A patient presented with a complex penetrating facial wound by high temperature steel. The hot steel penetrated right temple, ethmoid bone, and maxillary sinus, and then exited from his left cheek. He kept his right eye but lost his sight. For functional and esthetic considerations, treatments were provided in a staged procedure. First, the debridement was performed under the endoscope. The dead bone was removed, broken teeth were extracted, and necrotic tissue was cleaned. The exposed wounds were cleaned, and dressing was changed daily until the exudation was widely reduced. Then, a 50-mL exp

Visceral injuries, wound infection and sepsis were investigated in 226 inpatients who sustained electrical burns over a period of 15 years. Four patients who sustained thoracic and abdominal organ injuries were noted in this series. The patients had injuries of the small intestine, stomach, colon and the lung. All the patients received operative treatment. Two of them died of sepsis. Injuries to the internal organs should always be considered following high-voltage injuries, and they should be managed as early as possible. The data concerning wound infection and sepsis following electrical inj

A number of studies have recently demonstrated the effectiveness of CO2 laser irradiation for the repair and regeneration of scar tissue from injuries or surgical wounds. However, such studies of the oral mucosa are highly limited. Previous studies using CO2 laser irradiation have indicated that two factors contribute to esthetic healing, namely, artificial scabs, which are a coagulated and carbonized blood layer formed on the wound surface, and photobiomodulation therapy (PBMT) for suppressing wound scarring and promoting wound healing. This review outlines basic research and clinical studies

Orthopedic costs associated with gunshot wounds (GSWs) totaled approximately USD 510 million from 2005 to 2014. Previous studies have identified differences in injuries associated with self-inflicted (SI) GSWs; however, there remains a gap in understanding injury patterns. This study aims to expand upon the current literature and shed light on injury patterns and outcomes associated with SI vs. non-self-inflicted (NSI) GSWs. This is a retrospective cohort study of upper extremity GSWs from January 2012 to December 2022. Data were analyzed using the two-sample t-test, Pearson's chi-squared test

This paper reports the pathologic and functional changes of the sciatic nerve indirectly injured by firearms and its effect on wound healing. Twelve dogs were used. Their hind legs were shot with 1.03-gram steel spheres, avoiding direct injury to the sciatic nerve. The experimental animals were divided into two groups according to the impact velocity, that is, a 1,500 m/sec high velocity projectile (HVP) group and a 400 m/sec, low velocity projectile (LVP) group as a control. After wounding, the same procedure was followed in all dogs. At intervals, the tissues of the wound track were excised

OBJECTIVE: Firearm injuries are now the leading cause of death in children and adults younger than 25 years in the United States and are associated with high morbidity and mortality. However, literature on penetrating cerebrovascular injuries (PCVIs) after intracranial gunshot wound (GSW) injury in the pediatric population is limited. The authors investigated the incidence, radiological characteristics, and clinical outcomes associated with PCVI in children and young adults with intracranial GSW injuries.
METHODS: This was a retrospective cohort study of patients younger than 21 years admitte

This study evaluated the safety and feasibility of early aquatic physical therapy (APT) for gunshot wounds (GSWs) and/or blast injuries (BIs). Early aquatic physical therapy (eAPT) is defined as "therapy conducted within the first month after injury, considering the injury type, setting, and environment." APT facilitates the restoration of mobility and function after illness or injury. Although APT is helpful for patients with mobility issues, muscle weakness, and pain, its early use in high-functioning adults recovering from ballistic trauma remains underexplored. Usually, multidisciplinary r

BACKGROUND: This study reviews our experience with combined cardiac and abdominal stab wounds over 12 years and reviews how changes in technology and clinical approaches have impacted our management of these patients.
MATERIALS AND METHODS: A retrospective cohort study was conducted from January 2008 to January 2020 at a major trauma centre in South Africa. All patients with concurrent SWs to the chest and the abdomen and required both a thoracotomy for cardiac injury and a laparotomy for an intra-abdominal injury at the same setting were included.
RESULTS: Twenty-two cases were identified (

The optimal timing of surgical intervention for peripheral nerve injuries (PNIs) after gunshot wounds (GSWs) is controversial. Although delayed intervention has traditionally been preferred, recent reports suggest earlier intervention may improve outcomes. This study compares early (≤ 3 months) versus delayed (> 3 months) surgical management of PNIs following GSWs. We retrospectively reviewed patients treated at a tertiary academic center between 2000 and 2023 who underwent surgical intervention for PNIs secondary to GSWs. A systematic review was conducted following PRISMA guidelines. PubMed,

INTRODUCTION AND IMPORTANCE: Cardiac injuries are associated with high mortality rates and most affected individuals succumb to their injuries before arrival to the hospital. Even though they have a higher fatality rate, penetrating cardiac injuries are relatively easy to diagnose and they have straightforward management protocols. On the other hand unexpected non-penetrating cardiac injuries, especially in haemodynamically stable patients, are not looked out for and can be difficult to diagnose. They may have a delayed presentation leading to poor patient outcomes. Clinicians should have a hi

The management of blast-related soft tissue wounds requires a comprehensive surgical approach that acknowledges extensive zones of injury and the likelihood of massive contamination. The experiences of military surgeons during the last decade of war have significantly enhanced current understandings of the optimal means of mitigating infectious complications, the timing of soft tissue coverage attempts, and the reconstructive options available for definitive wound management. Early administration of antibiotics in the setting of soft tissue wounds and associated open fractures is the single mo

OBJECT: Gunshot wounds (GSWs) to the cervical spine have been examined in a limited number of case series, and operative management of this traumatic disease has been sparsely discussed. The current literature supports and the authors hypothesize that patients without neurological deficit need neither surgical fusion nor decompression. Patients with GSWs and neurological deficits, however, pose a greater management challenge. The authors have compiled the experience of the R Adams Cowley Shock Trauma Center in Baltimore, Maryland, over the past 12 years, creating the largest series of such inj

BACKGROUND: Injury activates a cascade of local and systemic immune responses.
METHODS: A literature review was undertaken of lymphocyte function in wound healing and following injury.
RESULTS: Lymphocytes are not required for the initiation of wound healing, but an intact cellular immune response is essential for a normal outcome of tissue repair. Injury affects lymphocyte immune mechanisms leading to generalized immunosuppression which, in turn, increases host susceptibility to infection and sepsis. Although the exact origin of post-traumatic immunosuppression remains unknown, stress hormo

BACKGROUND: Missed diaphragmatic perforation caused by penetrating trauma can lead to subsequent strangulation of a hollow viscus, which has prompted the use of invasive diagnostic procedures to exclude occult diaphragmatic injuries in asymptomatic, high-risk patients. The objective of this study was to determine the incidence of occult diaphragmatic injuries caused by stab wounds of the lower chest and upper abdomen, and to examine the natural history and consequences of missed diaphragmatic injuries.
METHODS: On the basis of patient data from two previous randomized studies from our institu

War chest wounds are very common, but wound balistic notions are not known of physicians. Different threats are being listed, and we might retain the important rate of shrapnels wounds. Concerning the bullet behavior in soft tissue, one can discern full jacketed war bullets which may tumble after a variable "neck", and non jacketed missiles which cause wound through "mushrooming" and/or fragmentation effect. Buckshot wounds obey the rule of "all or none". Body reactions, particularly the clash with a hard material like bone, may overturn everything described in soft tissues. Every kind of poss
