Chronic pain in patients with gunshot wounds.

BACKGROUND: In civilian life, from 11 to 40% of patients suffer from chronic pain after receiving injuries. There are almost no data on chronic pain in patients with gunshot wounds, isolated clinical cases have been published. The purpose of our study is to determine the factors that can potentially affect the results of treatment of such patients, namely the frequency of development of chronic pain, acute stress reactions, satisfaction with the results of treatment and the number of wound localizations. METHODS: The treatment of 769 patients was analyzed. Pain intensity was diagnosed using a

Expedited treatment of lower extremity gunshot wounds.

BACKGROUND: Patients with isolated lower extremity gunshot wounds are currently admitted for observation and often undergo angiography. We hypothesized that if such patients have a normal ankle-brachial index (ABI), they can be discharged safely from the emergency department without invasive imaging or admission. STUDY DESIGN: We retrospectively reviewed the records of hemodynamically stable patients with isolated lower extremity gunshot wounds seen at our urban, university-based trauma center and who were discharged from the emergency department. Evaluation consisted of determining which pat

Gunshot wounds to the face--acute management.

The complex facial trauma victim poses a genuine therapeutic challenge as a whole, and may be particularly challenging to the medical team. The literature on acute management of gunshot wounds to the face is scarce. We performed an extensive review of the English-language literature in an effort to better delineate the diagnosis and acute management of these injuries. Most of these injuries do not present with initial threat to life and can safely be managed non-operatively. Definitive treatment is often deferred in patients with multiple, or more severe, injuries. Airway compromise is the mos

Findings in gunshot wounds from tandem projectiles.

Tandem projectiles are unusual events which can cause erroneous case interpretations if one is unfamiliar with them. This paper presents two such cases. The first case involved three .32 caliber bullets, two of which became lodged in a revolver barrel because of faulty ammunition. A third fired bullet subsequently pushed itself and the lodged bullets out of the barrel resulting in a single entrance wound with three bullets in the body. The second case involved a 20 gauge shotgun shell which apparently was inadvertently loaded into a 12 gauge shotgun. This resulted in an unusual entrance wound

[Diagnostic and therapeutic problems in gunshot wounds].

BACKGROUND: Bullet wounds are a rare occurrence during times of peace. Recently, however, there has been a general increase in the relative number and severity of this type of trauma. In times of peace, gunshot wounds may be mainly caused by suicide attempts, negligent handling of firearms and especially violent conflicts. Bullet wounds, in contrast to wounds caused by a blow or impact to the viscerocranium, are characterised by an irregular path, entry and exit wounds, as well as localised demolition of bones with the associated defects. PATIENTS: 30 patients with gunshot wounds were treated

Management of gunshot wounds to the tibia.

The management of tibial gunshot wounds is unique because of the close proximity of the medial aspect of the bone to the skin. The minimal soft-tissue sleeve covering the medial tibia allows bullets to enter medially and strike the bone with greater energy and allows surface contaminants to reach the bone readily unlike areas such as the thigh where soft tissue can dissipate much of the energy. The consequence of this anatomic difference may affect infection rates, fracture stability, and surgical options. For these reasons, tibial gunshot wounds should be treated independently from other guns

Civilian gunshot wounds to the brachial plexus.

Many gunshot wounds (GSW's) to the brachial plexus do not improve spontaneously with time and are therefore candidates for surgery. Over an 18-year period, 141 patients with GSW's were evaluated, 90 of whom were operated on; 75 of the surgical cases were followed for 2 years or more. Thirty operative patients had initial vascular repair, while eight required thoracotomies. Total plexus palsy was present in 19 of those selected for operation. The average interval between injury and operation was 17 weeks. Six patients required early operation for an expanding aneurysm with progressive neural lo

[Maggot-induced postmortem changes simulating gunshot wounds].

In the warm season, human cadavers often are the breeding ground of fly larvae. In special conditions, the damage by maggots can take on the appearance of bullet holes. Preexisting injuries allow the maggots to penetrate the skin also in parts other than the body apertures commonly infested. Round tissue damage similar to gunshot-wounds are favored by the fact that the larvae of some types of flies bundle together to form feeding communities. The enzymes from the salivary glands of the fly larvae may in some rare instances even lead to perforation of bone lamellae (e.g. in the region of the or

Missile tract infections after transcolonic gunshot wounds.

Severe invasive infections around missiles or missile tracts may occur after transcolonic gunshot wounds. Observations in seven patients disclosed that infections of bone, soft tissue, retroperitoneum, and vascular structures resulted from the contamination of these tissues by missiles that had passed through the lumen of the colon. The infections were frequently obscure and difficult to diagnose unless the missile was considered to be a potential source of contamination. Two of seven patients died as a direct result of infection. Aggressive debridement of missile tracts, removal of accessible

Gunshot wounds of the extremities in children.

A ten-year review of gunshot trauma in Manitoba was prompted by an increasing incidence in gunshot trauma in children. Of 54 children treated from 1964 through 1973 60% sustained injury to one or more extremities. Fifteen children sustained long bone fractures, usually complete and often comminuted. All fractures united and there was only one instance of osteomyelitis. The major complication in gunshot wounds to the skeletally immature was limb length inequality secondary to epiphyseal plate injuries. This complication occurred in nine children. In three children there was in epiphyseal growth

Gunshot Wounds Causing Distal Arterial Bullet Embolisms.

We report two cases involving small-caliber gunshot wounds to the chest with embolization of the bullet which complete occluding arterial circulation into the left lower extremity. A 30-years-old and 19-years-old men suffered gunshots wound to the thorax and abdomen with subsequent arterial embolisms into their left legs. Image studies revealed the left popliteal and femoral arteries occlusion by the missiles. Arteriotomies were auspiciously performed to retrieve the projectiles along with Fogarty catheters thrombectomies which conclude successful outcomes. At a 6 and 36 months' follow-up, the

Current treatment procedures for civilian gunshot wounds.

INTRODUCTION: This work provides an overview of the incidence of gunshot wounds during peace conditions in a civilian population and aims to assess the principles of their treatment. METHOD: We evaluated a total of 104 patients with gunshot wounds with an average age of 38.7 years (range 1871). 84 men (80.8%) and 20 women (19.2%) were involved. The head was affected 7 times (6.7%). Out of those, penetrating injury occurred only once (1.0%). The throat was hit three times (2.9%). The chest was injured 15 times (14.4%), penetrating injury at this site was observed in 10 cases (9.6%). In five ca

Pediatric intracranial gunshot wounds: the Memphis experience.

OBJECTIVE Penetrating brain injury in civilians is much less common than blunt brain injury but is more severe overall. Gunshot wounds (GSWs) cause high morbidity and mortality related to penetrating brain injury; however, there are few reports on the management and outcome of intracranial GSWs in children. The goals of this study were to identify clinical and radiological factors predictive for death in children and to externally validate a recently proposed pediatric prognostic scale. METHODS The authors conducted a retrospective review of penetrating, isolated GSWs sustained in children who

Inpatients with gunshot wounds to the face.

Microvascular free tissue transfer (FTT) is an increasingly used method of reconstruction for traumatic defects of the head and neck. We describe the immediate management, FTT reconstruction techniques, and outcomes of 6 individuals who sustained maxillofacial gunshot trauma and were treated at a single tertiary-care level I trauma center. All 6 patients were white men with a mean age of 33 years. The mandible, nose, and orbital contents were the most frequently affected critical structures. All patients initially underwent primary wound debridement and tracheostomy, with concurrent maxilloman

Multiple thoracoabdominal gunshot wounds with complicated trajectories.

A 41-year-old man was shot three times with a custom-made handgun. There were six small round wounds on the surface from two bullets that went into the body and one bullet found in a wall behind the victim at the scene. The gunshot wound in the left supraclavicular region bled massively. He was transferred to our hospital and soon fell into shock. A chest radiogram showed right hemothorax. Resuscitation and blood transfusion were started, and right thoracic drainage was immediately undertaken. The left subclavicular region was incised, and bleeding from the left subclavian vein stopped. Right

[Adequate management of stab and gunshot wounds].

BACKGROUND: It is difficult to verify the treatment of stab and gunshot wounds with prospective randomized studies. That is why the results of observational studies are so important. MATERIAL: From 1 January 1989 to 31 December 1998, we saw 74 patients because of stab (64) and gunshot (ten) wounds. RESULTS: Most of the patients, whose injuries were caused mainly for criminal reasons (criminal 54, autoaggressive 14, accidental 4, unknown 2), came to hospital on weekends. We saw 38 abdominal, 23 thoracic, and 13 mixed injuries. On average, 3.8 h passed between the time of injury and getting fi

When to operate on abdominal gunshot wounds.

Historically, penetrating abdominal trauma was managed expectantly until the late 19th century. In World War I, with the high mortality and morbidity associated with penetrating abdominal trauma, operative management replaced expectant management. It was soon realized that not all penetrating abdominal injuries required an operation. Since the 1960's, selective nonoperative management of stab wounds to the anterior abdomen has become the standard of care. However, gunshot wounds to the abdomen are still treated by mandatory exploration based on an allegedly high incidence of intra-abdominal in

Facial gunshot wounds: a 4-year experience.

PURPOSE: Facial gunshot wounds can result in devastating functional and aesthetic consequences for patients. In an attempt to evaluate the management and outcome in these patients, a 4-year retrospective review was undertaken of all patients presenting with facial gunshot wounds at a level I trauma center. PATIENTS AND METHODS: A total of 121 patients were identified. Medical documentation could be obtained on 84 of those patients. The patients' maxillofacial injuries were treated by the 3 participating services: plastic surgery, oral and maxillofacial surgery, and otorhinolaryngology. The pa

Gunshot wounds to the male external genitalia.

We report on 23 patients with genital gunshot wounds treated since 1977. Of these 23 patients 16 received 1 shot and 7 received multiple shots. The bullet affected the scrotum in 12 patients, the penis in 4, and the penis and scrotum in the remaining 7. The same missile also compromised the thigh in 15 patients, with major vascular damage in 2. Management was based on serial clinical evaluation and aggressive imaging, related to the missile trajectory. Staging studies were 100% accurate in the patients in whom they were done. Nine patients had damage to the testis (7, 1 bilaterally) and/or spe
