Xylazine and Overdoses: Trends, Concerns, and Recommendations.

Xylazine is a nonopioid veterinary anesthetic and sedative that is increasingly detected in the illicit drug supply in the United States. Data indicate a striking prevalence of xylazine among opioid-involved overdose deaths. The emergence of xylazine in the illicit drug supply poses many unknowns and potential risks for people who use drugs. The public health system needs to respond by increasing testing to determine the prevalence of xylazine, identifying its potential toxicity at various exposure levels, and taking mitigating action to prevent harms. Currently, there is little testing capabl

Use of Xylazine in Drug-Facilitated Crimes.

Human xylazine poisoning is uncommon. This report describes the use of xylazine for intentional poisoning with criminal intent. Two incidents occurred within 3 weeks: the first involved one victim, and the second involved two victims. The clinical presentations were brief coma, bradycardia, hypotension, and hyperglycemia. The victims recalled having been given a drink from a stranger in a hospital waiting room before loss of consciousness. In the first case, general drug screening by gas chromatography/mass spectrometry (MS) revealed xylazine in the gastric contents, but liquid chromatography-

Effects of xylazine hydrochloride on urine in cattle.

Urine volume, urine pH, and urine glucose concentration were monitored for up to 24 hours after physiological saline or one of two dosage levels (0.22 mg/kg or 0.44 mg/kg) of xylazine was administered to cows. During the first 2 hours after xylazine was given, urine output was greatly increased (relative to the control animals), with the high dosage group having more output than the low dosage group. The influence of the drug on urine volume had ended by 5 hours after injection when urine output in both dosage groups had returned to that of the control group. Glucose was detected in the urine

The XYZs of xylazine: Management of withdrawal and complications.

PURPOSE: We describe the case of a 33-year-old female patient with a past medical history of substance use disorder admitted for bilateral lower extremity infections characteristic of intravenous xylazine use. The patient's medical management was complicated by co-occurring xylazine and fentanyl withdrawal, for which there is little evidence-based guidance. SUMMARY: Utilizing evidence from local guidance and case reports, the patient was treated using a multipronged approach. A wound care protocol from the Philadelphia Department of Public Health was added to systemic antibiotics to manage le

Not Carfentanil-A Case of Unexpected Xylazine Detection.

Historically, xylazine has been utilized in veterinary medicine for decades as an anesthetic and analgesic sedative to facilitate safe handling, diagnostic testing, and surgical procedures in large animals. Currently, xylazine is an emerging threat to human health. It has been detected in the illicit drug supply chain, often as an adulterant. It has been more commonly added to illicit substances, most notably fentanyl, by drugmakers to enhance drug effect. End users are often unaware of its presence. This is alarming given the large number of xylazine-involved overdose deaths while laboratory

Pharmacology and toxicology of xylazine: quid novum?

The current opioid overdose crisis is characterized by the presence of unknown psychoactive adulterants. Xylazine is an alpha-2 receptor agonist that is not approved for human use but is commonly used in veterinary medicine due to its sedative and muscle-relaxant properties. Cases of human intoxication due to accidental or voluntary use have been reported since the 1980s. However, reports of adulteration of illicit opioids (heroin and illicit fentanyl) with xylazine have been increasing all over Western countries. In humans, xylazine causes respiratory depression, bradycardia, and hypotension-

Evaluation of ketamine-xylazine in Syrian hamsters.

Few parenteral anesthetics are safe and effective in the Syrian hamster. This study evaluated the anesthetic efficacy and potential for tissue damage of ketamine-xylazine (KX). Two dosage levels were administered intraperitoneally. Ketamine at 50 mg/kg combined with 10 mg/kg xylazine did not produce a consistent, reliable level of immobilization or anesthesia. Ketamine at 150 mg/kg combined with 10 mg/kg xylazine administered IP produced an adequate level of anesthesia without tissue damage for most procedures, but supplementation with lidocaine was necessary for peritoneal incision. Careful p

Clinical use of epidural xylazine in the horse.

Xylazine was administered into the epidural space of nine horses to facilitate various perineal manipulations (ie rectovaginal laceration repair, replacement of prolapsed rectum and urethral extension). The resulting caudal analgesia was sufficient for all procedures. The duration of analgesia from a single injection of epidural xylazine (0.17 to 0.22 mg/kg bodyweight) was at least 3.5 h. No horses were ataxic during or after the treatment. This trial demonstrates that xylazine given into the epidural space of horses provides prolonged regional analgesia which is sufficient for clinical use.

State xylazine scheduling and changes in xylazine and medetomidine reports in the U.S. illicit drug supply: A quasi-experimental study.

BACKGROUND: Xylazine and medetomidine are veterinary sedatives increasingly detected as adulterants in the U.S. illicit drug supply. In response, several states have scheduled xylazine; none have scheduled medetomidine during the study period. Whether xylazine scheduling was associated with subsequent changes in xylazine and medetomidine detections remains unclear. METHODS: We conducted a state-level, semiannual, serial cross-sectional study using National Forensic Laboratory Information System (NFLIS) data on drug samples from 1999 to 2025. The primary outcomes were xylazine and medetomidine

Wound Characteristics Among Patients Exposed to Xylazine.

IMPORTANCE: The alpha-2 agonist xylazine is increasingly detected as an adulterant in illicitly manufactured fentanyl. There is concern that xylazine may be responsible for an emerging pattern of necrotizing wounds among people who use drugs, but the clinical features of wounds associated with xylazine remain poorly characterized. OBJECTIVE: To systematically characterize the location, wound bed surface, and chronicity of wounds among persons with confirmed xylazine exposure. DESIGN, SETTING, AND PARTICIPANTS: This case series at 3 academic medical hospitals in Philadelphia, Pennsylvania, in

Treatment of Xylazine-Associated Injection Skin Injuries.

With the introduction of fentanyl and xylazine in the drug supply market, injection-related skin injuries and wounds are becoming more common. Xylazine is an alpha-2 adrenergic agonist thought to cause deep ulcerative wounds due to peripheral vasoconstriction leading to poor wound healing. This case series describes four patients with injection drug use leading to severe xylazine-related skin injuries who were treated between 2022 and 2023. This paper provides visualization of the extent and severity of these "tranq wounds," as well as the healing progression when receiving medical treatment,

Efficacy of caudal epidural injection of lidocaine, xylazine and xylazine plus hyaluronidase in reducing discomfort produced by electroejaculation in bulls.

To test the hypothesis that epidural administration of lidocaine, xylazine or xylazine plus hyaluronidase provides reduced pain and stress during electroejaculation in bulls, eight 30-month-old Nellore bulls received saline solution (control), 2% lidocaine, 2% xylazine or 2% xylazine plus hyaluronidase injected into the first intercoccygeal (Co1-Co2) epidural space in randomized order. Heart rate, respiratory rate, mean arterial pressure, analgesia, animal behavior and motor blockade were evaluated before treatment and at predetermined intervals during and after treatment. Pain and stress were

Clinical Microbiology in Xylazine-Associated Wound Infections.

BACKGROUND: Injection drug use (IDU) containing xylazine has been associated with the development of chronic necrotic wounds, albeit not all are of infectious etiology. This study describes the clinical microbiology of xylazine-associated wound infections to guide antimicrobial prescribing. METHODS: We conducted a retrospective, single-center cohort study of adults hospitalized with xylazine-associated wound infections related to IDU from 1 April 2022 to 1 December 2023. Patients were included if they received antimicrobials for suspected wound infections, along with either a positive urine x

Detecting Xylazine: Prevalence, Pharmacokinetics, and Analytical Considerations.

Xylazine, an alpha-2 adrenergic agonist used as a veterinary sedative, has been detected in the illicit drug supply with increasing frequency and is thought to contribute to opioid-related overdose and fatality. Several mass spectrometry-based methods for the detection of xylazine have been described but very few clinical laboratories routinely perform testing and point of care assays for acute xylazine toxicity are not currently available. The development of new methods for timely, cost-effective detection of xylazine is a priority to reduce harms associated with this drug.
