This is a 1-year prospective study to investigate the prognostic value of coping strategies such as catastrophising for persistent pain after a whiplash injury. A consecutive series of 96 patients who were seen in the emergency room in the acute phase after the injury were followed prospectively for 1 year. Age, gender and whether or not pain in the neck preceded the accident was recorded. Cases involving fractures or dislocations of the cervical spine, head trauma or pre-existing neurological disorders were not included. The mean interval between the accident and the initial examination was 3

The authors undertook a case-control study of chronic neck pain and whiplash injuries in nine states in the United States to determine whether whiplash injuries contributed significantly to the population of individuals with chronic neck and other spine pain. Four hundred nineteen patients and 246 controls were randomly enrolled. Patients were defined as individuals with chronic neck pain, and controls as those with chronic back pain. The two groups were surveyed for cause of chronic pain as well as demographic information. The two groups were compared using an exposure-odds ratio. Forty-five

In 1928, more than 80 years ago, Crowe [6] coined the term "whiplash". Since then, there have been numerous scientific attempts to make a clearly defined profile of this injury. Unfortunately, these have all been unsuccessful. The discussion has not yet progressed beyond hypothetical approaches. The term "whiplash", which is such an impressive concept for those affected, still lacks any content. This gaping hole has provided enough scope for varied hypothetical explanations of why some people who have sustained whiplash, despite an initial absence of objective injury characteristics, develop a

Although rare, cerebrospinal fluid (CSF) leaks can prove to be detrimental if severe. Usually secondary to dural punctures, CSF leaks can present as severe headaches, neck pain, blurry vision, confusion, and nausea. However, patients can also be asymptomatic. Due to the rarity and variability in symptom presentation, the diagnosis of these leaks is often missed. We present a case of a 15-year-old female who had been experiencing severe headaches after she hyperextended her neck during a horseback ride. On diagnostic imaging, a CT myelogram confirmed a CSF leak with contrast extravasation along

OBJECTIVES: The objective of this study was to conduct a review investigating the relationship between temporomandibular disorders (TMDs) and whiplash injuries (WIs).
STUDY DESIGN: The authors conducted a search of PubMed/Medline, Cochrane Database, and EMBASE for studies discussing the prevalence, incidence, severity, treatment, and prognosis of TMDs following WIs. English language studies published between January 2010 and March 2016 were included. Study quality was assessed by using the Ottawa-Newcastle scale.
RESULTS: Eight studies investigating TMDs in patients with histories of WI were

Synopsis Recovery from a whiplash injury is varied and complex. Some individuals recover quickly and fully, while others experience ongoing pain and disability. Three distinct patterns of predicted recovery (trajectories) have been identified using disability and psychological outcome measures. These trajectories are not linear, and show that recovery, if it is going to occur, tends to happen within the first 3 months of the injury, with little improvement after this period. Identification of factors associated with poor recovery is accumulating, and since 2000 there have been at least 10 publ

A prospective study of 110 patients was carried out to determine the pathogenic significance of trauma to the upper body in the development of neural compressive irritation at the thoracic outlet. Twenty-nine patients were reviewed as cervical strain injuries (N group), 25 patients as probable neurogenic thoracic outlet syndrome (NTOS) (PT group), 39 patients as definite NTOS (T group), and 17 patients as NTOS associated with cervical disc disease (CD-T group). The time lapse between accident and diagnosis and the duration of treatment were significantly longer in T patients or CD-T patients t

A group of patients suffering major debility after minor whiplash head trauma, seen in one office practice, has been retrospectively studied. Typically, acute neck and upper back aches and headache evolved into a multiple somatic, affective and cognitive dysfunction syndrome. Neuropsychological evaluations noted impairments on tests of cognitive flexibility, non-verbal reasoning, new learning/memory, psychomotor agility, and attention. However, in the subacute period, neurological examination, imaging and clinical electrophysiological studies were unable to localize, structurally or functional

Amongst the many patients with persisting neck pain and headache following cervical injuries are a small number in whom the mechanism is compression of the second cervical nerve root and ganglion. This paper describes the clinical features in 14 patients seen by the author. The main features are unilateral pain in the upper cervical and occipital region, tenderness in the suboccipital region, and diminished sensation in the C2 dermatome. The anatomical basis for this syndrome is discussed and illustrated with dissections from a cadaver.

A series of examinations were carried out to elucidate the characteristics of vertigo due to whiplash injury. In patients with whiplash injury, there are over-excitation of the cervical and/or the lumbar proprioceptors on one hand, and dysfunction of the central nervous system, such as the hypothalamus, the brain stem and the cerebellum, on the other hand. These two etiological factors induce disequilibrium by a trigger-and-target relationship in which the above proprioceptors act as a trigger and the central nervous system acts as a target. This postulate is applicable to the explanation of a

This article is to provide insights into the mechanisms underlying chronic pain from whiplash injury. Studies show that injury produces plasticity changes of different neuronal structures that are responsible for amplification of nociception and exaggerated pain responses. There is consistent evidence for hypersensitivity of the central nervous system to sensory stimulation in chronic pain after whiplash injury. Tissue damage, detected or not by the available diagnostic methods, is probably the main determinant of central hypersensitivity. Different mechanisms underlie and co-exist in the chro

BACKGROUND: In 10-20% of patients with a simple whiplash-injury without severe structural lesions, a chronification of the complaints occurs. The question is whether some unidentified pathogenic factors exist.
SCIENTIFIC QUESTION: Investigations have demonstrated that mandibular and head-neck movements are coordinated and centrally controlled and that a craniocervical dysfunction (CCD) can lead to a temporomandibular dysfunction (TMD) by reflex action and vice versa. This study investigated whether a whiplash-injury can lead to a TMD.
METHODS AND RESULTS: IA total of 187 patients with whipla

Whiplash injury is not only limited to neck injury but also brainstem injury that does not involve direct damage to the neck or head. The symptoms of whiplash injury are polymorphous, with the most common complaints being cervical pain, headache and scapulodynia. Vertigo and dizziness are also reported in 25-50% of the cases. In otoneurologic studies, magnetic resonance angiography (MRA) is used for the evaluation of vertebrobasilar hemodynamics in patients who complain of dizziness and vertigo. It is reported that vertebrobasilar artery insufficiency (VBI) leads to brainstem and cerebellar is

CASE REPORT: A 20-year old female complained of a paracentral scotoma after having suffered a whiplash injury. Eye fundus examination and fluorescein angiography suggested the diagnosis of traumatic retinopathy.
DISCUSSION: There is not a clear explanation to justify the development of traumatic retinopathy after an indirect ocular trauma. Some authors favour a mixed mechanism, mechanic and vascular, to explain these alterations. We consider that a vascular aetiology is the most possible explanation for the development of traumatic retinopathy.

Thirty patients with whiplash injuries were examined 1 to 55 months after the accidents. Pain had appeared on the day of the accident in 24 (80%) of the patients and with different delays in the remainder. The mean pain intensity was 43 mm (SD 26) on a visual analogue scale (VAS). All patients had pain in the neck, 17-33% had headache and 6-17% had pain in various regions of the arms. Thirteen patients (43%) suffered from constant pain, while 17 (57%) had pain-free periods. Muscle tenderness was higher at all tested sites compared with controls. The tolerance level to pressure pain in the inde

OBJECTIVES: The aim of this study was to identify the characteristics of post-traumatic headache after whiplash injury in order to obtain some indications as to the pathogenesis of this particularly frequent clinical entity.
METHODS: A retrospective study of 63 patients who had suffered whiplash injury and who had persistent chronic pain was conducted.
RESULTS: Forty-seven patients complained of headache and pain in the cervical spine and 16 of pain in the cervical spine alone. Among the patients with headache 74% complained of tension-like pain, while only about 10% had identifiable post-tr

Otoneurological and respiratory tests were performed on 32 patients after whiplash injury caused by a rear-end car collision. Oculomotor and cervico-ocular test results were generally normal. In a significantly large number of case, the vestibulo-ocular reflex (VOR) was hyperactive (n = 17; 53%) and the respiratory test results were typical of the hyperventilation syndrome (HVS) (n = 12; 38%). Hyperactive VOR and the HVS occurred significantly more often in combination (n = 7; 22%) than could be accounted for by combined false positivity. Most of the significant findings were due to high relat
