Why protein-energy wasting leads to faster progression of chronic kidney disease.

PURPOSE OF REVIEW: Protein-energy wasting (PEW) is increasingly more prevalent as chronic kidney disease (CKD) progresses to more advanced stages. There is a global recognition of the importance of preventing and mitigating PEW in the CKD population not on dialysis given the goal of extending dialysis-free time and delaying dialysis initiation and growing evidence of the clinical consequences of PEW which include the risk of death, hospitalization and clinical conditions such as infections. We reviewed the association of PEW and the malnutrition characteristics indicative of PEW on CKD progres

Review on PRNP genetics and susceptibility to chronic wasting disease of Cervidae.

To date, chronic wasting disease (CWD) is the most infectious form of prion disease affecting several captive, free ranging and wild cervid species. Responsible for marked population declines in North America, its geographical spread is now becoming a major concern in Europe. Polymorphisms in the prion protein gene (PRNP) are an important factor influencing the susceptibility to prions and their rate of propagation. All reported cervid PRNP genotypes are affected by CWD. However, in each species, some polymorphisms are associated with lower attack rates and slower progression of the disease. T

Using megestrol acetate to ameliorate protein-energy wasting in chronic kidney disease.

BACKGROUND: Various populations are affected by chronic kidney disease (CKD), and a low dose appetite stimulant megestrol acetate (MA) is sometimes recommended in patients with CKD to ameliorate protein-energy wasting (PEW). Patients with CKD are at greater risk of developing PEW since the progression of their disease can cause decreased nutrient intake, catabolic effects, systemic inflammation and metabolic changes. Providers can detect PEW in CKD by identifying low serum levels ≤3.8 g/dl of albumin, <30 mg/dl of transthyretin, or <100 mg/dl of cholesterol. Other characteristics include BMI <

Latest consensus and update on protein-energy wasting in chronic kidney disease.

PURPOSE OF REVIEW: Protein-energy wasting (PEW) is a state of metabolic and nutritional derangements in chronic disease states including chronic kidney disease (CKD). Cumulative evidence suggests that PEW, muscle wasting and cachexia are common and strongly associated with mortality in CKD, which is reviewed here. RECENT FINDINGS: The malnutrition-inflammation score (KALANTAR Score) is among the comprehensive and outcome-predicting nutritional scoring tools. The association of obesity with poor outcomes is attenuated across more advanced CKD stages and eventually reverses in the form of obesi

Human prion protein sequence elements impede cross-species chronic wasting disease transmission.

Chronic wasting disease (CWD) is a fatal prion disease of North American deer and elk and poses an unclear risk for transmission to humans. Human exposure to CWD occurs through hunting activities and consumption of venison from prion-infected animals. Although the amino acid residues of the prion protein (PrP) that prevent or permit human CWD infection are unknown, NMR-based structural studies suggest that the β2-α2 loop (residues 165-175) may impact species barriers. Here we sought to define PrP sequence determinants that affect CWD transmission to humans. We engineered transgenic mice that e

Triggers and mechanisms of skeletal muscle wasting in chronic obstructive pulmonary disease.

Skeletal muscle wasting contributes to impaired exercise capacity, reduced health-related quality of life and is an independent determinant of mortality in chronic obstructive pulmonary disease. An imbalance between protein synthesis and myogenesis on the one hand, and muscle proteolysis and apoptosis on the other hand, has been proposed to underlie muscle wasting in this disease. In this review, the current understanding of the state and regulation of these processes governing muscle mass in this condition is presented. In addition, a conceptual mode of action of disease-related determinants

Assessment of prospective preventive therapies for chronic wasting disease in mule deer.

We compared prion infection rates among mule deer (Odocoileus hemionus) receiving pentosan polysulfate, tannic acid, tetracycline HCl, or no treatment 14 days before to 14 days after (dpi) oral inoculation with tonsil tissue homogenate. All deer were infected, but the rapid disease course (230-603 dpi) suggested our challenge was overwhelming.

Gene expression alterations in Rocky Mountain elk infected with chronic wasting disease.

BACKGROUND: The occurrence of thyroid carcinoma in patients with Graves' disease (GD) has been rising recently. However, the linkage between lipids and the incidence of thyroid carcinoma among GD patients is still not well-established. OBJECTIVE: The research aims to explore the relationship between serum lipid concentrations and the occurrence of thyroid cancer in patients diagnosed with GD. METHODS: We conducted a retrospective analysis of data from 512 patients with GD who underwent surgical procedures at our institution between 2015 and 2024. Our study focused on examining the correlatio

Fighting protein-energy wasting in chronic kidney disease: a challenge of complexity.

Chronic uremia is often characterized by wasting of muscle and fat mass, which has been defined as protein-energy wasting (PEW), and is responsible for substantial worsening of patient outcome in terms of morbidity and mortality, mostly from cardiovascular events. Despite major advances in patient treatment, nutritional outcome in patients with end-stage renal disease has not improved substantially in recent years. Extensive research in this field has provided plausible explanations for this limitation by indicating that the pathogenesis of PEW in kidney disease is complex and multifactorial.

Why is protein-energy wasting associated with mortality in chronic kidney disease?

Observational studies in chronic kidney disease (CKD) populations consistently have shown the strong mortality-predictability of such markers of protein-energy wasting (PEW) as hypoalbuminemia, low serum cholesterol levels, low body mass index, and reduced dietary protein intake. Even though the PEW-mortality association data traditionally are reported mostly in maintenance dialysis patients, emerging studies extend the existence of these associations to predialysis stages of CKD. Paradoxic risk factor patterns (reverse epidemiology) for both obesity and cholesterol recently have been reported

Prion-seeding activity in cerebrospinal fluid of deer with chronic wasting disease.

Transmissible spongiform encephalopathies (TSEs), or prion diseases, are a uniformly fatal family of neurodegenerative diseases in mammals that includes chronic wasting disease (CWD) of cervids. The early and ante-mortem identification of TSE-infected individuals using conventional western blotting or immunohistochemistry (IHC) has proven difficult, as the levels of infectious prions in readily obtainable samples, including blood and bodily fluids, are typically beyond the limits of detection. The development of amplification-based seeding assays has been instrumental in the detection of low l

Exercise treatment to counteract protein wasting of chronic diseases.

PURPOSE OF REVIEW: The objective is to summarize the findings from recent (June 2001-2002) studies that have examined the potential benefits of exercise training for the treatment of wasting associated with sarcopenia, cancer, chronic renal insufficiency, rheumatoid arthritis, osteoarthritis and HIV. In many clinical conditions, protein wasting and unintentional weight loss are predictors of morbidity and mortality. The pathogenesis of protein wasting in these conditions can be different, but the fundamental mechanism is an imbalance between muscle protein synthetic and proteolytic processes.

Evolution of Diagnostic Tests for Chronic Wasting Disease, a Naturally Occurring Prion Disease of Cervids.

Since chronic wasting disease (CWD) was first identified nearly 50 years ago in a captive mule deer herd in the Rocky Mountains of the United States, it has slowly spread across North America through the natural and anthropogenic movement of cervids and their carcasses. As the endemic areas have expanded, so has the need for rapid, sensitive, and cost effective diagnostic tests-especially those which take advantage of samples collected antemortem. Over the past two decades, strategies have evolved from the recognition of microscopic spongiform pathology and associated immunohistochemical stain

Modeling chronic wasting disease transmission risk in mule deer related to habitat characteristics.

Chronic wasting disease (CWD) is a prion disease of cervids that spreads to uninfected individuals through direct transmission (contact with infected individuals), vertical transmission (from mother to offspring), or indirect transmission (exposure to contaminated environments). The risk of indirect transmission is unevenly distributed on the landscape, and risk levels are expected to be controlled by patterns of habitat use by infected and uninfected individuals as well as environmental properties that alter the length of time prions remain infectious and available for uptake. Despite evidenc

The Regulation of CCN1 Contributes to Skeletal Muscle Wasting in Chronic Kidney Disease.

BACKGROUND: Sarcopenia is a prevalent complication of chronic kidney disease (CKD), yet reliable biomarkers remain limited. CCN1, a matricellular protein involved in cellular senescence, has been implicated in muscle wasting, but its role in CKD-associated muscle strength decline is incompletely understood. METHODS: Serum CCN1 levels were measured by ELISA in 40 stage 3-5 CKD patients and 27 age-matched controls and correlated with handgrip strength (HGS). A 5/6 nephrectomy (NX) mouse model was established to evaluate muscle strength and senescence markers. C2C12 myotubes were treated with re

Vertical transmission of chronic wasting disease in free-ranging white-tailed deer populations.

Chronic wasting disease (CWD) is a fatal neurodegenerative disease affecting cervids across North America, Northern Europe, and Asia. Disease transmission among cervids has historically been attributed to direct animal-to-animal contact with 'secreta' (saliva, blood, urine, and feces) containing the infectious agent, and indirect contact with the agent shed to the environment in these bodily components. Mounting evidence provides another mechanism of CWD transmission, that from mother-to-offspring, including during pregnancy (vertical transmission). Here we describe the detection of the infect

Dietary magnesium and copper affect survival time and neuroinflammation in chronic wasting disease.

Chronic wasting disease (CWD), the only known wildlife prion disease, affects deer, elk and moose. The disease is an ongoing and expanding problem in both wild and captive North American cervid populations and is difficult to control in part due to the extreme environmental persistence of prions, which can transmit disease years after initial contamination. The role of exogenous factors in CWD transmission and progression is largely unexplored. In an effort to understand the influence of environmental and dietary constituents on CWD, we collected and analyzed water and soil samples from CWD-ne

[Clinical findings of cows who originated from dairy herds with chronic wasting disease].

Since Spring 1999, a number of dairy farms have reported problems with wasting in cattle. After calving, the physical condition of the cows deteriorated for reasons unknown. Chronic wasting is also associated with disorders such as lameness, abortion, endometritis, mastitis, and respiratory problems. Between April 1999 and March 2000, 19 cows were sent to the Faculty of Veterinary Medicine for further investigation of the chronic wasting. Clinical and pathological investigations resulted in a broad range of diagnoses such as mastitis, abomasal displacement, and claw disorders. The latter, char
