As human space exploration advances to establish a permanent presence beyond the Low Earth Orbit (LEO) with NASA's Artemis mission, researchers are striving to understand and address the health challenges of living and working in the spaceflight environment. Exposure to ionizing radiation, microgravity, isolation and other spaceflight hazards pose significant risks to astronauts. Determining neurobiological and neurobehavioral responses, understanding physiological responses under Central Nervous System (CNS) control, and identifying putative mechanisms to inform countermeasure development are

The harsh environmental conditions in space, particularly weightlessness and radiation exposure, can negatively affect cardiovascular function and structure. In the future, preventive cardiology will be crucial in enabling safe space travel. Indeed, future space missions destined to the Moon and from there to Mars will create new challenges to cardiovascular health while limiting medical management. Moreover, commercial spaceflight evolves rapidly such that older persons with cardiovascular risk factors will be exposed to space conditions. This review provides an overview on studies conducted

Artificial gravity created by the astronauts themselves, without any external power supply, by pedalling on coupled counterrotating bicycles along the inner wall of the space module (Twin Bikes System, TBS), was previously suggested (Antonutto et al., 1991) to prevent musculo-skeletal decay and cardiovascular deconditioning during long term space flights. To investigate whether this unusual rotating environment would determine abnormal stimulations of the vestibular system due to Coriolis cross coupled accelerations, thus leading to acute motion sickness (AMS), the conditions of a rotating env

Space flight provides a model for the study of healthy individuals undergoing unique stresses. This review focuses on how physiological adaptations to weightlessness may affect nutrient and food requirements in space. These adaptations include reductions in body water and plasma volume, which affect the renal and cardiovascular systems and thereby fluid and electrolyte requirements. Changes in muscle mass and function may affect requirements for energy, protein and amino acids. Changes in bone mass lead to increased urinary calcium concentrations, which may increase the risk of forming renal s

Long-duration spaceflight is associated with pathophysiological changes in the intracranial compartment hypothetically linked to microgravity-induced headward fluid shift. This study aimed to determine whether daily artificial gravity (AG) sessions can mitigate these effects, supporting its application as a countermeasure to spaceflight. Twenty-four healthy adult volunteers (16 men) were exposed to 60 days of 6° head-down tilt bed rest (HDTBR) as a ground-based analog of chronic headward fluid shift. Subjects were divided equally into three groups: no AG (control), daily 30-min intermittent AG

INTRODUCTION: Symptoms of orthostatic intolerance, e.g., following prolonged bed rest and microgravity exposure, are associated with reductions in cerebral blood flow. We tested the hypothesis that spontaneously breathing through an impedance threshold device (ITD) would attenuate the fall in cerebral blood flow velocity (CBFV) during a hypotensive orthostatic challenge and reduce the severity of reported symptoms.
METHODS: While breathing through either an active ITD (-7 cm H2O inspiratory impedance) or a sham ITD (no impedance), 19 subjects performed a squat stand test (SST). Symptoms upon

The weightless environment of space imposes specific physiologic adaptations on healthy astronauts. On return to Earth, these adaptations manifest as physical impairments that necessitate a period of rehabilitation. Physiologic changes result from unloading in microgravity and highly correlate with those seen in relatively immobile terrestrial patient populations such as spinal cord, geriatric, or deconditioned bed-rest patients. Major postflight impairments requiring rehabilitation intervention include orthostatic intolerance, bone demineralization, muscular atrophy, and neurovestibular sympt

In a number of flights, cosmonauts and astronauts have experienced aggravation of their health status and general condition in the initial hours and days in a weightless environment. One of the trigger mechanisms for the onset of these unfavourable conditions at the start of space flight is a redistribution of body fluids and a blood shift towards the head. To ensure controlled hypohydration as a countermeasure to the deleterious effects of 0-g and to investigate the feasibility to control adaptation, six cosmonauts were administered lasix once a day during the first 3 days of a mission. All d

Short-radius centrifugation offers a promising countermeasure to the adverse effects of prolonged weightlessness. Head movements made in a rotating environment elicit Coriolis effects, which seriously compromise sensory and motor processes. We have previously found that, contrary to common belief, participants can adapt to the Coriolis effects associated with single-quadrant yaw head turns during 23-rpm short-radius centrifugation, while maintaining their adaptation to stationary environments. Here, we focus on motion sickness and illusory motion, the most problematic subjective side effects.

Cardiovascular changes occur during spaceflight. Because of the loss of hydrostatic pressure there is, during weightlessness, a fluid shift of extra- and intra-vascular volumes to the upper part of the body. This provokes objective and subjective symptoms, especially in the first days of spaceflight. The fluid shift is perceived by baroreceptors as a relative central hypervolaemia and induces neuro-hormonal mechanisms which provoke a real hypovolaemia. Cardiovascular deconditioning exists during spaceflight with the following symptoms when astronauts return to earth: decreased orthostatic tole

Four payload crewmembers were exposed to sustained linear acceleration in a centrifuge during the Neurolab (STS-90) flight. In contrast to previous studies, otolith-ocular reflexes were preserved during and after flight. This raised the possibility that artificial gravity may have acted as a countermeasure to the deconditioning of otolith-ocular reflexes. None of the astronauts who were centrifuged had orthostatic intolerance when tested with head-up passive tilt after flight. Thus, centrifugation may also have helped maintain post-flight hemodynamic responses to orthostasis by preserving the

In 1975 the Soviet Union performed two manned flights on board the orbital station Salyut 4. The first mission of 30 days was flown by A. A. Gubarev (Commander) and G. M. Grechko (Flight Engineer) and the second mission of 63 days by P.I. Klimuk (Commander) and V.I. Sevastyanov (Flight Engineer). The present paper gives preliminary data on the general health and cardiovascular investigations of the crew members during and after flight. The clinical and physiological examinations of the cosmonauts showed that the work-rest cycles and counter-measures used helped to maintain their good health an

Strict 6° head-down tilt bed rest (HDTBR) induces a chronic headward fluid shift similar to weightlessness that may underlie the development of spaceflight-associated neuro-ocular syndrome. We tested the efficacy of two headward fluid shift countermeasures, using either 25 mmHg lower body negative pressure (LBNP, 2 × 3 h daily, n = 12) or use of venoconstrictive thigh cuffs (VTC) following 1 h of moderate aerobic cycle exercise (Ex + CUFF, 6 h daily for 6 of 7 days, n = 12) across 30 days of HDTBR. The study also included a seated posture group (Upright, 2 × 3 h daily, n = 11), which was condu

As a direct consequence of exposure to microgravity astronauts experience a number of physiological changes, which can have serious medical implications when they return to Earth. Most immediate and significant are the head-ward shift of body fluids and the removal of gravitational loading from bone and muscles, which lead to progressive changes in the cardiovascular and musculoskeletal systems. Cardiovascular adaptations result in an increased incidence of orthostatic intolerance (fainting) post-flight, decreased cardiac output and reduced exercise capacity. Changes in the musculoskeletal sys

-6 degrees head down bed rest (-6 degrees HDBR) 21d were used to simulate weightlessness. 15 healthy males, aged 19-22, were divided into 3 groups with 5 people in each group: control group (-6 degrees HDBR), hypoxia group (-6 degrees HDBR + Low Oxygen), Qigong group (-6 degrees HDBR + Qigong). The results showed that the reduction of weight, perimeter of shank, daily average cardic output, basal cardic output and the pulse pressure of the Qigong group were lower than those of the control group (P<0.05). The basal cardic output and the perimeter of shank of the Hypoxia group reduced significan

In a series of studies, we have examined the effects of exposure to simulated microgravity, varying states of vascular volume, and acute exercise on the function of the carotid-cardiac baroreflex in man. In the first study, exposure to simulated microgravity (6 degrees headdown bedrest) reduced the sensitivity and buffer capacity of the vagal baroreceptor-cardiac reflex mechanisms and this impaired baroreflex function was associated with orthostatic hypotension. Since the reduction in plasma volume during BR was not correlated with impaired baroreflex function, a second study was conducted whi

AIM: Recent results from animal experiments have shown that radix astragali (RA), a traditional Chinese herbal tonic, alleviates muscle atrophy under simulated weightlessness conditions, rendering RA a candidate for human use as a countermeasure against muscular atrophy. Possible cardiovascular side effects have not yet been investigated. We analyzed the effects of RA on the orthostatic stability of healthy men.
METHODS: There were 10 test subjects who were assigned to a double-blinded, randomized crossover design using RA or placebo (PL) for 14 d each, respectively. Test runs were separated

Previous studies of cognitive, mental and/or motor processes during short-, medium- and long-term weightlessness have only been descriptive in nature, and focused on psychological aspects. Until now, objective observation of neurophysiological parameters has not been carried out--undoubtedly because the technical and methodological means have not been available--, investigations into the neurophysiological effects of weightlessness are in their infancy (Schneider et al. 2008). While imaging techniques such as positron emission tomography (PET) and magnetic resonance imaging (MRI) would be hard
