Patterns of weight bearing impact sagittal spinal balance.

UNLABELLED: A physiologic sagittal alignment of the spine with lumbar lordosis and thoracic kyphosis is the most stable position of the spine, while in patients with Idiopathic Scoliosis (IS) the sagittal profile of the spine is flattened or inversed. It has been shown that a correction of the sagittal profile also corrects coronal plane deformity in patients with IS. Therefore sagittal corrections seems to play an important role in the conservative treatment of IS. Within the "Best Practice" PT program simple tools are used to correct scoliosis in 3D. One of these tools is the sagittal realig

[Management of weight-bearing area fracture of acetabulum].

Acetabulum, as the important factor for weight bearing of the upper body, has its unique anatomic features and complicated physiological function. The integrity and stability of the lunata articular surface in the dome region of acetabulum, is the important base to bear the physiological function of acetabulum. The fracture related to this part will cause relation change of contact area and stress between head of femur and acetabulum. Furthermore, the deep anatomical position of the dome region, the complicated surrounding anatomical relation, and the irregular bony structure will also increas

Magnetic resonance imaging of the weight-bearing spine.

Weight-bearing magnetic resonance (MR) imaging of the spine can either be simulated by imaging the patient in the supine position in combination with a special axial loading device or be achieved by using vertically open-configuration MR systems, which allow for in vivo MR images of the spine under upright weight-bearing conditions in either seated or standing body positions. Weight-bearing MRI of the spine permits the study of physiological as well as pathological changes in the relationships of the intervertebral disk, the spinal canal, and the neural foramina as well as the assessment of se

Carpal instability in the weight-bearing upper extremity.

The prevalence of carpal instability in a paraplegic population was investigated to establish an association between chronic repetitive stress on the wrist and the development of such instability. Nine of 162 paraplegic patients had static carpal instability and no history of an acute injury of the wrist. The predominant pattern of instability, found in eleven wrists (six patients), was non-dissociative volar intercalated segmental instability. The prevalence of carpal instability increased with the duration of weight-bearing on the upper extremity. Eighteen per cent of the patients in whom th

Early weight bearing of lateral tibial plateau fractures.

From 1975 to 1984, 134 consecutive lateral tibial plateau fractures were treated as part of this prospective study. Eighty-six fractures (64%) were followed until union, with an average follow-up period of 22.7 months. The two aims of this study were: First, to analyze the results of operative and nonoperative therapy in each fracture type, and second, to evaluate the effect of early weight bearing in a cast brace in both types of treatment. All lateral tibial plateau fractures with greater than 5 mm depression or displacement were treated operatively. Forty-four patients were treated operativ

[Weight-bearing deficiency of the 1st metatarsal ray].

The authors have analysed a special disturbance of the statics and dynamics of the forefoot - deficient weight-bearing on the first metatarsal ray whilst walking. From the clinical point of view, the condition resulted in the development of a dorsal bunion and metatarsalgia. Forty-nine cases were treated and seven causative factors were discovered: Club foot, Paralysis, Congenital deformity, Fracture-dislocation of Lisfranc's joint, Sequelae of surgery for hallux valgus, Sequelae of subtaler arthrodesis, "Functional" abnormality.

Immediate Weight-Bearing Compared with Non-Weight-Bearing After Operative Ankle Fracture Fixation: Results of the INWN Pragmatic, Randomized, Multicenter Trial.

BACKGROUND: There has been weak consensus and a paucity of robust literature with regard to the best postoperative weight-bearing and immobilization regime for operatively treated ankle fractures. This trial compared immediate protected weight-bearing (IWB) with non-weight-bearing (NWB) with cast immobilization following ankle fracture fixation (open reduction and internal fixation [ORIF]), with a particular focus on functional outcomes, complication rates, and cost utility. METHODS: This INWN (Is postoperative Non-Weight-bearing Necessary?) study was a prospective, pragmatic, randomized cont

Energetic cost of weight shifting and asymmetric weight-bearing during standing in humans.

During prolonged standing, humans commonly shift their weight and spontaneously alternate between symmetric and highly asymmetric standing, bearing most of their body weight on one leg. Here, we tested the hypothesis that humans adopt asymmetric weight-bearing to reduce metabolic cost by exploiting the sublinear scaling of muscle activation maintenance costs predicted by metabolic models. We measured energy expenditure using indirect calorimetry while participants (n = 20) completed three randomized standing conditions: 1) static symmetric standing, 2) symmetric standing with periodic lateral

Distinct Subsets of Noncoding RNAs Are Strongly Associated With BMD and Fracture, Studied in Weight-Bearing and Non-Weight-Bearing Human Bone.

We investigated mechanisms resulting in low bone mineral density (BMD) and susceptibility to fracture by comparing noncoding RNAs (ncRNAs) in biopsies of non-weight-bearing (NWB) iliac (n = 84) and weight bearing (WB) femoral (n = 18) postmenopausal bone across BMDs varying from normal (T-score > -1.0) to osteoporotic (T-score ≤ -2.5). Global bone ncRNA concentrations were determined by PCR and microchip analyses. Association with BMD or fracture, adjusted by age and body mass index, were calculated using linear and logistic regression and least absolute shrinkage and selection operator (Lasso

Antagonistic compensation as a function of reduced weight bearing.

The principle of antagonistic compensation was presented by RIESENFELD in 1966 to explain the relative shortening and broadening of hypofunctional bones. Few studies have concerned themselves with this phenomenon, even though the causative agent(s) have not been delineated. In this study, RIESENFELD induced the hypofunction by complete unilateral bilateral forelimb amputation. The resulting hypofunctional condition may be attributed to the lack of muscle function or to the lack of weight bearing or both in combination. In the present study weight bearing was eliminated in such a manner as to p

Effect of training on the use of weight-bearing orthoses.

Weight-bearing orthoses have been disigned with the primary goal of reducing or eliminating weight bearing in skeletal structures. In evaluating various designs and adjustments of these orthoses, we found that the training of the subject in the use of his orthosis was one of the most significant factors in controlling weight bearing. Subjects fitted with instrumented ischial weight-bearing and patellar-tendon-bearing orthoses walked over a force plate which measured forces imparted to the floor. The force data from the orthoses and force plate were computer-processed with single frame, multipl

A new upper extremity sparing non-weight bearing orthosis.

OBJECTIVES: Axillary and forearm crutches are commonly utilized in the treatment of foot and ankle injuries. In order to decrease the energy expenditure during mobilization, to prevent upper extremity complications, and to let the upper extremity free for other usages, we designed a new orthosis. The study is conducted to compare walking energy parameters of this newly designed orthosis with the axillary and forearm crutches. MATERIALS AND METHODS: 10 healthy young male volunteers walked on treadmill with three different orthoses in randomized order. Oxygen expenditure, oxygen cost, rate of p

Tibial torque generation in a flexed weight-bearing stance.

Internal and external torque generated about the long axis of the lower extremity was measured in 18 male subjects who were instructed to twist with maximal effort against a fixed footplate containing an instrumented torque cell. Mean torque values ranged from 30 to 71 newton meters (Nm) depending upon the test conditions. Torques recorded during the flexed single-leg stance were 19% to 49% higher than those measured while seated. Values at 45 degrees of knee flexion were 11% to 16% greater than those at 20 degrees. Torques generated while wearing a ski boot were 8% to 11% greater than those r

Weight-bearing symmetry changes after asymmetric surface stiffness walking.

Mechanical gait asymmetry is a prevalent deficit in many forms of locomotion impairment. Although spatial gait asymmetry adaptations can be elicited with split-belt treadmill training, weight-bearing and propulsion asymmetry remain resistant to improvement. As an alternative approach, we tested asymmetric surface stiffness walking to induce neuromotor adaptation of weight-bearing and propulsion asymmetries. We hypothesized that a bout of asymmetric stiffness walking would elicit aftereffects in the form of asymmetries in weight bearing, propulsion, and plantar flexor activity. Twelve healthy y

Weight-Bearing Computed Tomography Analysis of Scapholunate Ligament Injury.

PURPOSE: Weight-bearing computed tomography allows for assessment of carpal alignment and stability under physiologic load conditions, which may allow for detection of subtle dynamic instabilities that may not be apparent in nonweight-bearing images. The purpose of this study was to evaluate carpal position under physiologic load in healthy and scapholunate interosseous ligament (SLIL)-injured wrists using weight-bearing computed tomography. METHODS: Ten healthy controls and 10 participants with arthroscopy (Geisler III or IV) or magnetic resonance imaging-proven SLIL injury underwent compute

Weight-bearing CT Scans in Foot and Ankle Surgery.

Weight-bearing CT (WBCT) scans of the foot and ankle have improved the understanding of deformities that are not easily identified on radiographs and are increasingly being used by orthopaedic surgeons for diagnostic and preoperative planning purposes. In contrast to standard CT scans, WBCT scans better demonstrate the true orientation of the bones and joints during loading. They have been especially useful in investigating the alignment of complex pathologies such as adult-acquired flatfoot deformity in which patients have been found to have a more valgus subtalar joint alignment than in a no
