Zoledronic acid treatment in children with osteogenesis imperfecta.

BACKGROUND: Intravenous disodium pamidronate has become an established treatment in osteogenesis imperfecta (OI). Another bisphosphonate, zoledronic acid, has been indicated for the treatment of adult osteoporosis. We studied its efficacy and safety in children with mild OI. METHODS: Patients were treated for 1.0-3.2 years with 0.05 mg/kg zoledronic acid intravenously every 6 months as part of their clinical care. They were carefully followed for clinical and biochemical parameters, side effects, bone mineral densities (BMD) and compression fractures. RESULTS: The study included 17 patients

Bisphosphonate (zoledronic acid)-induced osteonecrosis of the jaw.

OBJECTIVE: The use of bisphosphonates (zoledronic acid) in the treatment of metastatic bone disease has been raised during recent years. The purpose of this treatment is mainly to reduce skeletal-related events, e.g. pain and pathological fractures. The aim of this study was to report the incidence of bisphosphonate-related osteonecrosis of the jaw (ONJ). MATERIAL AND METHODS: All patients treated with bisphosphonates during a 5-year period (2003-2007) were reviewed. RESULTS: Fifty-three patients, median age 69 years (range 56-81 years) were treated with bisphosphonates during the period. Fi

Zoledronic acid : Treatment option for Gorham-Stout disease.

Gorham-Stout disease is a rare bone resorption disease, the etiology and prognosis of which is uncertain but it is thought to be benign. It can involve one or more bones and can cause pain, swelling, deformity and fractures in affected bones. Diagnosis is made with a combination of clinical, radiological and histopathological examinations once other causes of osteolysis have been excluded. Due to its rarity, there is no standard therapeutic approach.

Zoledronic acid and atrial fibrillation in cancer patients.

BACKGROUND AND PURPOSE: Treatment with a bisphosphonate was found to be associated with a significantly increased risk for atrial fibrillation (AF) in a few studies. A recent study showed that once-yearly infusions of intravenous zoledronic acid (ZA) significantly increased the risk of serious AF in postmenopausal women with osteoporosis. This study was conducted to determine the frequency of atrial fibrillation among cancer patients receiving the standard treatment of ZA. METHODS: Patients with bone metastases who presented to our outpatient clinic for any reason (routine control, chemothera

Zoledronic acid - a multiplicity of anti-cancer action.

Bisphosphonates (BPs) are inhibitors of bone-resorption and have become the current standard of care for preventing skeletal complications associated with bone metastases. Among BPs, zoledronic acid (ZOL) has the strongest activity of anti-bone resorption and shows diverse direct anti-cancer effects in vitro. Some chemical and biological characteristics of ZOL indicate the potential for in vivo growth inhibition and the mechanisms responsible for the observed anti-cancer effects are beginning to be elucidated. ZOL inhibits farnesyl pyrophosphate synthase, a key enzyme in the mevalonate pathway

[Sunitinib and zoledronic acid induced osteonecrosis of the jaw].

The tendency for bisphosphonate and non-bisphosphonate (eg.: antiresorptive or anti-angiogenesis drugs) induced osteonecrosis is increasing. Treatment of these patients is a challenge both for dentists and for oral and maxillofacial surgeons. Cooperation with the drug prescribing general medicine colleagues to prevent osteonecrosis is extremely important. Furthermore, prevention should include dental focus elimination, oral hygienic instructions and education, dental follow-up and, in case of manifest necrosis, referral to maxillofacial departments. Authors outline the difficulties of conserva

Zoledronic acid in the treatment of metastatic breast cancer.

Bone is the most common location of metastatic disease. Approximately 80% of all bone metastases are observed in patients with breast or prostate tumours and are responsible for more than 300 000 deaths every year. Treatment of malignant bone disease with bisphosphonates has been shown to reduce bone events and delay their onset, and several reviews and meta-analyses have confirmed the benefit of these drugs in controlling bone metastases. Zoledronic acid is a bisphosphonate that has been shown to delay or prevent the development of skeletal-related events in patients with bone metastases. Fur

Role of zoledronic acid in the prevention and treatment of osteoporosis.

Taken once a year, intravenous zoledronic acid (Zol) (Reclast® or Aclasta®) is a third-generation nitrogen-containing bisphosphonate that is effective compared with placebo in reducing the risk of fractures in patients with postmenopausal osteoporosis and recent low-trauma hip fracture. In glucocorticoid-induced osteoporosis, there is no significant difference between Zol and risedronate for new fractures. Improvements in bone mineral density and early reduction of bone remodeling markers are observed in postmenopausal osteoporosis, recent low-trauma hip fracture, and glucocorticoid-induced os

Zoledronic acid infusion for prevention and treatment of osteoporosis.

Osteoporotic fractures are associated with significant morbidity, reduced quality of life, increased mortality, and high health care costs. Bisphosphonates are standard therapy for treatment of osteoporosis. However, patient compliance and persistence with oral weekly or monthly bisphosphonate therapy are suboptimal and may lead to reduced effectiveness. Zoledronic acid (ZOL) is an intravenous bisphosphonate that is given once yearly for the treatment of osteoporosis via a medically supervised 15-minute infusion. This ensures compliance for a full 12 months. In clinical trials, an annual infus

Update on the use of zoledronic acid in the management of osteoporosis.

Zoledronic acid is a third-generation bisphosphonate that is administered as an annual infusion, and it has some interesting features. After a successful preclinical development program, it has undergone extensive testing in postmenopausal osteoporosis where it has substantial antifracture efficacy at the vertebral, nonvertebral, and hip sites. It has been shown to be effective in reducing vertebral and nonvertebral fractures in patients who had recently suffered a femoral neck fracture, and has demonstrated efficacy in preventing and reversing bone loss in glucocorticoid osteoporosis. The dru

[Effects of zoledronic acid in adults with osteogenesis imperfecta].

BACKGROUND AND OBJECTIVE: Osteogenesis imperfecta (OI) is a genetic disorder that results in bone fragility. Several studies have demonstrated the effectiveness of bisphosphonate therapy. The aim of this study was to evaluate the effects of intravenous zoledronic acid on bone mineral density (BMD) and biochemical markers of bone turnover in adults with OI. MATERIAL AND METHODS: We carried out a prospective non-randomized study in patients with osteoporosis or severe osteopenia (T score < -2) related to OI and intolerance or contraindication to oral bisphosphonates. The patients were treated w

Use of zoledronic acid in the treatment of Gorham's disease.

Gorham's disease (Gorham-Stout syndrome) is a rare condition of unknown etiology involving a localized endothelial proliferation of lymph vessels resulting in destruction with bone resorption. The syndrome is rarely seen in the facial skeleton and has a large variety of prognoses and treatments. A case of this syndrome in a 9-year-old boy is presented. The clinical aspects are described, together with the treatment involving zoledronic acid. Other treatments described in the literature are reviewed. The authors believe that this report is one of the first cases in which a child afflicted in th

The use of zoledronic acid in pediatric cancer patients.

BACKGROUND: The third generation bisphosphonate zoledronic acid has demonstrated efficacy in reducing skeletal-related events in adult patients with multiple cancer types that have skeletal disease. The use of zoledronic acid in pediatric oncology patients with bone metastases for the purpose of reducing pain, improving bone strength and altering the progression of metastatic disease has not been thoroughly evaluated. PROCEDURE: From October 2005 to December 2008, 19 patients at the Aflac Cancer Center received one or more doses of zoledronic acid as part of their therapy. A retrospective rev

Zoledronic acid in the management of metastatic bone disease.

Many patients with advanced cancer experience decreased bone strength due to metastatic foci, underlying osteoporosis and/or cancer treatment induced bone loss. The clinical consequences of metastatic disease involving the skeleton are widespread. This review focuses on the efficacy, pharmacology, and safety when using intravenous biphosphonate such a zoledronic acid for cancer bone metastases. Zoledronic acid is the gold standard for the medical management of metastatic bone disease. The indications for treatment include prevention of skeletal relevant events (SRE), osteoporotic complications

Fanconi syndrome induced by zoledronic acid: Two case reports.

RATIONALE: Zoledronic acid (ZA) is a potent third-generation bisphosphonate widely used to manage bone metastases in prostate cancer. Although effective, ZA is associated with nephrotoxicity, which in rare cases may lead to Fanconi syndrome (FS): a form of acquired proximal renal tubular dysfunction. Clinicians may overlook early signs of FS, especially in patients receiving long-term bisphosphonate therapy. PATIENT CONCERNS: We report 2 cases of prostate cancer complicated by FS following ZA administration. Patient #1, a 74-year-old male without prior renal dysfunction, developed hypouricaem
