Outcomes of stab wounds presenting to Kamuzu Central Hospital in Malawi.

INTRODUCTION: Injuries are a leading cause of morbidity and mortality worldwide, necessitating that we understand the local burden of injury to improve injury-related trauma care and patient outcomes. The characteristics, outcomes, and risk factors for mortality following stab wounds in Malawi are poorly delineated. METHODS: This is a retrospective, descriptive analysis of patients presenting to Kamuzu Central Hospital in Lilongwe, Malawi, with stab wounds from February 2008 to May 2018. Univariate and bivariate analyses were performed to compare patient and injury characteristics based on mo

Selective Management of Multiple Anterior Abdominal Stab Wounds: Is it Safe?

BACKGROUND: Selective management of stable patients with anterior abdomen stab wounds (AASWs) has become a gold standard management approach throughout the world. Evidenced-based options for supporting selective management include clinical follow-up, local wound exploration with or without diagnostic peritoneal lavage, diagnostic laparoscopy, and abdominal computerized tomography. The presence of multiple AASWs might signify a more aggressive attack and limit the safety of a selective management approach. OBJECTIVES: To evaluate whether multiple AASWs are associated with an increased risk of

Benefits of CT tractography in evaluation of anterior abdominal stab wounds.

INTRODUCTION: The study aimed to identify the presence of peritoneal penetration in management of anterior abdominal stab wound by using computed tomography (CT) tractography. MATERIAL AND METHODS: Hemodynamically stabile, CT tractography-performed patients who were admitted to our emergency clinic with anterior abdominal stab wounds between the years 2012 and 2014 were included in this study, and all images were evaluated in terms of peritoneal penetration and possible intra-abdominal injury. RESULTS: In the study CT tractography identified necessity of laparotomy accurately in 90% of the p

Abdominal stab wounds: diagnostic peritoneal lavage criteria for emergency room discharge.

OBJECTIVE: To prospectively evaluate a method for management of abdominal stab wounds that allows for immediate emergency room discharge. METHODS: Anterior abdominal stab wound patients were prospectively placed in a study trial during a 48-month period. Consent was obtained for all patients before study entry. Anatomic boundaries for abdominal stab wounds were costal margins, inguinal ligaments, and anterior axillary lines. Hemodynamically stable patients with negative physical examinations were entered in the study and evaluated with closed diagnostic peritoneal lavage (DPL). Patients with

Management of stab wounds to the thoracoabdominal region. A clinical approach.

We studied 324 patients admitted to Harlem Hospital Center from July 1981 to June 1986 with stab wounds of the thoracoabdominal region (area limited by a coronal circle through the fifth interspaces anteriorly and seventh interspaces posteriorly from above to a subcostal circle 5 cm caudad to the costal margins and 12th ribs from below). We divided this region into 12 zones (six symmetrical zones on each side) using the above upper and lower limits and the costal margins, the midlines, and the anterior and posterior axillary lines. This study was conducted to determine the incidence of transdi

Stab wounds involving the brachial plexus. A review of operated cases.

Of 64 patients with stab wounds involving the brachial plexus operated on by one surgeon, 18 were followed up in detail, with a view to reviewing operative techniques, results and the lessons to be learned. Primary nerve grafting produced better results than end-to-end repair, even in fresh cases. The recognition of nerve compression by a false aneurysm is important, since in these cases, vascular repair alone may not give recovery and neurolysis may be necessary. The overall results of operation were good; lesions of C5 and C6 roots recovered better than those of more distal roots.

Stab-wounds and other penetrating injuries of the abdomen and thorax.

A series of 100 penetrating wounds of the abdomen and thorax is reviewed. Eighty-three were due to stab-wounds. The recent increase in incidence is confirmed and the distribution throughout the day and months of the year is given. The results and methods of treatment of thirty-six abdominal, ten thoraco-abdominal and thirty-seven thoracic wounds are given and described. An operative approach to abdominal wounds is supported. Conservative measures, including more frequent use of intercostal-tube drainage for pneumothorax and haemothorax, are satisfactory for the majority of chest wounds. Cardia

Cervical Spine Immobilisation Following Stab Wounds to the Neck Is Unnecessary.

OBJECTIVE: This study reviews our experience with penetrating neck injury (PNI) secondary to a stab wound (SW) to clarify the incidence of cervical spinal injury (CSI) and to interrogate the role of cervical spinal motion with cervical collar in these patients. METHODS: A retrospective review of a prospectively captured data set was conducted at the Pietermaritzburg Metropolitan Trauma Service in South Africa from December 2012 to December 2022. All patients who presented with a PNI due to a SW were included. RESULTS: There were 1249 cases of PNI over the 10 year study period. 1028 were due

Singular stab wounds to the trunk: Is this suicidal or homicidal?

Differentiating homicidal or suicidal deaths in presence of a singular stab wound to the anterior or lateral trunk is still a challenge in forensic practice. There are numerous criteria in the literature and in current forensic textbooks to distinguish between self-inflicted injuries and homicide. The applicability of these criteria in single stab injuries was examined by elucidating 12 suicides and 33 homicides, each with a single stab injury to the anterior or lateral trunk and were largely confirmed. An instrumentality still stuck in the corpse was always associated with a suicide in the gi

Suicide by skull stab wounds: a case of drug-induced psychosis.

Suicide by stabbing to the head and/or driving sharp objects into the skull is of extreme rarity. This article reports the case of a 27-year-old man, who committed suicide by multiple knife stabs and cuts to the head, the torso, one shoulder and the forearms. Autopsy showed a perforating wound of the skull and the 10-cm long broken blade of the knife being still embedded in the right temporal lobe of the brain. The deceased had no history of psychiatric illness but was currently treated by mefloquine, a quinine derivative associated with a high rate of psychiatric adverse effects. Toxicologica

Mesenteric calcification following abdominal stab wound.

INTRODUCTION: Heterotopic ossification (HO) refers to the formation of bone in non-ossifying tissue. Heterotopic mesenteric ossification is a rare form of HO that is characterized by the formation of an ossifying pseudotumour at the base of the mesentery, usually following abdominal surgery. PRESENTATION OF CASE: We describe a case of mesenteric HO in a young male who presented for elective ventral incisional hernia repair following a stab wound to the abdomen requiring exploratory laparotomy 21 months earlier. Preoperative workup was unremarkable, but a hard, bone-like lesion was noted to en

Stab wound to the chest with acute pericardial tamponade.

A 21-year-old male presented to the trauma unit after he was stabbed once with a knife in the left anterior chest. The patient was awake and combative, very intoxicated, with a systolic blood pressure of 90 mmHg by palpation and a heart rate of 110 beats per min. Physical examination revealed distended neck veins, a single 1.5 cm stab wound to the left parasternal area at the third ICS (noted by clip on x-ray, Fig. 1), poorly auscultated heart sounds, and palpable femoral pulses that went away with inspiration. Chest radiograph (Fig. 1) showed a "pear-shaped" cardiac silhouette, though not mar

Isolated paraplegia from a remote stab wound.

A stab wound to the right flank of a 24-year-old man produced immediate paralysis of both legs but spared sensation. Recovery was slow but sufficient to allow walking with a cane. Radicular artery interruption, in combination with systemic hypotension, is presumed to have caused selective watershed infarction of the spinal cord. Strokes of the spinal cord after trauma are rare, and those at a thoracic level usually represent delayed infarctions after pediatric chest or abdominal injuries. Our patient sustained an immediate and highly selective infarction.

Hybrid endovascular and open surgical management of retained stab wounds: illustrative case.

BACKGROUND: Penetrating brain injuries (PBIs) are rare cases of traumatic brain injuries and are associated with high mortality. Initial management for PBIs focuses on airway, circulation, and cranial imaging evaluation. In the next step, physical examinations are essential, including understanding the mechanism of injury, conducting a thorough neurological assessment, and reviewing medications. Imaging techniques such as CT and CT angiography are essential for assessing the extent of the injury and predicting outcomes. OBSERVATIONS: A 30-year-old male presented with a PBI in the left eye. A

Is diagnostic laparoscopy necessary in the management of left thoracoabdominal stab wounds?

BACKGROUND: The diagnosis of a diaphragmatic rupture and the identification of patients for surgical repair is challenging despite current diagnostic algorithms and imaging technologies. Unless treated on time, acute traumatic diaphragmatic injury due to stab wounds has a high mortality and morbidity rate, with an increasing trend in the presence of organ herniation. In this study, we aimed to investigate the efficacy of diagnostic laparoscopy in patients with an anterior thoracoabdominal stab wound and to compare the follow-up outcomes of cases. METHODS: We retrospectively reviewed our insti

Development of Diaphragmatic Hernia in Patients with Penetrating Left Thoracoabdominal Stab Wounds.

BACKGROUND: This study aimed to investigate the consequences of repairing versus not repairing diaphragmatic injury caused by penetrating left thoracoabdominal stab wounds. METHODS: Diagnostic laparoscopy was performed to evaluate the left diaphragm in patients with penetrating left thoracoabdominal stab wounds who did not have an indication for emergency laparotomy. Patients who did not consent to laparoscopy were discharged without undergoing surgery.
