Contraceptive use among U.S. women having abortions in 2000-2001.

CONTEXT: Knowing the extent to which contraceptive nonuse, incorrect or inconsistent use, and method failure account for unintended pregnancies ending in abortion, as well as reasons for nonuse and imperfect use, can help policymakers and family planning providers support effective contraceptive use. METHODS: Contraceptive use patterns among a nationally representative sample of 10,683 women receiving abortion services in 2000-2001 were examined, as well as reasons for nonuse, problems with the most frequently used methods and the impact emergency contraceptive pills have had on abortion rate

Selective progesterone receptor modulators.

PURPOSE OF REVIEW: Review of recent data from clinical trials and descriptions of endometrial morphology with administration of selective progesterone receptor modulators (SPRMs). RECENT FINDINGS: Recent reports concerning administration of SPRMs, specifically the efficacy of ulipristal acetate in reducing fibroid size and rapid control of menstrual blood loss, have renewed clinical interest in this class of compound. Histological data from studies with SPRMs report that this class of drugs is associated with progesterone receptor modulator-associated endometrial changes. Data on mechanisms o

When the morning after is Sunday: pharmacist prescribing of emergency contraceptive pills.

Pharmacists are well positioned to play a major role in increasing access to emergency contraceptive pills (ECPs). A pilot project in Washington State is testing direct pharmacist prescribing. Through a collaborative drug therapy agreement, a licensed prescriber, such as a physician, delegates to a pharmacist the authority to prescribe ECPs directly to women who meet the assessment criteria. Currently pharmacists at 111 Washington State pharmacies have collaborative agreements in place, and the number of participating pharmacies continues to increase. The response to this initiative has been e

Ulipristal acetate compared to levonorgestrel emergency contraception among current oral contraceptive users: a cost-effectiveness analysis.

OBJECTIVE: To estimate the cost-effectiveness of ulipristal acetate (UPA) and levonorgestrel (LNG) emergency contraception (EC) on pregnancy prevention among combined oral contraceptive (COC) pill users with an extended pill-free interval. We accounted for the potential interaction of COCs and obesity on EC efficacy. METHODS: We built a decision-analytic model using TreeAge software to evaluate the optimal oral EC strategy in a hypothetical cohort of 100,000 twenty-five-year-old women midcycle with a prolonged "missed" pill episode (8-14 days). We used a 5-year time horizon and 3% discount ra

Emergency contraceptive pill users' risk perceptions for sexually transmitted infections and future unintended pregnancy.

BACKGROUND AND PURPOSE: The availability of emergency contraception pills (ECP) over the counter (OTC) has the potential to reduce the incidence of unintended pregnancy; however, the increased risk for sexually transmitted infection (STI) acquisition, related to unprotected intercourse, has not been adequately addressed. The purpose of this study is to gain insight into risk perceptions for STIs and subsequent unintended pregnancy in women who have purchased ECP OTC. METHODS: Twenty-one women, aged 18-24, attending a private university in an urban setting, who purchased and used ECP OTC parti

Effect of changes to cost and availability of emergency contraception on users' profiles in an emergency department in Catalunya.

OBJECTIVE: Emergency contraception (EC) accessibility has evolved differently in Catalunya as compared with other autonomous communities in Spain. Free-of-charge access within the Public Health System was authorised in 2004, and over-the-counter (OTC) access was implemented in 2009. The aim of the study is to assess the impact of these measures on demand and users' profiles at our institution. METHODS: A retrospective study, in our Emergency Department, was conducted to evaluate EC requests in relationship to accessibility modifications. The age of women and which days of the week they attend

Adolescent input for designing an emergency department-based intervention about emergency contraception: results from an in-depth interview study.

OBJECTIVE: To explore the attitudes of urban, minority adolescent girls about an emergency department (ED)-based intervention to address emergency contraceptive pill (ECP) use. METHODS: We conducted an in-depth, semistructured interview study of healthy, 15- to 19-year-old African-American girls seeking care in a children's hospital ED. Purposive sampling was used to recruit sexually and nonsexually active adolescents and those with or without a history of pregnancy. We collected demographic data and opinions about an ED-based intervention addressing ECP use. A modified grounded theory approa

An update on emergency contraception use in adolescents.

Changing circumstances regarding access to emergency contraception (EC) warrant nurses' review of its current status. Nurses have the potential to improve adolescents' awareness of and access to EC so that adolescents may prevent pregnancy. Research suggests that adolescents lack awareness of EC methods that prevent pregnancy following an unprotected act of intercourse. Many pregnancies occurring in adolescence could be prevented with appropriate EC use. Studies show that adolescents are capable of using EC correctly and safely, and that access to EC is not associated with increased rates of u

Safety and effectiveness data for emergency contraceptive pills among women with obesity: a systematic review.

OBJECTIVE: This study aims to determine whether emergency contraceptive pills (ECPs) are less safe and effective for women with obesity compared with those without obesity. STUDY DESIGN: We searched PubMed for articles through November 2015 regarding the safety and effectiveness of ECPs [ulipristal acetate (UPA), levonorgestrel (LNG) and combined estrogen and progestin] among obese users. We assessed study quality using the United States Preventive Services Task Force evidence grading system. RESULTS: We identified four pooled secondary analyses (quality: poor to fair), two of which examined

Association between increased availability of emergency contraceptive pills and the sexual and contraceptive behaviors of women.

In the United States (US), access to emergency contraceptive pills (ECPs) expanded to nationwide in 2006 when regulators allowed Plan B, a brand of emergency contraception, to be sold without prescription. Using data from the National Health and Nutrition Examination Survey from 2001 to 2010, I examined any association between increased access to these ECPs in the US and negative consequences. I found an association between increased access to ECPs and a 2.2 per cent higher probability of any sexual activity, a 5.2 per cent increase in the likelihood of reporting sex with multiple partners, an

Emergency contraceptive pills over-the-counter: a population-based survey of young Swedish women.

One year after emergency contraceptive pills (ECP) had become prescription-free, we sent a questionnaire to 800 randomly selected women in mid-Sweden. The aim was to investigate women's knowledge, attitudes and practices regarding the method. The response rate was 71% (n = 564). The majority of the women, 65%, would prefer to purchase ECP over-the-counter (OTC) in a pharmacy. Attitudes toward the method were predominantly positive, but one fourth (24%) had worries about side effects and one third (33%) considered ECP to be a kind of abortion. Logistic regression showed that correct knowledge o

Emergency contraception: knowledge and practices of tertiary students in Durban, South Africa.

The aim of this study was to assess the knowledge, use and attitude to the use of emergency contraception among tertiary students in Durban, South Africa through the use of a self-administered confidential questionnaire. A scoring system was developed to analyse the response of each student. A total of 436 students (56.5%) had heard of emergency contraception. Few knew the specific methods of emergency contraception and only 11.8% knew the correct time limit in which it must be used. Only 60 students (7.8%) knew how effective emergency contraception was in preventing pregnancy. Ninety-one stud

Factors related to unwanted pregnancies and abortion in the northern district of the city of Tehran, Iran.

The aim of this study was to investigate the levels and determinants of unwanted pregnancies and therapeutic and elective induced abortions in an Iranian sample of women. This study was conducted from the beginning of 2013 to the end of July 2013, using data from 950 women aged 15-45 years. To evaluate the relationship of independent variables to the number of unwanted pregnancies, a negative binomial regression model was used. A multinomial logistic model was used to investigate the relationship of the history of at least one unwanted pregnancy to therapeutic and elective induced abortion. Th

Provision of Contraception: Key Recommendations from the CDC.

The Centers for Disease Control and Prevention has released comprehensive recommendations for provision of family planning services. Contraceptive services may be addressed in five steps, and counseling may be provided in a tiered approach, whereby the most effective options are presented before less effective options. Clinicians should discuss all contraceptive methods that can be used safely by the patient, regardless of whether a method is available on site and even if the patient is an adolescent or a nulliparous woman. Physical assessment is usually limited to blood pressure evaluation be
