The incidence of ulcers of the stomach and duodenum and their response to medical therapy, in patients with Zollinger-Ellison syndrome is well described. However, reflux esophagitis is less well recognized. A patient had severe peptic ulcer disease complicated by gastric outlet obstruction and choledochoduodenal fistula. Serum gastrin levels were elevated preoperatively to 340 ng/L. A 1.5-cm histologically benign carcinoid tumor of the antrum of the stomach was found at surgery, and surgical resection of the tumor resulted in normalization of serum gastrin levels and amelioration of the peptic acid diathesis. The treatment of patients with Zollinger-Ellison syndrome (ZES) has undergone dramatic evolution during the past decade. Although initially regarded as an incurable tumor, resection of gastrinoma for potential cure has been reported in 30% to 40% of selected patients in recent series. Conversely, although definitive control of acid hypersecretion is achieved by total gastrectomy, histamine (H2)-receptor antagonists and the newly introduced agents omeprazole and somatostatin analogues allow effective medical therapy of gastric acid overproduction. As effective tumour resection can be achieved in only a minority of patients with Zollinger-Ellison syndrome, therapy in such patients is usually directed to treatment and prevention of symptoms induced by hypersecretion of gastric acid. To evaluate the frequency of associated endocrine disorders, 26 patients with the Zollinger-Ellison syndrome were restudied in a uniform screening programme. The examinations were directed primarily against disorders in the pituitary, parathyroid, thyroid, adrenal cortex, and endocrine pancreas. The treatment of gastric ulcer by the H2-receptor-antagonist Cimetidine results in an ulcer healing rate of 60 to 80% during a 4 to 6 weeks therapy cure. In most studies Cimetidine is superior to placebo or equipotent to a high dose antacid regimen, the latter being accepted to be of beneficial effect on ulcer healing. In the Zollinger-Ellison-syndrome Cimetidine inhibits largely gastric acid hypersecretion and represents for the first time a true alternative to total gastrectomy.

A small proportion of patients with peptic ulcer disease harbor a hormone-secreting pancreatic tumor as the cause of the ulcer diathesis, and if this is not recognized, they may suffer undue morbidity and mortality. Close attention to the patient's history, roentgenographic findings and gastric secretory studies will allow identification of ulcerogenic tumors of the pancreas. Other clues to the diagnosis include associated endocrine abnormalities and diarrhea.

Computed tomography of the upper abdomen was performed in 11 patients with Z--E syndrome and 6 patients with carcinoid syndrome. The liver, pancreas, spleen and upper part of the retroperitoneal space were examined to localize a primary tumour and/or metastases. The CT findings were correlated to the final diagnoses obtained by operation or by other diagnostic procedures.
