The Influence of Climate Change on Vector-Borne Diseases in a Wilderness Medicine Context.

The imminent climate crisis has been labeled as the biggest health threat humanity must deal with. Vector-borne disease distribution and transmission as well as the population at risk are influenced to a great degree by environmental and climactic factors affecting both the vectors themselves and the causative pathogens. Paired with an increase in worldwide travel, urbanization, and globalization, along with population displacements and migration, elucidating the effects of anthropogenic climate change on these illnesses is therefore of the essence to stave off potential negative sequelae. Two Solitudes: Wilder Penfield, Ewen Cameron, and the Search for a Better Lobotomy.

In the 1940s, Wilder Penfield carried out a series of experimental psychosurgeries with the psychiatrist D. Ewen Cameron. This article explores Penfield's brief foray into psychosurgery and uses this episode to re-examine the emergence of his surgical enterprise. Penfield's greatest achievement - the surgical treatment of epilepsy - grew from the same roots as psychosurgery, and the histories of these treatments overlap in surprising ways. Within the contexts of Rockefeller-funded neuropsychiatry and Adolf Meyer's psychobiology, Penfield's frontal lobe operations (including a key operation on

Ethical, Legal, and Administrative Considerations for Preparticipation Evaluation for Wilderness Sports and Adventures.

Preparticipation evaluations (PPEs) are common in team, organized, or traditional sports but not common in wilderness sports or adventures. Regarding ethical, legal, and administrative considerations, the same principles can be used as in traditional sports. Clinicians should be trained to perform such a PPE to avoid missing essential components and to maximize the quality of the PPE. In general, participants' privacy should be observed; office-based settings may be best for professional and billing purposes, and adequate documentation of a complete evaluation, including clearance issues, shou

Mobility Solutions After a Lower Extremity Fracture and Applicability to Battlefield and Wilderness Medicine.

The potential for delayed evacuation of injured Service members from austere environments highlights the need to develop solutions that can stabilize a wound and enable mobility during these prolonged casualty care (PCC) scenarios. Lower extremity fractures have traditionally been treated by immobilization (splinting) followed by air evacuation - a paradigm not practical in PCC scenarios. In the civilian sector, treatment of extremity injuries sustained during remote recreational activities have similar challenges, particularly when adverse weather or terrain precludes early ground or air resc

137Cs Lake Sediment Depth Profiles and Inventories Within Four Lakes of Idaho's Sawtooth Wilderness.

A total of 68 sediment cores from four freshwater alpine lakes in Idaho's Sawtooth Wilderness were collected during the summer of 2017. The objectives of the study were to determine depth distributions of Cs in the cores using gamma spectrometry and to estimate the sedimentation rates of the lakes from the identified geochronological peaks linked to nuclear fallout. The Cs radioactivity above reference-area values was detected in all studied lakes. The maximum Cs concentrations measured in each lake's sediment ranged from 74.0 ± 6.09 to 255 ± 7.48 Bq kg. Factors such as elevation, latitude, an

Development and Preliminary Evaluation of the Wilder-Addison Narcolepsy Function Scale for Occupational Performance.

Narcolepsy is a neurological sleep disorder affecting occupational functioning; however, occupational therapy is rarely found in the interdisciplinary care of narcolepsy and occupation-based assessments specific to narcolepsy are limited. This study examined the preliminary performance of the Wilder-Addison Narcolepsy Function Scale (WANFS), a performance measure designed to detect functional disruption associated with narcolepsy symptoms. A cross-sectional survey design was employed. Participants with Narcolepsy Type 1 (n = 63), Narcolepsy Type 2 (n = 58), and control participants without nar

Environmental and Ecological Effects of Climate Change on Venomous Marine and Amphibious Species in the Wilderness.

INTRODUCTION: Recent analyses of data show a warming trend in global average air and sea surface ocean temperatures. The atmosphere and ocean have warmed, the amounts of snow and ice have diminished, the sea level has risen, and the concentrations of greenhouse gases have increased. This article will focus on climate change and projected effects on venomous marine and amphibious creatures with the potential impact on human health. METHODS: Retrospective analysis of environmental, ecological, and medical literature with a focus on climate change, toxinology, and future modeling specific to ven

Ocular toxoplasmosis: a 50th anniversary tribute to the contributions of Helenor Campbell Wilder Foerster.

In 1952, Helenor Campbell Wilder (later Helenor Campbell Wilder Foerster) confirmed the growing suspicion that Toxoplasma gondii was a cause of uveitis in otherwise healthy adults by identifying the presence of parasites in eyes enucleated because of severe intraocular inflammation. Ocular toxoplasmosis was previously known to occur only in newborns with congenital T gondii infection. Her report ushered in a new era in the field of uveitis in which toxoplasmosis, rather than tuberculosis, was confirmed to be the most common cause of retinochoroiditis. Fifty years later, issues raised in her la

Reference intervals for trace minerals and heavy metals in livers of cattle in wilderness areas in the Netherlands.

We examined the liver mineral concentrations of semi-wild cattle breeds (Highland, Galloway, Rode Geus) grazing in natural or near-natural flood-prone areas in the Netherlands. These breeds are employed as part of trophic rewilding initiatives aimed at maintaining biodiverse ecosystems through natural grazing patterns. Our primary goal was to derive RIs for liver mineral concentrations for these cattle populations. We collected and analyzed liver samples from 96 cattle from 14 sites in the Netherlands and used inductively coupled plasma mass spectrometry to measure concentrations of trace elem

Wilderness Medical Society Clinical Practice Guidelines for the Treatment and Prevention of Drowning: 2024 Update.

The Wilderness Medical Society convened a panel to review available evidence supporting practices for acute management of drowning in out-of-hospital and emergency care settings. Literature about definitions and terminology, epidemiology, rescue, resuscitation, acute clinical management, disposition, and drowning prevention was reviewed. The panel graded available evidence supporting practices according to the American College of Chest Physicians criteria and then made recommendations based on that evidence. Recommendations were based on the panel's collective clinical experience and judgment

Potential Environmental and Ecological Effects of Global Climate Change on Venomous Terrestrial Species in the Wilderness.

INTRODUCTION: Climate change has been scientifically documented, and its effects on wildlife have been prognosticated. We sought to predict the overall impact of climate change on venomous terrestrial species. We hypothesize that given the close relationship between terrestrial venomous species and climate, a changing global environment may result in increased species migration, geographical redistribution, and longer seasons for envenomation, which would have repercussions on human health. METHODS: A retrospective analysis of environmental, ecological, and medical literature was performed wi

Wilderness Medical Society Clinical Practice Guidelines for the Management of Exercise-Associated Hyponatremia: 2019 Update.

Exercise-associated hyponatremia (EAH) is defined by a serum or plasma sodium concentration below the normal reference range of 135 mmol·L-1 that occurs during or up to 24 h after prolonged physical activity. It is reported to occur in individual physical activities or during organized endurance events conducted in environments in which medical care is limited and often not available, and patient evacuation to definitive care is often greatly delayed. Rapid recognition and appropriate treatment are essential in the severe form to increase the likelihood of a positive outcome. To mitigate the r
