Five years of denosumab exposure in women with postmenopausal osteoporosis: results from the first two years of the FREEDOM extension.

The 3-year FREEDOM trial assessed the efficacy and safety of 60 mg denosumab every 6 months for the treatment of postmenopausal women with osteoporosis. Participants who completed the FREEDOM trial were eligible to enter an extension to continue the evaluation of denosumab efficacy and safety for up to 10 years. For the extension results presented here, women from the FREEDOM denosumab group had 2 more years of denosumab treatment (long-term group) and those from the FREEDOM placebo group had 2 years of denosumab exposure (cross-over group). We report results for bone turnover markers (BTMs),

Control strategies for high degree-of-freedom assistive devices using non-invasive body-machine interfaces: a survey and framework.

PURPOSE: Body-machine interfaces (BoMIs) provide individuals with movement impairments the ability to control assistive devices using signals from their body movements in a non-invasive and reliable way. However, high degree-of-freedom (DOF) assistive devices such as robotic manipulators require the user to control several device commands simultaneously and intuitively, which has resulted in limited adoption of these devices by the intended user population. In this review, we survey recent approaches for controlling high-DOF systems. MATERIALS AND METHODS: Based on a systematic literature sea

From shunt dependency to shunt freedom: Endoscopic third ventriculostomy in ventriculoperitoneal shunt-dependent communicating tetraventricular hydrocephalus: A case report.

BACKGROUND: Hydrocephalus represents pathological dilatation of the ventricular system caused by impaired cerebrospinal fluid (CSF) circulation and absorption, resulting in increased intracranial pressure. Ventriculoperitoneal shunting (VPS) remains the standard treatment for communicating hydrocephalus; however, it is associated with significant long-term complications, including infection and mechanical failure. Endoscopic third ventriculostomy (ETV), traditionally indicated for obstructive hydrocephalus, has increasingly been explored in selected cases of complex and communicating hydroceph

Brain perfusion imaging by arterial spin labeling predicts postsurgical seizure freedom in pediatric focal lesional epilepsy: A pilot study.

OBJECTIVE: This study was undertaken to determine whether integrating arterial spin labeling (ASL) perfusion imaging into presurgical planning improves postsurgical seizure outcomes in children with pharmacoresistant focal lesional epilepsy associated with focal cortical dysplasia (FCD) or low-grade epilepsy-associated tumors (LEATs). METHODS: We retrospectively analyzed magnetic resonance imaging (MRI) scans from 18 children (median age = 4.8 years, interquartile range = 1.9-11.5) who underwent resection for FCD- or LEAT-associated pharmacoresistant epilepsy, with at least 1 year of follow-u

Personal Freedom and Public Responsibility: Remaining Questions after First Case of HIV Transmission via Blood Transfusion in North Serbia.

The reduction in the risk of transfusion-transmissible infections (including HIV infection) is an essential part of transfusion patient care. Here, we report the first incident of HIV transmission via transfusion in north Serbia due to blood donor dishonesty, and the failure of the laboratory screen tests to detect the presence of HIV particles in his blood. Infected blood products were distributed to two recipients, and HIV infection was confirmed in one. This incident finally led to the implementation of Nucleic Acid Amplification Technology as mandatory testing of blood donors for HIV infec

Association analysis of KIR/HLA genotype with liver cirrhosis, hepatocellular carcinoma, and NUC freedom in chronic hepatitis B patients.

Natural killer cells are modulated through the binding of killer cell immunoglobulin-like receptors (KIRs) with human leukocyte antigen (HLA) class I ligands. This study investigated the association of KIR/HLA pairs with progression to liver cirrhosis, hepatocellular carcinoma (HCC) development, and nucleot(s)ide (NUC) treatment freedom in hepatitis B virus (HBV) infection. KIR, HLA-Bw, and HLA-C were genotyped in 280 Japanese HBV patients for clinical comparisons. No significant associations of KIR/HLA pairs were detected in terms of liver cirrhosis development. The KIR2DS3 positive rate was

Cortical and thalamic electrode implant followed by temporary continuous subthreshold stimulation yields long-term seizure freedom: A case report.

Neuromodulation strategies that target the epileptogenic network are options for treating focal drug-resistant epilepsy. These brain stimulation approaches include responsive neurostimulation and more recently, chronic subthreshold stimulation. Long-term seizure freedom with neuromodulation is uncommon. Seizure control typically requires ongoing froms of electrical stimulation. Here, we present the case of a patient implanted with three cortical electrodes targeting the inferior frontal lobe, insula, and one subcortical electrode targeting the ipsilateral anterior thalamic nucleus. This patien

Predeployment gender differences in stressors and mental health among U.S. National Guard troops poised for Operation Iraqi Freedom deployment.

Increased exposure of women soldiers to combat in current conflicts heightens interest in the question of whether risk and resilience factors differ for female and male military personnel prior to deployment. The authors examined this question in a panel of 522 National Guard soldiers (462 men and 60 women) poised for deployment to Iraq. Soldiers completed a battery of self-report measures, including the PTSD Checklist, Beck Depression Inventory-II, and scales from the Deployment Risk and Resilience Inventory. Modest differences were observed between women and men on predeployment risk factors

Well-being impact, freedom of expression, censorship and Islamophobia experienced by Muslim healthcare professionals during the current Gaza genocide.

The Israeli government has publicly expressed genocidal intent and conducted systematic attacks on civilians of Gaza and wider Palestine, following decades of illegal occupation and apartheid. Tens of thousands of Palestinian women, men and children have been killed; over 2-million people forcefully displaced and starved. The humanitarian catastrophe has been compounded by systematic destruction of healthcare facilities, schools, and places of worship. An online survey was conducted between 10th November and 5 December 2023 to assess UK healthcare professionals (HCPs) and students' experiences

Women, prisons, and freedom: experiences of former female inmates from the prison system in Rio Grande do Sul, Brazil.

This article analyzes vulnerabilities and prospects of restructuring the lives of women who experienced maternity in prison. This qualitative study was performed with a thematic analysis of the reports of women released from the prison system who experienced pregnancy and delivery in a female penitentiary in a Southern Brazilian capital. Three thematic categories were identified: "Between rupture and freedom", a category focused on the description of moments before the borderline date for the child's stay with the mother in prison, which produces great expectations due to the possibility of wo

Frontal Lobe Epilepsy Surgery in Childhood and Adolescence: Predictors of Long-Term Seizure Freedom, Overall Cognitive and Adaptive Functioning.

BACKGROUND: Although frontal lobe resections account for one-third of intralobar resections in pediatric epilepsy surgery, there is a dearth of information regarding long-term seizure freedom, overall cognitive and adaptive functioning. OBJECTIVE: To identify outcome predictors and define the appropriate timing for surgery. METHODS: We retrospectively analyzed the data of 75 consecutive patients aged 10.0 ± 4.9 yr at surgery that had an 8.1 yr mean follow-up. RESULTS: Etiology comprised focal cortical dysplasia (FCD) in 71% and benign tumors in 16% cases. All patients but one had a magnetic

Adaptive two-degree-of-freedom PI for speed control of permanent magnet synchronous motor based on fractional order GPC.

In this paper, an adaptive two-degree-of-freedom (2Dof) proportional-integral (PI) controller is proposed for the speed control of permanent magnet synchronous motor (PMSM). Firstly, an enhanced just-in-time learning technique consisting of two novel searching engines is presented to identify the model of the speed control system in a real-time manner. Secondly, a general formula is given to predict the future speed reference which is unavailable at the interval of two bus-communication cycles. Thirdly, the fractional order generalized predictive control (FOGPC) is introduced to improve the co

Freedom from graft vessel disease in heart and combined heart- and kidney-transplanted patients treated with tacrolimus-based immunosuppression.

BACKGROUND: In end-stage cardiomyopathy where concomitant chronic renal failure is a contraindication for cardiac transplantation (HTx), simultaneous heart and kidney transplantation (HKTx) may be the only feasible therapeutic option. Due to the increased donor shortage, the clinical outcome of combined HKTx patients on tacrolimus-based immunosuppression was assessed and compared with a group of HTx patients. METHODS: Three hundred forty-nine HTxs, including 13 (4%) combined HKTxs, were performed since 1995. Two hundred twenty-one HTx and all HKTx recipients received tacrolimus-based immunosu

Seizure freedom after vagus nerve stimulation in a patient with SCN3A-related network epilepsy and incomplete penetrance: A case report.

Genetic-based epilepsies often present as distributed neural network disorders, challenging traditional focal versus generalized classifications. We report a 20-year-old male with drug-resistant epilepsy associated with a previously unreported SCN3A missense variant (c.5970C > G; p.Ser1990Arg). Despite failure of multiple anti-seizure medications and a non-localizing electroclinical profile, the patient achieved complete seizure freedom following Vagus Nerve Stimulation (VNS) therapy. Segregation analysis revealed the variant in asymptomatic relatives, including a parent with subclinical inter
