Role of suture material and technique of closure in wound outcome following laparotomy for peritonitis.

BACKGROUND: There are no accepted guidelines for the closure of laparotomy incisions in patients of peritonitis. As these patients differ from the patients undergoing elective abdominal surgery, the same recommendations for closure may not be applicable in both groups. AIM: To compare wound outcome parameters following closure of the laparotomy incision with absorbable and non-absorbable suture material using the continuous and interrupted techniques in patients of peritonitis. METHOD: A single blinded randomised controlled trial using Polygalactin-910 and Polypropylene, number 1 sutures, to

Temporary closure of open abdominal wounds by the modified sandwich-vacuum pack technique.

BACKGROUND: A 5-year experience with the modified sandwich-vacuum pack technique, using an opened 3-litre urological irrigation bag and continuous high-pressure suction, for temporary abdominal wall closure is presented. METHODS: The records of all patients who underwent temporary abdominal wall closure using this method from January 1996 to December 2000 were examined. RESULTS: The modified sandwich-vacuum pack was used 139 times in 55 patients. Forty patients sustained penetrating trauma while 15 patients sustained blunt trauma. The mean Injury Severity Score was 19 (range 9-34). Intra-abd

Use of the Allgöwer-Donati suture technique for wound closure following transmetatarsal amputation.

INTRODUCTION: The Allgöwer-Donati suture technique reportedly has lower incision perfusion impairment compared with other suture techniques. The authors of the present case series hypothesized that suture technique selection to achieve lower incision perfusion impairment may result in improved healing rates and decreased incidence of wound complications in partial foot amputations, specifically TMAs. OBJECTIVE: To evaluate the efficacy of TMA with skin closure completed using the Allgöwer-Donati suture technique. MATERIALS AND METHODS: The records of 7 patients (8 limbs) who underwent closur

Closure technique for leaking wound resulting from thermal injury during phacoemulsification.

UNLABELLED: We describe a method of wound closure with a conjunctival flap in cases of wound burn. A fornix-based conjunctival flap is raised to adequately cover the wound. Then, a single horizontal mattress suture with 10-0 monofilament nylon is passed to appose the wound. The conjunctival flap is hinged to the corneal end of the loop so the conjunctival flap covers the wound when the suture is tied. The wound is thus sealed by conjunctiva with optimum suture tension. This suture was applied in 5 patients with clear corneal incisions who developed wound burn during phacoemulsification. All ha

Wound closure technique for lateral extensile approach to intra-articular calcaneal fractures.

Multiple wound closure techniques have been described for a lateral extensile calcaneal incision in the literature. In this article, a technique is presented that involves a subcutilar closure over a closed drain system, which has proven to be effective in minimizing sural nerve injury and wound dehiscence in open reduction internal fixation of 20 calcaneal fractures.

Dermatotraction: an effective technique for the closure of fasciotomy wounds: a preliminary report of fifteen patients.

Dermatotraction was evaluated as an alternative technique for the closure of dermatofasciotomy wounds, with a review of literature and of our clinical experience. The dermatotraction technique provides closure of fasciotomy wounds and avoids the use of skin grafting. Patients treated with dermatofasciotomy for an acute compartment syndrome of the limbs, without obvious tissue necrosis and without shock or urgent life saving surgery, had their fasciotomy wound closed with dermatotraction with vessel loops, the skin approximation system, or the prepositioned intracutaneous suture. In our experie

Evaluation of wound closure using different incision techniques with 23-gauge and 25-gauge microincision vitrectomy systems.

PURPOSE: To describe the anatomic and histopathologic outcomes using different incision techniques with transconjunctival 23-G and 25-G vitrectomy systems. METHODS: New Zealand rabbits were randomized to either 23-G or 25-G vitrectomy surgeries using angled incisions and straight incisions. After pars plana vitrectomy, the cannulas were removed and 0.1% trypan blue was injected to evaluate for leakage. The animals were killed on day 7 and the eyes enucleated for gross analysis and histopathologic analysis by frozen section. RESULTS: Leakage of trypan blue was noted from 10.8% and 5.7% of str

Primary closure versus the "Volcano" technique (PRIVEVO): a prospective observational study of purse-string wound closure for stoma reversal.

BACKGROUND: Stoma creation is an essential part of general surgery, used in procedures such as protective loop ileostomy formed during low rectal resection or coloanal anastomosis, and in Hartmann’s procedures, often performed in emergency settings. Despite its clinical importance, the optimal wound closure technique following stoma reversal remains controversial. This study aims to identify a more effective wound closure method following stoma takedown. METHODS: Between June 1, 2017, and January 31, 2020, we prospectively observed 75 patients at the University of Debrecen, Hungary, who underw

Sutureless biplanar closure of leaking wounds in micro-incision vitreous surgery: A novel technique.

AIM:: To describe a novel technique for sutureless hermetic closure of leaking scleral wounds in micro-incision vitreous surgery. METHODS:: This study is a two-centre, non-comparative, interventional case series. We studied 1240 scleral incisions on 870 eyes using different gauges (20, 23, 25) to evaluate the effectiveness of a new technique to seal scleral wounds without sutures. If wounds were found leaking after removing the trocar, we used the same trocar (or microvitreoretinal) blade to make an adjacent trans-scleral entry in the same direction as the main incision to access its internal

Wound complications after chemo-port placement in children: Does closure technique matter?

PURPOSE: Wound dehiscence after chemo-port placement is a rare but potentially significant complication. We hypothesize that by using a simple running skin closure technique during chemo-port placement the rate of wound dehiscence and overall wound complications can be significantly decreased. METHODS: IRB approval was obtained and patients <18years that received a tunneled central line with port from June 2012 to April 2016 were analyzed. Data collected on patients included patient demographics, skin closure type, and wound complications within 30days. Chi-square was performed to examine the

Modified sandwich vacuum pack technique for temporary closure of abdominal wounds: an african perspective.

INTRODUCTION: South Africa has very high levels of accidental trauma as well as interpersonal violence. There are more admissions for trauma in South Africa than for any other disease; therefore it can be regarded as the Number 1 disease in the country. Complex abdominal injuries are common, requiring specific management techniques. The aim is to document our experience with the Modified Sandwich Vacuum Pack technique for temporary closure of abdominal wounds. [Results of abdominal wall closure using the 4:1 suture/wound lenght technique].

BACKGROUND: Laparotomy is a daily procedure for the general surgeon and its main complication is the formation of hernias. OBJECTIVE: To determine if the suture length to wound length ratio 4:1 for wall closure decreases the incidence of hernia. MATERIAL AND METHODS: Data from patients (n = 86) in whom abdominal wall closure was performed from August 2017 to January 2018 were prospectively reviewed. Patients who could not undergo adequate follow-up, those managed with open abdomen, or those with use of non-absorbable suture materials were excluded. 2 groups were formed: in one, the suture le

Vessel loop closure technique in open fractures and other complex wounds in the foot and ankle.

A gaping wound of the foot and ankle can be difficult to close. In cases in which wound margin mobility is suitable, the use of a vessel loop, or loops, to provide sufficient tension for wound margin reapproximation can be a useful adjunct to the surgical management of wounds that would otherwise be very difficult to close without the use of a skin graft or flap coverage. In this report, we describe the use of a vessel loop, or loops, for reapproximation of the margins of gaping wounds of the foot or ankle.

Comparison of wound closure techniques after thyroid and parathyroid surgery: an updated systematic review and network meta-analysis.

We designed and conducted the present study to evaluate the optimal wound closure technique after thyroid and parathyroid surgery in terms of perioperative safety, efficacy, and cosmetic outcomes. A systematic literature review and network meta-analysis (NMA) was performed according to the Cochrane Handbook for Systematic Reviews of Interventions and the PRISMA guidelines. Literature screening was completed at 10/12/2021. A random effects multivariate network meta-analysis under a frequentist framework was implemented. Statistical significance was considered at the level of p < 0.05. Overall,

Gradual Elastic Suture (Shoelace Technique) Approximation and Platelet-Rich Plasma Infiltration Technique in the Closure of Open Fracture Wound and Infected Wound With Significant Skin Loss.

Infected wounds can be really hard to manage in cases of open fractures, chronic osteomyelitis, and superficial infection. When the skin is damaged, bacteria can quickly enter the underlying tissue and cause a potentially fatal infection. Regular wound dressing with antimicrobial agents has become available in vials as a way to decrease the chance of bacterial colonization and infection and speed up the healing of wounds. In this report, we discuss the shoelace suture technique and platelet-rich plasma (PRP) infiltration in the wound. Due to its ability to stimulate and fasten the healing of w

V-Y fasciocutaneous flap closure technique is a safe and efficacious alternative to primary closure of the perineal wound following abdominoperineal resection.

BACKGROUND: The aim of this study was to compare wound complications from V-Y flap vs primary closure in the setting of abdominoperineal resection. METHODS: This was a single institution retrospective review (1999-2014). The main outcome measures were any wound complication, hospital length of stay, and unplanned readmissions. RESULTS: Among 80 patients included, 21 (26%) received reconstruction with V-Y flap. Compared with those who received primary closure, patients who underwent V-Y flap reconstruction had lower rates of overall wound complications (14.3% vs 64%, P = .002), superficial su

Prediction of incisional hernia after kidney transplantation: analysis of wound closure technique and risk factors.

PURPOSE: Incisional hernia (IH) is a common complication after kidney transplantation, impacting morbidity and quality of life. This retrospective study aimed to identify IH risk factors and develop a predictive model. METHODS: We retrospectively analysed 667 adult kidney transplant recipients (2010-2017) from two transplant centres. Medical records were screened for symptoms of abdominal wall impairment, postoperative CT scans assessed, and factors associated with IH analysed. Using the Penn Hernia Calculator, hernia probability was calculated. Adult kidney recipients transplanted 2018-2019
