Finger stiffness may arise from injuries, surgeries, or hand-related medical conditions, impacting hand function and overall well-being in daily life. Rehabilitation and hand therapy play a crucial role in restoring optimal range of motion, strength, and functionality. This article introduces the Self-Assisted Finger Stiffness Splint (SFSS), a dynamic splint designed for active finger movement applicable in post-trauma or postoperative rehabilitation. SFSS empowers patients to perform self-administered stretching exercises, expediting recovery and improving compliance. Its versatility extends

The fibrous components of the metacarpophalangeal (MP) joint including the palmar plate, the collateral ligament and the dorsal plate were studied with particular attention paid to the fibrous structure of the fibrous tendon sheath and the deep transverse metacarpal ligament. The tough fibrillar structure around the MP joint, especially the force nucleus, consisted of three types of mixed fibers: the fibrous tendon sheath of the A1 pulley, the deep transverse metacarpal ligament, and the palmar plate. The tendon sheath was located on the ulnar side in the index and middle fingers, on the centr

We reviewed 110 trigger digits, treated surgically, to compare the outcomes of trigger finger and trigger thumb in terms of peri-operative characteristics and complications. The patients with trigger thumb complained mainly of pain on motion, while those with trigger finger complained of triggering or limited range of motion. Trigger fingers had a significantly longer duration before surgery than did trigger thumbs. Trigger fingers took significantly longer for the symptoms to subside. In our series, 64% of trigger fingers had a flexion contracture of the PIP joint more than three weeks after

Arthrodesis using a headless screw has been shown to be an effective surgical treatment for pain, deformity, or instability arising at the distal interphalangeal joint of the fingers or interphalangeal joint of the thumb. Previous publications have documented the utility of this treatment using open techniques. Common indications include degenerative, inflammatory, or posttraumatic arthritis and chronic mallet finger or chronic flexor digitorum profundus injury. The purpose of this study is to present an arthroscopic technique for arthrodesis of the distal interphalangeal joint of the fingers

This retrospective study describes closed finger crush injuries in seven patients (eight fingers) in which each finger sustained a loss of blood supply. Clinical findings included numbness, decreased two-point and sharp/dull sensation, cyanosis or pallor, and decreased capillary filling. Fractures, especially transverse fractures near the proximal interphalangeal joint or distal interphalangeal joint, were usually present and often showed longitudinal crush fracture lines. Exploration and revascularization were carried out in seven fingers, all of which survived. The only finger not explored p

A classical guitarist performing for at least 5 hours each day developed a traumatic synovitis at the left wrist joint that was first erroneously considered to be rheumatoid arthritis. Comparison with members of the same guitar class suggested that unusual joint laxity of the fingers and wrist, probably inherited from the patient's father, was of more importance in the aetiology of the synovitis than a wide range of movement acquired by regular practice. Hyperextension of the metacarpophalangeal joint of the left index finger, quantified by the hyperextensometer, was less marked in the guitari

Dactylitis is a typical phenomenon in psoriatic arthritis, presenting as a "sausage-shaped" finger. Although classically the distal interphalangeal joints are affected, proximal interphalangeal joints and metacarpophalangeal joints are frequently involved. We present a patient who presented with severe psoriatic arthritis of the proximal interphalangeal joint, leading to subtotal disappearance of the phalangeal bones and important shortening of the fourth finger after 10 years of evolution.

Palliative treatment of palsy of intrinsic muscles of the four fingers classically aims at suppressing claw. But functional restoration is mainly ensured in setting up again the volar flexion, the stage preparing prehension. Three hundred and thirty seven hands have been operated. Each one of the utilized interventions has advantages and drawbacks that have to be known in order to select the best adapted technics, the more or less important stiffness of fingers being the most important criteria. Finally a simple choice is easy and possible, particularly for inexperienced surgeons in the treatm

PURPOSE: Although several factors have been related to the development of digital stiffness after external fixation of distal radius fractures, distraction can be considered one of the most important. This study determined a threshold for distraction during external fixation of distal radius fractures and documented the effect of distraction of the wrist on metacarpophalangeal (MCP) joint motion.
METHODS: Eight fresh-frozen cadaveric forearms were mounted on a testing frame and the flexor digitorum superficialis (FDS) tendon of each finger was pulled individually until a 90 degrees flexion at

The subsynovial connective tissue is an integral component of flexor tendon gliding in the carpal tunnel, which is strained during longitudinal tendon displacement. We tested the effects of repetition frequency and finger load on flexor tendon function throughout active finger movement. Eleven participants performed metacarpophalangeal joint flexion/extension of the long finger cyclically at three repetition frequencies (0.75, 1.00, 1.25 Hz) and two finger loads (3.5, 7 N). Relative displacement between the flexor digitorum superficialis tendon and subsynovial connective tissue was assessed as

In this paper we presented the mechanical design and evaluation of a low-profile and lightweight exoskeleton that supports the finger extension of stroke patients during daily activities without applying axial forces to the finger. The exoskeleton consists of a flexible structure that is secured to the index finger of the user while the thumb is fixed in an opposed position. Pulling on a cable will extend the flexed index finger joint such that objects can be grasped. The device can achieve a grasp size of at least 7 cm. Technical tests confirmed that the exoskeleton was able to counteract the

Staged extensor tendon reconstruction using a silicone implant followed by tendon grafting was done to restore proximal interphalangeal (PIP) joint extension in 6 fingers with severe injuries to the dorsal skin and extensor mechanism. Abrasions to the joint capsule and cortical surfaces were also present. To avoid finger stiffness, the reconstruction was delayed and range of motion exercises were initiated early. The skin injury was managed by split-thickness skin grafting or allowed to heal by secondary intention to avoid prolonged immobilization. During surgery, the peritendinous fascia of t

The conformability is yet a challenge for most soft robotic grippers due to the continuous motion and deformation of these machines under external force. Herein, inspired by the movement mechanism of human fingers, we propose a novel tendon-driven soft robotic finger with a preprogrammed bending configuration and a human finger like sequential motion that can be obtained by matching the stiffness gradient of the finger joints with three-dimensional (3D) printing technology. The contents of this article are organized as follows. First, the effect of the anisotropy caused by 3D printing filling

We aimed to investigate the relationship between nail involvement and joint manifestations and whether there was a correlation between nail psoriasis severity and bone manifestations in psoriatic patients without symptomatic psoriatic arthritis in plaque type psoriasis. Thirty-one patients with nail involvement (16 men, 15 women, mean age 45.29+/-18.73) and 39 patients without nail involvement (16 men, 23 women, mean age 38.41+/-17.33) were enrolled in the study. X-ray of the hands and feet with magnification were performed. The distal interphalangeal (DIP) joint and bone (tuft of terminal pha

OBJECTIVE: The objective of this study was to investigate the effects of a simulated peripheral median nerve lesion on precision pinch movement by the thumb and index finger.
METHODS: A median neuropathy was created by blocking the median nerve at the wrist using an anesthetic. The subjects (n=5) were asked to perform pulp-to-pulp precision pinch movements before and after the nerve block. Digit motion data was obtained with a marker-based motion analysis system.
RESULTS: The radial offset of the thumb tip, as defined by the minimum distance of the thumb tip to the flexion-extension plane of

Five cases of closed rupture of the finger extensor tendon due to osteoarthritis of the distal radioulnar joint were studied. Difficulty in extension began at the little finger and extended to the ring and long fingers. Pain and swelling in the dorsal aspect of the wrist preceded the tendon rupture. Osteoarthritic changes at the distal radio-ulnar joint were more severe than those at the radio-carpal and intercarpal joints. The distal end of the ulna showed the plus variant, as well as dorsal dislocation or subluxation. All patients underwent a tendon graft or tendon transfer, with excision of

PURPOSE: Several fixation techniques for distal interphalangeal (DIP) joint arthrodesis have been described, with good clinical results and complication rates between 10% and 20%. We propose an alternative technique and fixation method using a lateral approach and 1.3-mm plate and screws fixation.
METHODS: Between March and September 2005, 11 patients, totaling 15 fingers, had DIP joint arthrodesis by the described technique. The indications were posttraumatic arthritis in 8 fingers, degenerative or rheumatoid arthritis in 5 fingers, and isolated flexor digitorum profundus tendon lesions in 2

Rheumatoid arthritis (RA) can cause severe disability of the hand and fingers. Ankylosis of the finger joints is a known yet underreported manifestation of RA of the hand. We report the case of a patient who had RA and developed autofusion of the proximal interphalangeal (PIP) joints. At presentation, the PIP joints were fused in 15° of flexion. Silicone PIP arthroplasty was performed. Function improved with 60° of PIP joint motion and no pain.
