Amblyopia in the four to nine year age group. A four-year survey.

A guide to the ophthalmologist in the choice of the older child with amblyopia most likely to respond to active intensive visual stimulation in short bursts is presented. A series of 99 children treated on a CAM vision-stimulator is described. Visual improvement of at least two rows of letters on the Snellen chart was maintained for at least three years after treatment in 62% of these patients aged between five and nine years with visual fixation within 3 degrees of the fovea. Amblyopia following minor trauma or minor pathology also responded well. Children maintaining the visual improvement w

Evolving Strategies in Amblyopia Management: From Traditional Therapies to Cutting-Edge Innovations.

Amblyopia, commonly referred to as "lazy eye," is a prevalent visual disorder affecting children and, in some cases, adults. Traditional treatments, including occlusion therapy and pharmacological approaches, have demonstrated efficacy but also face limitations related to adherence, age dependency, and recurrence rates. This narrative review explores evolving strategies in amblyopia management, from conventional therapies to emerging innovations. Dichoptic therapy, which leverages binocular visual stimulation, represents a paradigm shift by directly addressing neural suppression and improving

Leber Hereditary Optic Neuropathy (LHON) in Patients with Presumed Childhood Monocular Amblyopia.

BACKGROUND: Most Leber hereditary optic neuropathy (LHON) cases are bilateral and sequential; however, there are rare unilateral examples, or those in which the delay of onset of vision loss between one and the other eye is longer. In the case of presumed childhood amblyopia in one eye, vision loss in the good eye may be the only symptom of bilateral disease, which was unnoticed in the previously amblyopic eye, or a preexisting episode of LHON in the "amblyopic" eye. The clinical decision in such cases may be difficult and suggestive of other forms of atypical optic neuropathy until confirmed

Visual outcomes after spectacles treatment in children with bilateral high refractive amblyopia.

PURPOSE: The aim was to investigate the visual outcomes of treatment with spectacles for bilateral high refractive amblyopia in children three to eight years of age. METHODS: Children with previously untreated bilateral refractive amblyopia were enrolled. Bilateral high refractive amblyopia was defined as visual acuity (VA) being worse than 6/9 in both eyes in the presence of 5.00 D or more of hyperopia, 5.00 D or more of myopia and 2.00 D or more of astigmatism. Full myopic and astigmatic refractive errors were corrected, and the hyperopic refractive errors were corrected within 1.00 D of th

Visual deficits in amblyopia constrain normal models of second-order motion processing.

It is well established that amblyopes exhibit deficits in processing first-order (luminance-defined) patterns. This is readily manifest by measuring spatiotemporal sensitivity (i.e. the "window of visibility") to moving luminance gratings. However the window of visibility to moving second-order (texture-defined) patterns has not been systematically studied in amblyopia. To address this issue monocular modulation sensitivity (1/threshold) to first-order motion and four different varieties of second-order motion (modulations of either the contrast, flicker, size or orientation of visual noise) w

1999 Costenbader Lecture. Outcome study in amblyopia: treatment and practice pattern variations.

This study retrospectively evaluates the results of amblyopia therapy and suggests hypotheses for future study. We address the various methods of treatment and evaluate the results from the most common therapy techniques. Practice pattern variations were analyzed in addition to the analysis of overall results. For ophthalmologists, there is a need to determine whether actual medical practice approaches the established standard of care, if it exists at all. How often are medical procedures, thought to be appropriate, based on anecdotal observation (case reports) rather than hard data (clinical

Are amblyopia therapy outcomes related to socioeconomic situation for children in Greater Manchester?

BACKGROUND: Limited evidence exists around health inequalities in amblyopia therapy. This cohort study explores amblyopia therapy outcomes and socioeconomic scores by postcode, across two boroughs within Greater Manchester. METHODS: All available orthoptic records for school vision screening referrals from the academic year 2017-2018 were reviewed by one Research Orthoptist, to identify children diagnosed with unilateral amblyopia. Clinical data were extracted and the proportion amblyopia deficit corrected and appointment attendance rates over 1 year of therapy were calculated. Home postcodes

Outcome of Occlusion and Supplemental Role of Mobile Games in the Treatment of Adult Amblyopia.

PURPOSE: To investigate the supplemental role of dichoptic mobile video games in the treatment of adults with amblyopia. METHODS: Patients aged 18 to 40 years with unilateral mild to moderate amblyopia and no prior history of amblyopia treatment were recruited after 6 weeks of optimal refractive correction. Patients were randomly assigned to two groups, each comprising 15 participants. The control group was prescribed 6 hours of daily patching. The study group played dichoptic mobile video games (stereoblocks) in addition to patching. They attended 2-hour sessions weekly for 12 weeks. Follow-

Visually Guided Saccades in Amblyopia and Strabismus: The Roles of Sensory Deficits and Nystagmus.

PURPOSE: To determine how amblyopia subtype, visual acuity, stereoacuity, and nystagmus influence visually guided saccades across viewing conditions in subjects with amblyopia and strabismus. METHODS: Visually guided saccades were recorded using video‑oculography in controls, anisometropic amblyopia, strabismic amblyopia, and strabismus without amblyopia subjects under binocular and monocular viewing. Saccadic latency, gain, disconjugacy, movement strategies, and main sequence relationships were quantified. Linear mixed effects models assessed group and viewing condition effects, and χ2 tests

Equivalent effectiveness of a prescription binocular treatment for amblyopia in real-world practice.

BACKGROUND: Gaps often exist between a therapy's performance in a controlled research setting and outcomes in routine clinical practice. A prescription binocular treatment delivered via head-mounted display has demonstrated efficacy for improving visual acuity in children with amblyopia in a pivotal randomized controlled trial (RCT). A retrospective patient registry has been established to collect real-world data on the treatment. OBJECTIVE: To evaluate the real-world effectiveness of this treatment by benchmarking outcomes from a patient registry against those from the RCT. METHODS: A retro

Advances in behavioral vision training for the treatment of childhood amblyopia: a narrative review.

BACKGROUND AND OBJECTIVE: Childhood amblyopia is a common neurodevelopmental disorder that impairs visual function and binocular integration. Traditional treatments, such as occlusion and pharmacological penalization, are often limited by poor adherence, incomplete restoration of binocular function, and variable response across patients. Behavioral vision training, grounded in principles of neural plasticity, has emerged as a promising alternative. This narrative review synthesizes the literature on behavioral vision training for pediatric amblyopia, focusing on its theoretical basis, main tec

Understanding amblyopia from the perspective of neurovascular units: changes in the retina and brain.

As a developmental vision disorder, amblyopia has traditionally been studied with a focus on neurons. However, the neurovascular unit (NVU), a dynamic functional complex of neurons, blood vessels, and glial cells, has recently been implicated in amblyopia. This review systematically discusses the pathological changes and functional interactions of the NVU in retina and brain in amblyopia patients and experimental models, providing a new perspective for clinical intervention.
