation During Wakefulness.

Obstructive apnea during sleep elevates the set point for efferent sympathetic outflow during wakefulness. Such resetting is attributed to hypoxia-induced upregulation of peripheral chemoreceptor and brain stem sympathetic function. Whether recurrent arousal from sleep also influences daytime muscle sympathetic nerve activity is unknown. We therefore tested, in a cohort of 48 primarily nonsleepy, middle-aged, male (30) and female (18) volunteers (age: 59±1 years, mean±SE), the hypothesis that the frequency of arousals from sleep (arousal index) would relate to daytime muscle sympathetic burst

The maintenance of wakefulness test in pediatric narcolepsy.

Objective tools are needed to assess the response to treatment in pediatric narcolepsy. This article presents a single-center experience documenting the use of the maintenance of wakefulness test (MWT) in a pediatric series. This study reviewed the charts of children with narcolepsy who had an MWT performed between January 2008 and June 2012. A cutoff was used for mean sleep latency: <8 minutes for inadequate control of hypersomnia, and >20 minutes to indicate adequate control on medications. Thirteen tests were performed on 10 children (median age 15.8 years, range 8.7-20.3 years) with narcol

Influence of wakefulness on pharyngeal airway muscle activity.

BACKGROUND: Whether loss of wakefulness itself can influence pharyngeal dilator muscle activity and responsiveness is currently unknown. A study was therefore undertaken to assess the isolated impact of sleep on upper airway muscle activity after minimising respiratory/mechanical inputs. METHODS: Ten healthy subjects were studied. Genioglossus (GG), tensor palatini (TP) and diaphragm (DIA) electromyography (EMG), ventilation and sleep-wake status were recorded. Non-invasive positive pressure ventilation was applied. Expiratory pressure was adjusted to yield the lowest GGEMG, thereby minimisin

Sleep and wakefulness disturbances in Swiss pharmacy customers.

BACKGROUND AND OBJECTIVE: Sleep disturbances are prevalent but often overlooked or underestimated. We suspected that sleep disorders might be particularly common among pharmacy customers, and that they could benefit from counselling. Therefore, we described the prevalence and severity of symptoms associated with sleep and wakefulness disorders among Swiss pharmacy customers, and estimated the need for counselling and treatment. METHODS: In 804 Swiss pharmacies (49% of all community pharmacies) clients were invited to complete the Stanford Sleep Disorders Questionnaire (SDQ), and the Epworth S

Wakefulness: an eye-opening perspective on orexin neurons.

Orexin-containing neurons regulate wakefulness, and loss of orexin produces narcolepsy. Recent studies of mice lacking orexin neurons have shown that these cells also play essential roles in the control of feeding and energy balance.

[Disorders of wakefulness and sleep in blind patients].

The goal of this paper was to summarize three studies focused on sleep/wake disorders in blind subjects. The first study was an epidemiology survey performed in 1073 blind subjects in comparison with non-blind controls. The blind had more episodes of insomnia and free running rhythms. They also took more sleeping pills and complained of more daytime somnolence. The seriousness of the sleep disorders was related to the seriousness of the blindness. In the second study, 78 blind children were compared with seeing children. They had more insomnia and more parasomnias but there was not any more fr

Fear in Dreams Predicts Stronger Affect Reactivity in Wakefulness.

Sleep plays an important role in affective processing and affect regulation during wakefulness. Dreaming-the subjective experiences occurring during sleep-has been suggested to reflect, or even contribute to, waking affect regulation. However, the role of dream affect in waking affect regulation has rarely been studied directly. Here, we investigated how dream affect relates to waking affect reactivity and regulation at the between-person level. Over 7 days, 87 participants (18 - 70 y, M = 27.23, SD = 11.75; 76 females) kept dream journals and rated their dream affect and sleep quality each mo

The Brain Toxin Cleansing of Sleep Achieved During Wakefulness.

A primary purpose of sleep for humans is to remove toxins and metabolic wastes from the brain (e.g., Aβ, tau, lactate) that would otherwise build up and compromise brain functionality. There are currently no drugs or devices that have been clinically shown in humans to enhance brain toxin removal, either during sleep or wakefulness. This perspective article focuses on a recently (re)discovered major route of toxin drainage from the human brain through meningeal lymphatic vessels (mLVs) and the primary enhancer of their flow-the cytokine Vascular Endothelial Growth Factor (VEGF). The purpose of

Rethinking O2, CO2 and breathing during wakefulness and sleep.

We have examined the importance of three long-standing questions concerning chemoreceptor influences on cardiorespiratory function which are currently experiencing a resurgence of study among physiologists and clinical investigators. Firstly, while carotid chemoreceptors (CB) are required for hypoxic stimulation of breathing, use of an isolated, extracorporeally perfused CB preparation in unanaesthetized animals with maintained tonic input from the CB, reveals that extra-CB hypoxaemia also provides dose-dependent ventilatory stimulation sufficient to account for 40-50% of the total ventilatory

Adherence to wakefulness promoting medication in patients with narcolepsy.

OBJECTIVE: Narcolepsy management usually requires lifelong pharmacotherapy. However, we know little about adherence to prescribed treatment in narcolepsy. We assessed adherence to wakefulness-promoting agents in narcolepsy patients. PATIENTS AND METHODS: We retrospectively assessed adherence to wakefulness promoting medication in patients with narcolepsy using the Medicines Possession Ratio (MPR). Three levels of adherence were defined: poor (≤50%), intermediate (51-79%), and good (≥80%). Refractory daytime sleepiness was defined as an Epworth sleepiness scale (ESS) score >12 despite triallin

[Non-epileptic motor paroxysmal phenomena in wakefulness in childhood].

Paroxysmal events in childhood are a challenge for pediatric neurologists, given its highly heterogeneous clinical manifestations, often difficult to distinguish between phenomena of epileptic seizure or not. The non-epileptic paroxysmal episodes are neurological phenomena, with motor, sensory symptoms, and/or sensory impairments, with or without involvement of consciousness, epileptic phenomena unrelated, so no electroencephalographic correlative expression between or during episodes. From the clinical point of view can be classified into four groups: motor phenomena, syncope, migraine (and a

Proton- and ammonium-sensing by histaminergic neurons controlling wakefulness.

The histaminergic neurons in the tuberomamillary nucleus (TMN) of the posterior hypothalamus are involved in the control of arousal. These neurons are sensitive to hypercapnia as has been shown in experiments examining c-Fos expression, a marker for increased neuronal activity. We investigated the mechanisms through which TMN neurons respond to changes in extracellular levels of acid/CO(2). Recordings in rat brain slices revealed that acidification within the physiological range (pH from 7.4 to 7.0), as well as ammonium chloride (5 mM), excite histaminergic neurons. This excitation is signific

Multiple sleep latency test and maintenance of wakefulness test.

Excessive daytime sleepiness and fatigue are common complaints in the sleep clinic. The objective evaluation and quantification of these symptoms is important for both the diagnosis of underlying health problems and for gauging treatment response. The multiple sleep latency test measures physiologic sleepiness, whereas the maintenance of wakefulness test (MWT) aims to measure manifest sleepiness. Neither test correlates well with subjective measures of sleep such as the Epworth sleepiness scale and the Stanford sleepiness scale. Although in the past methodological testing differences existed,
