Emergent Meningoencephalitis Following Spinal Anesthesia: A Case Report.

Background: Iatrogenic meningitis is a rare but increasingly reported condition, particularly following invasive spinal procedures. The incidence is uncertain, ranging from none to two cases per 10,000 operations. Most cases involve infections with viridans streptococci. Septic meningitis presents with neurologic symptoms and elevated white blood cell counts in the cerebrospinal fluid (CSF), potentially leading to significant long-term neurologic damage or death if not promptly treated. Case Presentation: A 32-year-old previously healthy male presented with severe headache, phonophobia, photop

Efficacy and safety of three antiseptics for neonatal skin disinfection: a cohort study.

BACKGROUND: The choice of the ideal antiseptic is not only based on its efficacy but also on safety and skin-friendliness. There are no standard recommendations regarding ideal skin preparation in neonates. METHODS: This was a prospective cohort study to evaluate the efficacy of 3 antiseptics[10% Povidone Iodine(PI), 70% isopropyl alcohol(AL), 2% chlorhexidine in 70% alcohol(CHG-IPA)] in disinfecting the skin before venipuncture in term neonates as assessed by logarithmic reduction in skin bacterial colony counts post-application. Secondary objectives were to assess the changes in skin condit

The danger of the toxicity and inefficacy of alcohol-based hand rubs in Iran during COVID-19: a cross-sectional study.

BACKGROUND: The use of disinfectants and alcohol-based hand rubs (ABHR) to prevent COVID-19 transmission increased in the first wave of the infection. To meet the increased demand, the Iranian Ministry of Health issued an emergency use authorization allowing new manufacturers to enter the market, despite the limited capacity for surveillance of these products during COVID-19. Methanol poisoning outbreaks spread rapidly, and more people died from methanol poisoning than COVID-19 in some cities. The aim of this study was to analyze some ABHRs in the Iranian market to see if (a) ABHRs are standar

Factors affecting microbial contamination rate of cord blood collected for transplantation.

BACKGROUND: Collection and processing of cord blood (CB) is associated with significant risk of microbial contamination and hence relevant standards mandate microbial screening of the final product. This study aimed to determine the contamination rate and associated risk factors during 14 years of banking at the Sydney Cord Blood Bank. STUDY DESIGN AND METHODS: CB was collected and processed using a closed system and tested for contamination using blood culture bottles (BacT/ALERT, bioMérieux) incubated for a minimum of 5 days. Four microbial screening methods were used with different combina

Pathogenesis and management of wound infections in domestic animals.

Abstract  The pathogenesis of wound infections is largely dependent on adherence mechanisms and toxins. Studies of Staphylococcus intermedins indicate that pyoderma isolates can bind to extracellular matrix proteins exposed in wounds but interpretation of adherence studies is complicated as some organisms possess multiple mechanisms. Studies of blocking and promotion of adherence by antibiotics also show variability. Epidermolytic toxins are recognized in S. aureus and S. hyicus but not in S. intermedins, although a synergohymenotropic toxin has been described and canine syndromes occur where

Basics of Sterile Compounding. Quality Assurance and Quality Control: Being Prepared for U.S. Food and Drug Administration Inspections and Staying in Compliance with United States Pharmacopeia Chapter <797>: Part 2.

Compounders engaged in making sterile preparations need to employ a quality-assurance system of documented policies and procedures to attempt to reduce the possibility of contamination. The quality-assurance program will be monitored through the facility's quality-control system. Compounders should be aware of the requirements of each state they are licensed in as well as the inspection observations commonly noted in 483s issued by the U.S. Food and Drug Administration. Part 1 of this 2-part article discussed the currently evolving regulatory environment and why sterile compounding requires pl

Longitudinal analysis of the hand microbiome in response to chlorine-based antiseptic use during a military field exercise.

Hand hygiene is essential for infection control, yet the impact of frequent antiseptic use on the skin microbiome, which is crucial for skin barrier function and pathogen exclusion, remains underexplored, especially in field conditions. In high-risk military settings, there is a need for safe, multipurpose antiseptics that avoid the drawbacks of alcohol-based options, including skin irritation, flammability, and unsuitability for wound application. We conducted a longitudinal study during the Norwegian-led military exercise Cold Response 2022 to assess the effects of repeated use of stabilized

Multimodal assessment of the prevention of surgical site infections in breast surgery in a French university hospital.

INTRODUCTION: Monitoring surgical site infections (SSIs) incidence through surveillance systems enables tailored infection prevention interventions. In our facility, annual surveillance of SSIs in breast surgeries revealed a significant increase of SSI incidence rates six times higher than the national average in France in 2024. This study aimed to investigate the factors contributing to the increase in SSIs following breast surgery at the South Hospital Group of the Hospices Civils de Lyon (HCL), Pierre-Bénite, France. METHODS: The study was divided into three parts, all including breast sur

Different methods of vaginal preparation before cesarean delivery to prevent postoperative infection: a systematic review and network meta-analysis.

OBJECTIVE: Precesarean vaginal antisepsis can benefit pregnant women with ruptured membranes. However, in the general population, recent trials have shown mixed results in reducing postoperative infections. This study aimed to systematically review clinical trials and summarize the most suitable vaginal preparations for cesarean delivery in preventing postoperative infection. DATA SOURCES: We searched PubMed, Web of Science, Cochrane Library, SinoMed databases, and the ClinicalTrials.gov clinical trials registry for randomized controlled trials and conference presentations (past 20 years, 200

Preparing the wound bed 2003: focus on infection and inflammation.

Wound bed preparation is the promotion of wound closure through diagnosis of the cause, attention to patient-centered concerns, and correction of systemic and local factors that may delay healing. To enhance the evidence base that may be lacking, a review of relevant literature was conducted and combined with input from the International Wound Bed Preparation Advisory Board and the Canadian Chronic Wound Advisory Board to create an updated examination of practices. A template based on expert opinion of the clinical actions corresponding to each step in the paradigm of preparing the wound bed i

[Perioperative and peri-interventional antibiotic prophylaxis: general and urology-specific recommendations from the German Interdisciplinary S3 guideline].

BACKGROUND: Perioperative and peri-interventional antibiotic prophylaxis (PAP) is a central instrument for the prevention of infectious complications, yet it has to consider concerns of individual and general patient safety, antibiotic stewardship, and antibiotic resistance dynamics. The interdisciplinary AWMF (Arbeitsgemeinschaft Medizinisch-Wissenschaftlicher Fachgesellschaften) S3 guideline finalized in 2024 (AWMF S3, 2024) provides a comprehensive, evidence-based framework for indication, substance selection, dosing, timing, and quality assurance of PAP across all surgical disciplines, wit

Comparison of Clinical Outcomes According to Preoperative Skin Preparation in Hip Arthroplasty for Hip Fracture: A 3-Arm Randomized Controlled Trial.

BACKGROUND: Patients with hip fractures undergoing arthroplasty are typically old and frail, and superficial surgical site infections (SSIs) can delay mobilization and prolong hospitalization. Preoperative skin antisepsis is a low-cost, universally used intervention. However, clinical practice varies, and evidence from elective arthroplasty may not fully generalize to urgent hip fracture surgery. Sequential dual-antiseptic protocols combining povidone-iodine (PVI) and chlorhexidine gluconate (CHG) are increasingly adopted; however, the optimal order for "scrub" versus "paint" remains unclear.

Global Scaling of a Surgical Infection Prevention Program to 5 Low-Resource Countries.

IMPORTANCE: Surgical site infections (SSI) are a leading cause of morbidity and mortality from surgery, with higher rates in low- and middle-income countries (LMICs). Clean Cut is a multimodal, adaptive quality improvement program that aims to reduce SSI by improving compliance with perioperative infection prevention standards. The program has been successfully implemented in Ethiopia at 12 hospitals with an associated 35% reduction in SSI. OBJECTIVE: To assess whether this surgical infection prevention program implemented in Ethiopia can be effectively scaled to a variety of geographical and

Vaginal cleansing with chlorhexidine gluconate or povidone-iodine prior to cesarean delivery: a randomized comparator-controlled trial.

BACKGROUND: Several randomized controlled trials have demonstrated that preoperative abdominal skin preparation with chlorhexidine gluconate is superior to povidone-iodine for the prevention of surgical site infections. Despite these results, povidone-iodine is still the most commonly used agent for vaginal preparation, even though it may not be ideal. OBJECTIVES: The objectives of the study were as follows: (1) to determine whether vaginal cleansing with a 4% chlorhexidine gluconate solution results in fewer wound infections as compared with povidone-iodine when used for vaginal antisepsis p
