alpha 1-antitrypsin deficiency in early childhood.

Among 200,000 infants screened for alpha 1-antitrypsin (alpha 1-AT) deficiency, 125 Pi Z, 48 Pi Z, 1Pi S-, and 2 Pi Z- children were followed up prospectively. Eleven percent of the Pi Z infants had neonatal cholestasis, and at 2 years of age three of them had cirrhosis. About 50% of the asymptomatic Pi Z and Pi Z- subjects occasionally had serum alanine aminotransferase (ALAT) levels above normal, and in 15% of them the levels were probably permanently increased during the first two years of life. Two previously healthy Pi Z children had transient symptoms of liver disease at age 2 years in c

[Alpha 1 antitrypsin deficiency and liver diseases].

Alpha-1 antitrypsin is mainly responsible for the inhibitory power with regard to serum of numerous enzymes, in particular, trypsin. Alpha-1 antitrypsin deficiency is linked to pulmonary and hepatic disease. The special characteristic constantly found during liver disease, associated with alpha-1 antitrypsin deficiency, is the existence of intra-cytoplasmic globules within the hepatocytes due to accumulation of alpha-1 antitrypsin. This latter enzyme differs chemically from serum alpha-1 antitrypsin.

Synthesis of alpha 1-antichymotrypsin and alpha 1-antitrypsin by human trophoblast.

alpha 1-Antichymotrypsin (alpha 1-ACHY) and alpha 1-antitrypsin (alpha 1-AT) are closely related glycoprotein protease inhibitors, present in plasma and other extracellular fluids, that neutralize proteases released by leukocytes in response to trauma and inflammatory stimuli. Both inhibitors are synthesized primarily by hepatocytes, although lower levels of synthesis by monocytes and breast and intestinal epithelial cells have been demonstrated. Recently, the immunohistochemical localization of alpha 1-AT and alpha 1-ACHY in intrauterine and extrauterine human trophoblastic tissue has been re

Emphysema: looking beyond alpha-1 antitrypsin deficiency.

Distinct pathologies can cause chronic obstructive pulmonary disease (COPD). Emphysema is a COPD-phenotype characterized by destruction of lung parenchyma. Alpha-1 antitrypsin deficiency (AATD) is a genetic cause of emphysema, whereas smoking is the most important risk factor of non-AATD emphysema. A general underappreciation of non-AATD emphysema has hampered progress in the field, and clinical guidelines have prohibited the use of emphysema as a diagnosis. Non-AATD emphysema, however, is far from irrelevant as it associates with dyspnea, reduced exercise capacity and relevant outcome measure

Alpha-1 Antitrypsin Therapy for Autoimmune Disorders.

Autoimmune diseases are conditions caused by an over reactive immune system that attacks self-tissues and organs. Although the pathogenesis of autoimmune disease is complex and multi-factorial, inflammation is commonly involved. Therefore, anti-inflammatory therapies hold potential for the treatment of autoimmune diseases. However, long-term control of inflammation is challenging and most of the currently used drugs have side effects. Alpha-1 antitrypsin (AAT) is an anti-inflammatory protein with a well-known safety profile. The therapeutic potential of AAT has been tested in several autoimmun

Lung Transplantation in Alpha-1-Antitrypsin Deficiency.

BACKGROUND: Lung transplantation is a therapeutic option for patients with end-stage lung disease and a survival benefit has been described in patients with alpha-1-antitrypsin deficiency (A1ATD). The aims of the current study were to determine the survival and health benefits of lung transplantation in UK patients with A1ATD compared to carefully matched non-transplant patients. METHODS: Patients with the PiZZ (alpha-1-antitrypsin deficiency) genotype who had undergone lung transplantation between 1996 and 2011 were identified from the UK A1ATD registry. Lung physiology, health status and su

Alpha-1-antitrypsin deficiency in early childhood.

Alpha-1-antitrypsin deficiency (AATD), which predisposes liver disease in children, is often undiagnosed. Isoelectric focusing in 161 infants with liver dysfunction revealed 14.7% severe and 12.2% moderate AATD. Positive PAS-D and immunohistochemical staining was found in 60% of severe AATD, but in moderate AATD, only immunohistochemistry was positive in 100%. Bilirubinostasis, hepatomegaly, splenomegaly, cholestasis, hepatomegaly associated with cholestasis, acholia, high transaminases, and low birthweight were significantly more frequent in severe than in moderate AATD. Both AATDs showed sig

Cluster headache and Alpha 1-antitrypsin deficiency.

OBJECTIVE: Metabolic syndrome is a cluster of risk factors for cardiovascular disease and type 2 diabetes. Definitions exist to identify those "at risk." Treatment of HIV infection with highly active antiretroviral therapy can induce severe metabolic complications including lipodystrophy, dyslipidemia, and insulin resistance. The purpose of this study was to report the prevalence of metabolic syndrome in HIV-infected patients and compare insulin resistance and total body, limb, and visceral fat and adipokines in those with and without metabolic syndrome. RESEARCH DESIGN AND METHODS: This was

Fecal alpha 1-antitrypsin in newborn infants.

BACKGROUND: Fecal alpha 1-antitrypsin is used as a marker for intestinal protein loss reflecting increased intestinal permeability. Exact data of fecal alpha 1-antitrypsin in newborn infants are not available. METHODS: 30 healthy mature neonates and three infants with impaired gastrointestinal passage due to stenoses and atresia respectively, were investigated during the first days of life. The amniotic fluid of 13 and the serum of 17 infants was available. alpha 1-antitrypsin was determined using the radial immunodiffusion method. RESULTS: Normal newborns showed mean fecal alpha 1-antitryps

Acquired alpha-1-antitrypsin deficiency and dysgammaglobulinemia.

Alpha-1-antitrypsin (AAT) levels were measured in patients with three types of dysgammaglobulinemia: monoclonal gammopathy, polyclonal hypergammaglobulinemia and hypogammaglobulinemia. A fourth group comprised of subjects with normogammaglobulinemia served as a control. The frequency of hypoalpha-1-antitrypsinemia was significantly higher in those with monoclonal gammopathy compared with controls. The basis for this evaluation is unknown. It was not associated with phenotypic variation nor was it related to sex, age, total protein, albumin and immunoglobulin levels, liver function, bone marrow

Alpha 1-antitrypsin, alpha 1-antichymotrypsin and alpha 2-macroglobulin in human liver fluke (opisthorchiasis).

In mild and moderate Opisthorchis viverrini infection it is difficult to find clinical signs for diagnosis. Also routine clinical chemical reactions often fail to demonstrate changes in these patients. This study showed a significant increase of serum proteinase inhibitors, alpha 2-macroglobulin, alpha 1-antitrypsin and alpha 1-antichymotrypsin in patients suffering from O. viverrini infection compared with healthy controls. The increase of proteinase inhibitors together with other biochemical liver function tests may be useful as an indication of the impairment of liver function and the host

alpha 1-Acid glycoprotein, alpha 1-antitrypsin, and ceruloplasmin in human intestinal helminthiases.

alpha 1-acid glycoprotein, alpha 1-antitrypsin, and ceruloplasmin levels were followed in 150 children from the rural tropics after arrival in the temperate zone. It is postulated that multiple intestinal helminthiases, especially ancylostomiasis, cause an acute phase type of reaction which is enhanced by immunization with T.A.B.-cholera vaccine.

Monocytes and Macrophages in Alpha-1 Antitrypsin Deficiency.

Alpha-1 antitrypsin deficiency (AATD) is a genetic condition characterised by low circulating levels of alpha-1 antitrypsin (AAT), a serine proteinase inhibitor. The most common deficiency variants are the S and Z mutations, which cause the accumulation of misfolded AAT in hepatocytes resulting in endoplasmic reticular stress and insufficient release of AAT into the circulation (<11μmol/L). This leads to liver disease, as well as an increased risk of emphysema due to unopposed proteolytic activity of neutrophil-derived serine proteinases in the lungs. AATD has been traditionally viewed as an i

The Role of Neutrophils in Alpha-1 Antitrypsin Deficiency.

Alpha-1 antitrypsin deficiency (AATD) is characterized by low levels of circulating alpha-1 antitrypsin and an increased risk for emphysema, liver disease, and panniculitis. The reduced levels of alpha-1 antitrypsin in AATD predispose the lung to unopposed proteolytic activity, predominantly from neutrophil-derived proteases, chiefly neutrophil elastase. This leads to emphysema. The mechanisms subtending the liver disease are less well understood, but are probably due to a "gain-of function" inflammatory process in the liver, stoked by intracellular retention of aberrantly folded alpha-1 antit
