Injury frequency and characteristics (location, type, cause and severity) differed significantly among athletics ('track and field') disciplines during 14 international championships (2007-2018): implications for medical service planning.

OBJECTIVE: To analyse differences between athletic disciplines in the frequency and characteristics of injuries during international athletics championships. METHODS: Study design, injury definition and data collection procedures were similar during the 14 international championships (2007-2018). National medical teams and local organising committee physicians reported all newly incurred injuries daily on a standardised injury report form. Results were presented as number of injuries and number of injuries per 1000 registered athletes, separately for male and female athletes, and for each dis

Exertional Heat Stroke: Are We Cool Enough? Retrospective Observational Study of Patients of Running Events.

BACKGROUND: Early recognition and immediate cooling are paramount in the treatment for exertional heat stroke (EHS). The most effective method is cold water immersion, however, practicalities may limit its use in the prehospital setting. An alternative is rotating ice water-soaked towels, which requires fewer personnel, can be deployed quickly, and allows easier patient monitoring. OBJECTIVE: The authors aimed to investigate the effectiveness of treatment with rotating ice water-soaked towels for patients with EHS to reduce (tympanic) temperature to < 104°F (40°C) within 30 min. METHODS: In

Impact of sodium alginate energy gel on the marathon performance of amateur runners: a randomized controlled study.

OBJECTIVE: Sodium alginate hydrogel can encapsulate high-sugar substances, alleviate gastric discomfort, accelerate gastric emptying speed, and gradually expand in the small intestine while slowly releasing carbohydrates. This study aimed to evaluate the impact of sodium alginate energy gels versus traditional energy gels on glycemic stability and running performance in marathoners. METHODS: Eighty-one amateur runners were selected and randomly assigned to four groups: elite sodium alginate energy gel (ES), elite traditional energy gel (ET), advanced sodium alginate energy gel (AS), and advan

Effect of stride length on the running biomechanics of healthy women of different statures.

BACKGROUND: Tibial stress fracture is a debilitating musculoskeletal injury that diminishes the physical performance of individuals who engage in high-volume running, including Service members during basic combat training (BCT) and recreational athletes. While several studies have shown that reducing stride length decreases musculoskeletal loads and the potential risk of tibial injury, we do not know whether stride-length reduction affects individuals of varying stature differently. METHODS: We investigated the effects of reducing the running stride length on the biomechanics of the lower ext

Acute morphological changes of impending rupture of vertebral artery dissection: clinical management of symptomatic but unruptured vertebral artery dissection. Illustrative case.

BACKGROUND: The reported actual risk of rupture for vertebral artery dissection (VAD) in patients presenting with headache is very low, ranging from 0.4% to 1.0%. The authors report a case in which the dissection site dilated rapidly within several hours after the dissection occurred resulting in subarachnoid hemorrhage (SAH). OBSERVATIONS: A 49-year-old healthy man who had participated in a marathon noticed a headache while running. Magnetic resonance imaging (MRI) performed 2 days later revealed no findings suspicious for right VAD, but a string sign was observed in the left side, suggestin

Long-term electrocardiographic follow-up from childhood of an adult patient with Brugada syndrome associated with sick sinus syndrome.

We had the unique opportunity of following the electrocardiographic (ECG) course of a 13-year-old male with sinus dysfunction and atrial flutter who subsequently developed a Brugada-type ECG pattern associated with sick sinus syndrome at 25 years old. Family history showed that the patient's mother and maternal grandfather suddenly died while sleeping at night. When the patient was 13 years old, he lost consciousness after running a marathon. The patient was diagnosed with sinus dysfunction and atrial flutter, and he underwent pacemaker implantation at 15 years old. ECG examinations performed

Pre-Participation Screenings Frequently Miss Occult Cardiovascular Conditions in Apparently Healthy Male Middle-Aged First-Time Marathon Runners.

INTRODUCTION: The optimal pre-participation screening strategy to identify athletes at risk for exercise-induced cardiovascular events is unknown. We therefore aimed to compare the American College of Sports Medicine (ACSM) and European Society of Cardiology (ESC) pre-participation screening strategies against extensive cardiovascular evaluations in identifying high-risk individuals among 35-50-year-old apparently healthy men. METHODS: We applied ACSM and ESC pre-participation screenings to 25 men participating in a study on first-time marathon running. We compared screening outcomes against

Real-Time Performance Prediction in Long-Distance Trail Running: A Practical Model Based on Terrain Difficulty and Pacing Variability.

Trail running is a demanding endurance sport where performance prediction models often rely on laboratory testing or pre-race data, limiting their practical application. This study presents a real-time predictive model for marathon and ultra-trail races, based on variables recorded during the race, including uphill/downhill pace-times, terrain difficulty coefficients, and partial rankings. A total of 947 runners from the 'Trail Valle de Tena' event (Spain) were analyzed to develop equations that estimate total race time using only the first third of the race. The model incorporates weighted ti

Femoral Neck Stress Fractures in Sport: A Current Concepts Review.

Femoral neck stress fractures (FNSFs) account for 3% of all sport-related stress fractures. The commonest causative sports are marathon and long-distance running. The main types of FNSF are compression-sided, tension-sided and displaced. The most common reported symptom is exercise-related groin pain. Radiographs form the first line of investigation, with MRI the second-line investigation. The management of FNSFs is guided by the location and displacement of the fracture. Delay in diagnosis is common and increases the likelihood of fracture displacement. Sporting outcomes are considerably wors

Evaluating sports nutrition advice provided by general-purpose large language models for recreational female marathon runners: a multidimensional descriptive audit.

BACKGROUND: General-purpose large language models (LLMs) are increasingly used for health and nutrition information. We evaluated the quality and practical limitations of LLM-generated sports nutrition advice for a standardized recreational female marathon runner, focusing on numerical recommendations, safety boundaries, and female-specific considerations. METHODS: We conducted a time-stamped cross-sectional descriptive audit of five LLM configurations using 15 consumer-style prompts covering pre-race nutrition, in-race fueling and hydration, unexpected race situations, supplements, and femal

Prognostic Relevance of Ischemic Late Gadolinium Enhancement in Apparently Healthy Endurance Athletes: A Follow-up Study Over 5 years.

BACKGROUND: In many cardiac diseases, myocardial scar tissue detected by late gadolinium enhancement (LGE) is a risk factor for cardiac arrhythmia and sudden cardiac death. Previous studies in athletes reported an increased risk for cardiac events in this group of ostensibly healthy subjects. However, the currently available longitudinal studies on this topic included fairly old marathon runners with a mean age of 57 ± 6 years or represent a case-control study in athletes with preexisting ventricular arrhythmia. The purpose of this prospective study was to analyze the prognostic relevance of L

Belt-type electrocardiograph for diagnosing palpitation occurring only during high-intensity exercise in athletes.

UNLABELLED: Belt-type ambulatory electrocardiograph (EV-201) is an arrhythmia diagnostic device that can record an electrocardiogram (ECG) for a maximum of 2 weeks. Here, we report the novel utility of EV-201 in detecting arrhythmias in two professional athletes. Treadmill exercise test and Holter ECG failed to detect arrhythmia because of insufficient exercise, electrocardiogram noise. However, by wearing EV-201 only during a marathon run, supraventricular tachycardia onset and termination were successfully detected. Throughout, both athletes were diagnosed with fast-slow atrioventricular nod

Comparative Genomics Uncovers the Genetic Diversity and Characters of Veillonella atypica and Provides Insights Into Its Potential Applications.

Veillonella atypica is a bacterium that is present in the gut and the oral cavity of mammals and plays diverse roles in different niches. A recent study demonstrated that Veillonella is highly associated with marathon running and approved that V. atypica gavage improves treadmill run time in mice, revealing that V. atypica has a high biotechnological potential in improving athlete performance. However, a comprehensive analysis of the genetic diversity, function traits, and genome editing method of V. atypica remains elusive. In the present study, we conducted a systemically comparative analysi

Investigating Biventricular Intracardiac Flow Remodeling in Marathon Athletes With 4D Flow Cardiac MRI.

BACKGROUND: Exercise-induced cardiac remodeling (EICR) may share features with early cardiomyopathy, thereby rendering differentiation challenging with conventional imaging modalities. Identification of novel imaging biomarkers would be useful. Given that intracardiac hemodynamics is closely associated with EICR, its assessment may hold clinical importance. Four-dimensional flow MRI (4D Flow MRI) enables quantification of ventricular flow components, which may provide a complementary hemodynamic perspective alongside conventional structural and functional assessment. PURPOSE: To characterize

Adult hypophosphatasia manifests in a marathon runner.

A 49-year-old woman, previously healthy, presented with recurrent fractures provoked by minimal trauma. She had sustained seven fractures over the previous 2 years. While she was an avid runner, her injuries were determined to be out of proportion to the degree of trauma. Initial evaluation, exploring the more common causes such as low bone density and abnormal vitamin D metabolism, was unremarkable. On repeat of the some of the tests, a low alkaline phosphatase (AP) was noted, which raised suspicion for hypophosphatasia (HPP), a rare cause of recurrent fractures. Subsequent workup revealed a

A Multimodal Approach for the Management of Moderate Hip Osteoarthritis in A Runner: A Case Report.

BACKGROUND AND PURPOSE: Evidence concludes that aerobic exercise, exercise therapy, and manual therapy effectively improve pain and function in individuals with hip osteoarthritis (OA). The American College of Rheumatology (ACR) reports there is no consensus on which type of exercise elicits the greatest benefit for those with OA. The American Academy of Orthopedic Surgeons (AAOS) cites gaps in the physical therapy literature regarding failure to stratify severity of hip OA in the context of exercise interventions. The purpose of this case report was to examine the benefits of a stratified, ta
