Treatment of endolymphatic hydrops with cochleosacculotomy. Clinical results and experimental findings.

Cochleosacculotomy was performed on 25 patients with Meniere's syndrome. Long-term relief of vertigo was obtained in 19 out of 23 (82%). Postoperatively dead ears occurred in three cases. The same operation was performed on 12 guinea pigs in which hydrops had been surgically induced by blockage of the endolymphatic duct and sac. All fistulas were healed and cochleosacculotomy did not decrease or prevent the induced endolymphatic hydrops in these animals. Although histologic confirmation of persistent fistulas in human ears is lacking, the relief of vertigo in patients may not be caused by "dra

A comprehensive finite-element human ear model to estimate noise-induced hearing loss associated with occupational noise exposure.

BACKGROUND AND OBJECTIVE: Noise is a common occupational and environmental hazard; however, little is known about the use of computational tools to quantitively analyze data on basilar membrane (BM) damage in noise-induced hearing loss (NIHL). Here, we established a comprehensive three-dimensional finite-element human ear model to quantify the impact of noise exposure on BM and perilymph fluid. METHODS: We used auditory risk units (ARUs) to evaluate the BM damage for subjects (3 men and 5 women; mean age, 32.75 ± 8.86 years; age range, 24-44 years). A 90-dB sound pressure level (SPL) was norm

The Effect of Round Window Sealants on Delayed Hearing Loss in a Guinea Pig Model of Cochlear Implantation.

AIM: To determine whether the type of material used to seal the cochlea after round window cochlear implantation influences delayed hearing loss. BACKGROUND: Cochlear implants are now prescribed to patients with residual, low-frequency hearing. This hearing-which provides perceptual benefits for the implanted ear-is frequently lost for unknown reasons weeks to months after surgery in a proportion of patients. A post-surgical change in cochlear mechanics, related to the material used to seal the cochlea after round window implantation, may contribute to this loss. METHODS: An electrode array

Effects of glycerol intake and body tilt on otoacoustic emissions reflect labyrinthine pressure changes in Menière's disease.

It is known that by influencing stapes stiffness thus the ear's impedance, changes in intracranial and intralabyrinthine pressure induce a characteristic phase shift in otoacoustic emissions (OAE) around 1 kHz in human ears. Thus, if the regulation of pressure in intralabyrinthine compartments were abnormal in Menière patients, OAEs might help detect it. Body tilt, which acts on intracranial pressure, and administration of an osmotically active substance provide two simple ways of manipulating intralabyrinthine pressure. Here, 14 patients with typical signs of an attack of unilateral endolymph

Derived-band auditory brain-stem response estimates of traveling wave velocity in humans: II. Subjects with noise-induced hearing loss and Meniére's disease.

Estimates of cochlear traveling wave velocity (TWV) were computed from derived-band auditory brain-stem response (ABR) latencies in subjects with noise-induced hearing loss (NIHL) or Meniére's disease (MD). ABR wave V latencies were determined for each of six derived frequency bands (unmasked-8 kHz, 8-4 kHz, 4-2 kHz, 2-1 kHz, 1 kHz-500 Hz, and 500-250 Hz). Representative frequencies were assigned to the derived bands by estimating their energy midpoints, and cochlear positions corresponding to these frequencies were determined using Greenwood's (1961) place-frequency function for humans. Potentiation of noise induced threshold shifts and hair cell loss by carbon monoxide.

Previous studies have determined that severe systemic hypoxia disrupts cochlear function acutely, but have suggested that augmentation of cochlear perfusion may successfully protect cochlear function under all but the most profound hypoxic treatments. In the current study we report on the chronic effects of simultaneous exposures to noise and carbon monoxide on pure tone thresholds and hair cell survival in rats. Following initial threshold determination, rats received acute exposure to carbon monoxide, noise, or both agents concurrently. Thresholds were evaluated 2-4 and 6-8 weeks later. The

Discharge patterns in the cochlear nucleus of the chinchilla following noise induced asymptotic threshold shift.

Chinchillas were exposed to an 86 dB SPL octave band of noise centered at 4.0 kHz for 3.5--5 days. The noise elevated the hearing thresholds between 4.0 and 16.0 kHz to between 60 and 75 dB SPL. Measurements from single neurons in the cochlear nucleus revealed abnormalities in the response properties of neurons with characteristic frequencies (CF) above 2.0 kHz. Units above 2.0 kHz had elevated thresholds (between 50 and 90 dB SPL) and broad tuning curves due to a greater loss in sensitivity near CF than at lower frequencies. The tuning curve Q10dB values for high frequency neurons were genera

3D Finite Element Modeling of Blast Wave Transmission from the External Ear to Cochlea.

As an organ that is sensitive to pressure changes, the ear is often damaged when a person is subjected to blast exposures resulting in hearing loss due to tissue damage in the middle ear and cochlea. While observation of middle ear damage is non-invasive, examining the damage to the cochlea is difficult to quantify. Previous works have modeled the cochlear response often when subjected to an acoustic pressure input, but the inner ear mechanics have rarely been studied when the ear is exposed to a blast wave. In this study we aim to develop a finite element (FE) model of the entire ear, particu

Diagnosing cochlear dead regions in children.

OBJECTIVE: A dead region (DR) is defined as a region in the cochlea where inner hair cells and/or neurons are functioning so poorly that a tone producing peak vibration in this region is detected by off-frequency listening, i.e., via a place on the basilar membrane with a characteristic frequency different from that of the tone. The presence of a DR can have a significant effect on the perception of speech. People with and without DRs may differ in the benefit obtained from amplification and require different hearing aid settings. The Threshold Equalizing Noise (TEN) test and psychophysical tu

Combined rupture of Reissner's membrane and round window: an experimental study in guinea pigs: experimental double-membrane rupture.

OBJECTIVE: Hearing loss caused by combined rupture of Reissner's membrane and the round window (RW) membrane (the double-membrane rupture) may differ depending on the site of the lesion on Reissner's membrane. The purpose of this experimental study was to reveal the relationship between the hearing impairment and the site of the lesion on Reissner's membrane. BACKGROUND: According to experimental studies on perilymphatic fistula (PLF), profound hearing loss is not induced by rupture of RW alone, but by the double-membrane rupture. However, the mechanism responsible for hearing loss in the dou

Study on the Mechanism of Hearing Loss Induced by USH2A Gene Knockout.

BACKGROUND: Usher syndrome (USH) is an autosomal recessive disorder characterized by hearing loss, retinitis pigmentosa, and variable vestibular dysfunction. USH2A is one of the causative genes of USH. This study is aimed at exploring the mechanism of hearing loss induced by USH2A gene knockout. METHOD: USH2A knockout (Ush2a-/-) mice were used, and auditory brainstem response testing was performed on WT, Ush2a-/-, and Ush2a+/- mice. Then, the cochlea tissues were used to carry out immunofluorescence staining, hematoxylin and eosin (H&E) staining, and scanning electron microscopy (SEM). The mR

Synchrotron-Based Trauma Assessment of Robotic Electrode Insertions in Cochlear Implantation.

OBJECTIVES: Minimizing intraoperative trauma during cochlear implant electrode insertion is crucial for preserving residual hearing. Manual insertions are subject to variability due to surgeon experience and anatomical differences, often leading to inconsistent outcomes. Robotic-assisted systems have been developed to improve insertion precision and reduce trauma. This study evaluates the safety and efficacy of the OTODRIVE system for cochlear implant electrode insertion. METHODS: Fifteen cadaveric human temporal bones were implanted with a variety of electrodes (FLEX26, FLEX28, FLEXSOFT, FLE

How the Human Cochlea Moves: Biomechanical Modeling of a Wide, Layered Osseous Spiral Lamina.

PURPOSE: The human cochlear partition (CP) at the high-frequency region features a radially wide, layered osseous spiral lamina (OSL) and a soft-tissue bridge connecting it to the basilar membrane (BM). The OSL consists of two thin bony plates separated by a cavernous space that serves as a conduit for auditory nerve fibers. We used a finite element model with two fluid chambers, incorporating novel implementations of the CP features, to study the human cochlea. Model results were compared with experimental measurements of CP motion. METHODS: Model geometrical and material properties either c

Blocking acid-sensing ion channel 1a attenuates bilirubin-induced ototoxicity in cochlear organotypic culture.

Neonatal hyperbilirubinemia leads to neural dysfunction, including sensorineural hearing loss. Our previous studies demonstrated that elevated bilirubin induced apoptotic pathway activation and led to cell malformation in spiral ganglion neurons of neonatal rat cochlear organotypic explants. However, the underlying mechanisms remained unclear. Concurrent hyperbilirubinemia and acidemia would exacerbate neuronal damage. Acid-sensing ion channels (ASICs), which belong to the epithelial Na+ channel/degenerin (ENaC/DEG) superfamily, were activated during acidic pH fluctuations. To elucidate the ro

High doses of radiation cause cochlear immunological stress and sensorineural hearing loss.

Radiotherapy is a crucial treatment for head and neck malignancies, but it can sometimes cause sensorineural hearing loss (SNHL). Changes in the immune microenvironment and sensory neuroepithelium of the inner ear after radiation exposure remain poorly understood. This study investigated cochlear morphology and macrophages in the inner ear after high-dose irradiation. The heads of heterozygous 8-week-old Cx3cr1GFP/+ male mice were irradiated with 30Gy of X-rays and biological samples were collected on days 1, 7, and 10 after irradiation. Auditory brainstem responses were used to assess auditor

The anterior ventriculo-cisternostomy: the pioneers' work revisited.

BACKGROUND: There is no satisfactory medical treatment for hydrocephalus and most often surgical treatment is necessary, including shunting or endoscopic third ventriculostomy (ETV), both with their well-known advantages and drawbacks. The objective of this study is to describe a contemporary series of anterior ventriculocisternostomy. PATIENTS: Twenty-two patients with hydrocephalus treated at two institutions between 2005 and 2009 were presented. The authors employed a technique called anterior ventriculocisternostomy to treat selected cases. This technique includes supraciliary incision, s

MCU knockdown attenuates cisplatin-induced damage of auditory cells: potential involvement of PI3K/AKT/mTOR signaling and autophagy-related processes.

INTRODUCTION: The mitochondrial calcium uniporter (MCU) mediates mitochondrial Ca2⁺ uptake, but its involvement in cisplatin-induced ototoxicity remains poorly understood. We investigated MCU expression and its association with cisplatin-induced injury in mouse cochlear basilar membranes and HEI-OC1 cells. METHODS: MCU protein abundance and localization were assessed by Western blotting and immunofluorescence. MCU expression was genetically reduced by lentiviral knockdown, and Ru360 was used as a pharmacological intervention. Apoptosis-related proteins and transcriptomic changes were evaluate

Expression of nestin, parvalbumin and otoferlin during cochlear development in the mouse: an immunofluorescence study.

To elucidate the proteins associated with cochlear development and auditory formation from a histomorphological point of view, this study examined the spatio-temporal expression pattern of nestin, parvalbumin, and otoferlin in the mouse cochlea from embryonic day 17 (E17) to postnatal day 28 (P28) using immunofluorescence. Our findings revealed that nestin was broadly expressed in developing otic mesenchyme cells beneath the basilar membrane, medial to the greater epithelial ridge, and adjacent to the developing stria vascularis during late embryonic stages (E17 and E18.5). From P1 to the onse

Three-dimensional quantification of fibrosis and ossification after cochlear implantation via virtual re-sectioning: Potential implications for residual hearing.

Hearing preservation may be achieved initially in the majority of patients after cochlear implantation, however, a significant proportion of these patients experience delayed hearing loss months or years later. A prior histological report in a case of delayed hearing loss suggested a potential cochlear mechanical origin of this hearing loss due to tissue fibrosis, and older case series highlight the frequent findings of post-implantation fibrosis and neoosteogenesis though without a focus on the impact on residual hearing. Here we present the largest series (N = 20) of 3-dimensionally reconstr
