Brazilian Trauma Society's (SBAIT) management guidelines of splenic injuries.

The splenic injuries are frequent among victims of blunt trauma, and the most appropriate approach must be based on the physiological status and anatomical complexity of the lesions balance. The alternatives must balance, in stable patients without peritonitis, the simplicity and prompt resolution of splenectomy and the potential complications and immunobiological consequences, considering modern minimally invasive diagnostic and therapeutic tools. This paper presents the Brazilian Trauma Society's (SBAIT) management guidelines, based on the most recent evidence and tailored to the reality of

Extrahepatic bile duct injury in blunt trauma: A systematic review.

BACKGROUND: Extrahepatic bile duct injuries (EHBDIs) are a rare consequence of blunt abdominal trauma. The purpose of this study was to establish mechanisms of injury, clinical indicators of EHBDI following blunt trauma (both with investigative modalities and intraoperatively), method and timing of injury detection, and definitive treatment options. METHODS: A systematic review was performed to gather data on patients with an EHBDI secondary to blunt trauma. Three databases (MEDLINE, PubMed, and EMBASE) were searched to July 19, 2018. RESULTS: Our systematic review included 51 studies, compr

[Blunt tracheal transection].

Blunt tracheobronchial injuries are rare, but can be life-threatening. A precise preoperative diagnosis and well recognized plan of surgical treatment, which may be unique for each patient are needed to restore the continuity of tracheobronchial tree in a one-stage intervention. We encountered 2 patients with complete tracheal transection of neck and 1 patient with complete tracheal transection in mediastinum and 15 cm tear in the posterior membranous trachea, whose tracheal injury was difficult to repair using direct intubation of distal airway by bronchoscopy. We achieved a good result of re

Detecting blunt pancreatic injuries.

Pancreatic injury after blunt abdominal trauma is exceedingly uncommon, occurring in less than 5% of major abdominal injuries. When blunt pancreatic injury does occur, however, it is notoriously difficult to identify. The use of serum amylase has been advocated in the diagnosis of such injury, yet it is neither sensitive nor specific. Computed tomography has become widely accepted in the evaluation of hemodynamically stable patients after blunt abdominal trauma, although it is clearly not a sensitive modality for the detection of pancreatic injury. In fact, numerous examples of normal CT scans

Long-term sequelae following blunt thoracic trauma.

People experiencing blunt thoracic trauma may sustain multiple rib fractures, flail chest, cardiac or pulmonary contusions, injury to the great vessels, sternal fractures, clavicular fractures, neck injuries, and lacerations of the liver and/or spleen. Long-term sequelae from blunt chest trauma include chest wall deformities, persistent dyspnea, and cardiac, neurologic, or esophageal complications. Chronic pain, depression, and loss of functional status are also frequent components of recovery from trauma.

Blunt abdominal trauma in children.

Blunt abdominal trauma is the commonest cause of intra-abdominal injuries in children. The use of computerized axial tomography and non-operative management of haemoperitoneum are two significant developments in the last two decades in the management of blunt abdominal trauma in children. The concept of non-operative management was introduced in late 1979 and wherever possible remains the optimum treatment. Computerized tomography scan for paediatric abdominal trauma was first described in 1980 and remains the investigation of choice. There is no substitute, however, for a good history, astute

Blunt injury to the supra-aortic arteries.

BACKGROUND: Blunt trauma causing injury to the vessels of the aortic arch is uncommon but may be attended by serious consequences. Most surgeons will experience only an occasional case and will need to rely on published literature for guidance. METHODS: A Medline search over 1986-1995 was carried out using the following keywords: brachiocephalic trunk, common carotid artery and subclavian artery; injury was used as a subheading. RESULTS AND CONCLUSION: After the aortic isthmus, the innominate is the most commonly injured artery in the chest. Whatever the site of an arterial lesion, however,

Ultrasonic study of hepatic and splenic traumatic lesions.

Traumatic lesions involving the parenchyma of liver and spleen give heterogeneous echo patterns on sonograms. Associated haematomas appear as sonolucent or sonotransparent areas. Subcapsular haematomas appear as peripheral sonolucent or sonotransparent areas. High resolution sectorial real-time ultrasound demonstrates such abnormalities. When examining for splenic or hepatic lesions, associated lesions such as renal contusion, juxta-renal or retroperitoneal haematoma and haemoperitoneum are sought. Haemoperitoneum may be shown in Morrison's pouch, in the vicinity of the spleen (moon-crescent s

[Complications and sequelae of blunt renal trauma: the problem of terminological differentiation of concept].

General issues related to the conceptual distinction between the terms "complication" and "sequela" in renal injuries are addressed in the article. Selected principles of medical terminology from the perspective of linguistic description of the terms in question are analyzed. In addition, an analysis of synonymy among their English equivalents is presented. The need to systematize term types based on logical genus-species definitions is discussed from the standpoint of domain experts in order to enable effective expert assessment, adequate representation of scientific knowledge and viewpoints,
