Enhanced detection of mpox virus in wastewater using a pre-amplification approach: A pilot study informing population-level monitoring of low-titer pathogens.

A recent outbreak of the mpox virus (MPXV) occurred in non-endemic regions of the world beginning in May 2022. Pathogen surveillance systems faced pressure to quickly establish response protocols, offering an opportunity to employ wastewater-based epidemiology (WBE) for population-level monitoring. The pilot study reported herein aimed to: (i) develop a reliable protocol for MPXV DNA detection in wastewater which would reduce false negative reporting, (ii) test this protocol on wastewater from various regions across the United States, and (iii) conduct a state of the science review of the curr

Spatiotemporal genome diversity of SARS-CoV-2 in wastewater: a two-year global epidemiological study.

Wastewater surveillance locally and globally is important for the investigation of the molecular epidemiological features of SARS-CoV-2 in the environment. The current study investigated the genomic diversity and mutation profile of SARS-CoV-2 variants in wastewater for the period spanning COVID-19 pandemic up to December, 2022. A total of 3618 complete SARS-CoV-2 genome sequences from waste water samples submitted to the GISAID database were retrieved. The SARS-CoV-2 sequences were subjected to pairwise alignment against reference, followed by clade and lineage assignment (based on Nextstrain

Rapid identification of COVID wastewater surges in the absence of case data.

Genetic testing of community wastewater (wastewater surveillance) is a valuable tool for following trends in the abundance of SARS-CoV-2 and other infectious disease pathogens over time. Wastewater surveillance is increasingly important in the absence of corresponding epidemiological data, particularly for infectious diseases with limited timely data on clinical case incidences. Due to the inherent noise in wastewater data, a single sample is not sufficient to identify a sustained trend in the abundance of a target. This challenge is magnified in resource-limited settings where samples may be

Decay and solid-liquid partitioning of mpox and vaccinia virus DNA in primary influent and settled solids to guide wastewater-based epidemiology practices.

Wastewater-based epidemiology (WBE) has proven to be a powerful tool for tracking the spread of viral pathogens, such as SARS-CoV-2; however, as WBE has expanded to include new pathogens, like mpox virus (MPXV), more data is needed to guide practitioners on how to design WBE campaigns. Here, we investigated the decay rates of heat-inactivated MPXV (HI-MPXV) and attenuated vaccinia virus (VV) viral signal in primary influent and settled solids collected from a local POTW at 4 °C, 22 °C, or 35 °C using digital PCR. Subsequently, we studied the solid-liquid partitioning of the viruses in primary

Spatial and temporal assessment of crack cocaine use in 13 European cities through wastewater-based epidemiology.

Already in early 2000s, concerns have been growing in the EU about increasing use of cocaine and it is estimated that below 1 % of the population administer the drug by smoking crack cocaine. New available data suggests an increase in the use of crack cocaine and an increase in the number of crack cocaine users entering treatment has been reported in several European countries. Robust estimations of crack cocaine use are however not available yet. The use of crack cocaine has long been associated with severe adverse socio-economic conditions as well as mental health problems, such as suicide i

Biomarker selection strategies based on compound stability in wastewater-based epidemiology.

The specific compositions of human excreta in sewage can be used as biomarkers to indicate the disease prevalence, health status, and lifestyle of the population living in the investigated catchment. It is important for guiding and evaluating public health policies as well as promoting human health development. Among several parameters of wastewater-based epidemiology (WBE), the decay of biomarkers during transportation in sewer and storage plays a crucial role in the back-calculation of population consumption. In this paper, we summarized the stability data of common biomarkers in storage at

Performance- and image-enhancing drug use in the community: use prevalence, user demographics and the potential role of wastewater-based epidemiology.

Performance- and image-enhancing drug (PIED) misuse is a significant public health issue. Currently, seizure data, surveys, anti-doping testing, and needle service provider data are used to estimate PIED use in populations. These methods are time consuming, single point-in-time measurements that often consist of small sample sizes and do not truly capture PIED prevalence. Wastewater-based epidemiology (WBE) has been used globally to assess and monitor licit and illicit drug consumption within the general community. This method can objectively cover large populations as well as specific subpopu

Methodological Approaches to Dengue Virus Detection in Wastewater: A Systematic Review and Meta-Analysis of Positivity Rate.

Dengue fever, with a high proportion of asymptomatic infections, poses a major global public health challenge that traditional surveillance systems frequently underestimate. Wastewater-based epidemiology (WBE) has emerged as a promising approach to monitoring infectious diseases beyond enteric viruses. Dengue virus is shed in urine, feces, and saliva, providing a biological basis for wastewater detection alongside clinical surveillance. This systematic review and meta-analysis synthesize current evidence on dengue virus (DENV) detection in wastewater and evaluate methodological factors influen

Emergence of genetically linked vaccine-originated poliovirus type 2 in the absence of oral polio vaccine, Jerusalem, April to July 2022.

We report an emergence and increase in poliovirus type 2 detection via routine wastewater surveillance in three non-overlapping regions in the Jerusalem region, Israel, between April and July 2022. Sequencing showed genetic linkage among isolates and accumulation of mutations over time, with two isolates defined as vaccine-derived polioviruses (VDPV). This demonstrates the emergence and potential circulation of type 2 VDPV in a high-income country with high vaccine coverage and underscores the importance of routine wastewater surveillance during the polio eradication.

A multivariate analysis to explain residue errors in pathogen concentration in wastewater-based epidemiology.

With the beginning of the COVID-19 pandemic, wastewater-based epidemiology (WBE), which according to Larsen et al. (2021), describes the science of linking pathogens and chemicals found in wastewater to population-level health, received an enormous boost worldwide. The basic procedure in WBE is to analyse pathogen concentrations and to relate these measurements to cases from clinical data. This prediction of cases is subject to large errors, due to various factors such as dilution effects, decay or wastewater matrix and inhibitors. In this study we used different models to identify the most im

Assessment of environmental factors influencing SARS-CoV-2 in Vietnam's surface water across two years of clinical data.

Wastewater-based epidemiology (WBE) is an effective, non-invasive method for monitoring the spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) by tracking viral prevalence in water. This study aimed to investigate the presence of SARS-CoV-2 in surface water in Vietnam over two years. One-step quantitative reverse transcription polymerase chain reaction (qRT-PCR) assays were employed to quantify SARS-CoV-2 and its variant-specific mutation sites (G339D/E484A) and pepper mild mottle virus (PMMoV) from a total of 315 samples (105 samples per site) to compare with reported Coro

Hospital wastewater surveillance for SARS-CoV-2 identifies intra-hospital dynamics of viral transmission and evolution.

Wastewater testing has emerged as an effective, widely used tool for population-level severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) surveillance. Such efforts have been implemented primarily at wastewater treatment plants, providing data for large resident populations but hindering the ability to implement targeted interventions or follow-ups. Conversely, building-level wastewater data exhibit increased variability due to rapid daily population dynamics but allow for targeted follow-up or mitigation efforts. Here, we implemented a three-site wastewater sampling strategy on our u

Sewers to Solutions: A Guide to Wastewater Pathogen Monitoring.

Wastewater-based epidemiology (WBE) is the analysis of wastewater to detect pathogen levels or activity for public health awareness or action. Pioneered in the 1940s, WBE underwent a resurgence during the COVID-19 pandemic, providing important information about number of cases, outbreaks, and seasonal impact. With advancements in detection technologies and growing interest in environmental surveillance, WBE is poised to become a standard practice in public health monitoring. Here, we provide an overview of the current state of the art of pathogen WBE, including methods of molecular detection,

Preparing for Mpox Resurgence: Surveillance Lessons From Outbreaks in Toronto, Canada.

BACKGROUND: With many global jurisdictions, Toronto, Canada, experienced an mpox outbreak in spring/summer 2022. Cases declined following implementation of a large vaccination campaign. A surge in early 2023 led to speculation that asymptomatic and/or undetected local transmission was occurring in the city. METHODS: Mpox cases and positive laboratory results are reported to Toronto Public Health. Epidemic curves and descriptive risk factor summaries for the 2022 and 2023 outbreaks were generated. First- and second-dose vaccination was monitored. Mpox virus wastewater surveillance and whole ge

A Sensitive and Rapid Wastewater Test for SARS-COV-2 and Its Use for the Early Detection of a Cluster of Cases in a Remote Community.

Throughout the coronavirus disease 2019 (COVID-19) pandemic, wastewater surveillance has been used to monitor trends in severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) prevalence in the community. A major challenge in establishing wastewater surveillance programs, especially in remote areas, is the need for a well-equipped laboratory for sample analysis. Currently, no options exist for rapid, sensitive, mobile, and easy-to-use wastewater tests for SARS-CoV-2. The performance of the GeneXpert system, which offers cartridge-based, rapid molecular clinical testing for SARS-CoV-2 in a
