Metabolic changes in the pig liver during warm ischemia and reperfusion measured by microdialysis.

AIM: Portal triad clamping can cause ischemia-reperfusion injury. The aim of the study was to monitor metabolic changes by microdialysis before, during, and after warm ischemia in the pigliver. MATERIAL AND METHODS: Eight pigs underwent laparotomy followed by ischemia by Pringle's maneuver. One microdialysis catheter was placed in each of four liver lobes. A reference catheter was placed in a muscle. Microdialysis samples were collected at intervals of 30 min starting 2 h before 1 h of total ischemia followed by 3 h of reperfusion. Glucose, lactate, pyruvate, and glycerol concentrations were

Donor Warm Ischemia Time Modifies Susceptibility to Cold Ischemia in Donation After Circulatory Death Kidney Allografts Under Hypothermic Machine Perfusion.

BACKGROUND: Donation after circulatory death (DCD) kidneys are exposed to donor warm ischemia time (dWIT) in addition to cold ischemia. Although porcine studies suggest synergy between these insults, evidence in clinical transplantation is limited. METHODS: Adult DCD kidney-only transplants performed using hypothermic machine perfusion (n = 28 698) in the United Network for Organ Sharing database (January 1, 2015, to December 31, 2023) were included. We performed 1:1:1 propensity score matching across 3 cold ischemia time (CIT) categories (short, <18 h; intermediate, 18-23 h; and long, ≥24 h)

Oxygen Consumption by Warm Ischemia-Injured Porcine Kidneys in Hypothermic Static and Machine Preservation.

Static cold storage (SCS) and hypothermic machine perfusion (HMP) are currently standard methods for renal grafts clinical preservation. Both methods are predominantly implemented without the active delivery of oxygen, even for donation after circulatory death-like kidneys. However, even under severe hypothermia (4°C-6°C), kidneys can consume oxygen and produce ATP. What is not established, though, is to what extent and how SCS and HMP compare in terms of oxygen. Using a porcine preclinical model of renal warm ischemia (WI) to compare SCS and HMP methods, we continuously monitored and quantifi

Cytochrome P450 induction by phenobarbital exacerbates warm hepatic ischemia-reperfusion injury in rat livers.

Recent studies have shown that cytochrome P450 inhibitors reduce oxidative stress and injury to the liver following warm ischemia-reperfusion (IR). The aim here was to test the effect of P450 induction by phenobarbital on the IR injury in rat livers. Rats were pre-treated with saline or phenobarbital and subjected to IR or sham operation. IR significantly increased the plasma alanine aminotransferase concentrations. Phenobarbital further exacerbated the injury by an additional 50% increase in the alanine aminotransferase levels. Phenobarbital also caused an approximately 40% increase in the to

Splenectomy ameliorates acute multiple organ damage induced by liver warm ischemia reperfusion in rats.

BACKGROUND: Liver ischemia/reperfusion (I/R) results in the release of destructive proinflammatory cytokines and oxygen-derived radicals, which in turn cause injury to liver and other organs such as kidney, lung, and intestine. Splenectomy protects organs from intestinal I/R injury. Therefore, the present study aims to investigate whether splenectomy could also ameliorate multiple organ damage caused by liver I/R. METHODS: Wistar rats randomly assigned into 4 groups underwent sham-operation, splenectomy, hepatic I/R induced by occlusion of hepatic artery and portal vein, and splenectomy plus

Revascularization of digits after thirty-three hours of warm ischemia time: a case report.

A case of digits revascularized after 33 hours of warm ischemia time is described. Ischemic changes had occurred already with ultimate loss of the distal segments, but most of the fingers survived to provide the patient with adequate function. Although the commonly accepted limit of warm ischemia time has been reported to be within 6 to 12 hours, revascularization of digits can be attempted in selected cases even after an apparent prolonged warm ischemia time.

Recovery of renal function after warm ischemia. I. The effect of chlorpromazine and phenoxybenzamine.

The effects of treatment with chlorpromazine (4 mg/kg) and phenoxybenzamine (1 and 5 mg/kg) on renal function and morphology after warm ischemia and contralateral nephrectomy were studied. Chlorpromazine pretreatment by intravenous injection 15 min before warm ischemia of 60 min resulted in the survival of all animals (cf. 75% in untreated group), with better renal function in the first week. Necrosis of the proximal convoluted tubule and ultimate residual cortical damage were less severe than in the untreated groups. Chlorpromazine was also beneficial after 75 min warm ischemia, although mort

Impact of Donor Age and Prolonged Warm Ischemia in Normothermic Machine Perfusion of Rat Livers.

BACKGROUND: Expanding the liver donor pool increasingly depends on extended criteria donors (ECD), including older individuals and those after circulatory death. However, ECD grafts are more susceptible to ischemia-reperfusion injury. Normothermic machine perfusion (NMP) offers a promising solution to preserve the metabolic activity of liver grafts and to assess the graft quality at the same time. METHODS: We used a standardized rat NMP model to examine the combined effects of donor age and donation after cardiac death on graft viability. Following circulatory arrest and 30 min of warm ischem

Normothermic Machine Perfusion Preserves Endothelial Clearance, Modulates Inflammation After Prolonged Warm Ischemia in Liver Grafts.

BACKGROUND: Prolonged warm ischemia time (WIT) increases susceptibility to ischemia-reperfusion injury in liver grafts. Viability assessment during normothermic machine perfusion (NMP) mainly targets hepatocyte and cholangiocyte function, whereas the contribution of liver sinusoidal endothelial cells (LSECs) is less explored. We evaluated whether clearance of hyaluronan (HA) and N-terminal propeptide of procollagen type III (PIIINP) reflects LSEC clearance function and predicts inflammatory activation during NMP. METHODS: In a porcine model with 45 min of WIT, livers were randomized to either

Anti-heparanase shields against endothelial dysfunction in a rat model of warm pulmonary ischemia-reperfusion.

BACKGROUND: Primary graft dysfunction (PGD) occurs within the first three days after lung transplantation (LT), primarily due to ischemia–reperfusion (I/R) injury. Heparanase is an enzyme involved in I/R mechanisms. We investigated the effects of heparanase inhibition using derivative from λ-Carrageenan referred to as “Carrageenan”, and Heparin on endothelial dysfunction, inflammation, and glycocalyx integrity in a warm I/R rat model. METHODS: Three groups of male Wistar rats were studied: Control (NaCl 0.9%)(n = 14), Heparin (n = 14), and Carrageenan (n = 16). Rats underwent 1 h of left pulmo

Overcoming the Boundaries of Heart Warm Ischemia in Donation After Circulatory Death: The Padua Case.

A 45 year old male obese patient with a previous history of repaired congenital heart disease developed worsening heart failure making heart transplantation listing mandatory. Unfortunately, due to his anthropometric measures, the search for a suitable brain-dead donor was unsuccessful. For this reason, he accepted to be enrolled in the controlled donation after circulatory death (cDCD) program. According to the Italian Law regulating death declaration after cardiac arrest (no-touch period of 20 minutes-one of the longest in the world), we faced a 34 minute cardiac asystole, after which the he

Oxaloacetate Protects Rat Liver From Experimental Warm Ischemia/Reperfusion Injury by Improving Cellular Energy Metabolism.

Liver ischemia/reperfusion injury (IRI) is an important cause of liver damage especially early after liver transplantation, following liver resection, and in other clinical situations. Using rat experimental models, we identified oxaloacetate (OAA) as a key metabolite able to protect hepatocytes from hypoxia and IRI. In vitro screening of metabolic intermediates beneficial for hepatocyte survival under hypoxia was performed by measures of cell death and injury. In vivo, the effect of OAA was evaluated using the left portal vein ligation (LPVL) model of liver ischemia and a model of warm IRI. L

The influence of warm ischemia elimination on kidney injury during transplantation - clinical and molecular study.

Kidney surface cooling was used during implantation to assess the effect of warm ischemia elimination on allograft function, histological changes and immune-related gene expression. 23 recipients were randomly assigned to a group operated on with kidney surface cooling during implantation (ice bag technique, IBT group), and the other 23 recipients receiving the contralateral kidney from the same donor were operated on with a standard technique. Three consecutive kidney core biopsies were obtained during the transplantation procedure: after organ recovery, after cold ischemia and after reperfus

Resveratrol Prevents Warm Ischemia-Reperfusion Injury in Liver Grafts From Non-Heart-Beating Donor Rats.

BACKGROUND: Successful liver transplantation from non-heart-beating donors (NHBDs) might enlarge donor source. Some studies have reported that resveratrol (RES), an activator of sirtuins, has cytoprotective effects on ischemia-reperfusion (I/R) injury. The purpose of this study was to investigate the effects of RES on warm I/R injury in rats. METHODS: Male Wister rats were divided into 5 groups: (1) the heart-beating (HB) group, whose livers were retrieved from HB donors; (2) the NHB group, whose livers were retrieved under apnea-induced NHB conditions; (3) the ethanol group, retrieved in the

Warm ischemia-induced alterations in oxidative and inflammatory proteins in hepatic Kupffer cells in rats.

The aim of the study was to investigate the impact of ischemia/reperfusion injury on the proteome of Kupffer cells. Lean Zucker rats (n = 6 each group) were randomized to 75 min of warm ischemia or sham operation. After reperfusion for 8 h, Kupffer cells were isolated by enzymatic perfusion and density gradient centrifugation. Proteins were tryptically digested into peptides and differentially labeled with iTRAQ (isobaric tags for relative and absolute quantitation) reagent. After fractionation by cation exchange chromatography, peptides were identified by mass spectrometry (ESI-LC-MS/MS). Spe

Efficacy of prostaglandin I2 analog on liver grafts subjected to 30 minutes of warm ischemia.

Due to the fact that no effective conditioning agent for hemodynamically unstable donors exists, the number of suitable donors is limited. The efficacy of OP-41483, a stable analog of prostaglandin I2, as a conditioning agent of the liver was investigated in this study using pigs. OP-41483 was administered via the portal tributary for 10 min before 30 min of warm ischemia. Graft livers were procured after perfusion with OP-41483 in cold normal saline, then flushed with OP-41483 in Euro-Collins solution and placed in cold storage prior to orthotopic transplantation. OP-41483 was also administer

Anti-apoptosis effects of oxymatrine protect the liver from warm ischemia reperfusion injury in rats.

Warm ischemia and reperfusion (WI/R) results in the release of destructive proinflammatory cytokines and oxygen free radicals, which in turn cause injury to the liver. Apoptosis is regarded as the central mechanism of liver injury during WI/R. Oxymatrine, an extract from a traditional Chinese herb, Sophora flavescens Ait, has been widely used for the treatment of chronic hepatitis, by virtue of its anti-inflammatory and anti-apoptotic activity. The objective of this study was to investigate whether administration of oxymatrine could protect livers against WI/R. The experimental design consiste
