O. E. Van Alyea stated that "The sphenoid sinus is the most neglected of the nasal sinuses." It is neglected by disease because of its location, by the physician because of its subtle symptoms when diseased, and the surgeon because of its inaccessability. An accurate knowledge and understanding of the anatomy of the area help the physician diagnose and manage problems in this area. Eleven cases are used to illustrate the wide spectrum of pathologic conditions in and about the sphenoid sinus. Only two of the patients were asymptomatic. All of the others had deep-seated, retrobulbar headaches. E

As early as 1955, there were studies on the multiple bellies of the temporalis muscle (TM), which is considered to contain two or three layers. However, some historical literature proposed that the sphenomandibularis (SM), one of the bellies of TM, should be considered a newly discovered independent muscle. Many scholars have extremely inconsistent views on this statement. Here, we conducted an anatomical study to clarify the existence and morphological characteristics of the SM. We dissected 14 formaldehyde-fixed and 6 fresh-frozen human cadaver heads using three approaches, including vascula

BACKGROUND: Recent literature highlights anomalous cranial nerves in the sinonasal region, notably in the sphenoid and maxillary sinuses, linked to anatomical factors. However, data on the suspended infraorbital canal (IOC) variant is scarce in cross-sectional imaging. Anatomical variations in the sphenoid sinuses, including optic, maxillary, and vidian nerves, raise interest among specialists involved in advanced sinonasal procedures. The infraorbital nerve's (ION) course along the orbital floor and its abnormal positioning within the orbital and maxillary sinus region pose risks of iatrogeni

Peripheral nerve injury induces sprouting of sympathetic nerve fibers in dorsal root ganglia after spinal nerve injury. In the present study, we sought to determine the extent of intraganglionic noradrenergic sprouting in the trigeminal system. The inferior alveolar nerve, a major branch of the mandibular division, or the infraorbital nerve of the maxillary division was either ligated or chronically constricted in Sprague-Dawley rats and recovery permitted for either 2-3 or 6-9 weeks. In some animals both nerves were injured. Using immunohistochemistry with tyrosine hydroxylase antibodies, we

Accurate knowledge of anatomical structures is essential for safe and effective dental implant placement. Although conventional imaging modalities provide valuable diagnostic information, their inherent limitations may hinder comprehensive anatomical assessment. Cone beam computerized tomography (CBCT) has emerged as a reliable 3D imaging technique, offering high-resolution, cross-sectional views for precise evaluation. The canalis sinuosus (CS), a bony canal carrying neurovascular bundles from the infraorbital canal, is frequently overlooked due to its subtle radiographic presentation. This r

Post-traumatic direct carotid-cavernous fistulas may develop in patients with a closed head injury. The classical presentation is the Dandy's triad-chemosis, pulsatile proptosis and orbital bruit. Associated findings may include orbital pain, dilated episcleral corkscrew vessels, vision deficit and cranial nerve palsies. Cranial nerves-oculomotor (III), trochlear (IV), ophthalmic (V1), and maxillary (V2) divisions of trigeminal and the abducens (VI) lie in close association of the cavernous sinus. Abducens nerve (VI) lies close to the intracavernous internal carotid artery, within the substanc

Invasive aspergillosis is a rare disease and is often misdiagnosed. The clinical course is quite aggressive and it is a potentially fatal disease. We report a case of invasive aspergillosis involving the dura mater and optic nerves which was successfully treated with voriconazole, even though the patient had residual monocular blindness. An 86-year-old Japanese man complained of developing loss of vision in his left eye while taking oral fluconazole prescribed by an otolaryngologist for mycosis of the left maxillary sinus. He was referred to our hospital. At the first visit, he already had no

Fractures of the zygomatic-orbito-maxillary complex (ZOM) are among the most frequent in maxillo-facial surgery. The study evaluates treatment for this type of fracture in the long-term and the sequelae linked to it. Patients who had been operated between february 1998 and november 1999 to reduce and retain ZOM fractures were subjected to check-up examination at the Maxillo-Facial Surgery Operative Unit, Florence. Cases selected had been operated via trans-oral access with incision of the superior vestibular fornix and where necessary with application of means of retention (Foley balloon and/o

We report a case of a 61-year-old man with granulomatosis with polyangiitis (GPA) complicated with refractory optic neuritis and maxillary osteomyelitis. He had been treated with prednisolone (PSL) as cryptogenic organizing pneumonia in the respiratory department for 2 years. Afterward, he complained tenderness of paranasal sinuses and rapidly progressive visual loss of the left eye. Although both MPO-ANCA and PR3-ANCA were negative, he was diagnosed as GPA based on the American College of Rheumatology 1990 criteria. Ophthalmologic and oral examination revealed left optic neuritis and destruct

PURPOSE: To report the case of a 67-year-old male patient with odontogenic maxillary sinusitis who presented with unilateral parainfectious intraorbital optic neuritis without clinical signs and symptoms suggestive of orbital cellulitis. Despite the absence of clinical signs, taking a thorough history and obtaining the appropriate neuroimaging study raised the suspicion for an infectious etiology as the cause of optic neuropathy and stopped the continuation of corticosteroid treatment.
METHODS: Retrospective case report.
RESULTS: The only abnormal findings in the ophthalmic examination were

We report a case of neuropathic pain affecting the third branch of the trigeminal nerve caused by periodontitis associated with a low-level impacted mandibular third molar on the left side. The patient also experienced neuropathic pain in the second branch, which may have resulted from sensitization of the third branch. A 57-year-old woman presented with percussion and contact pain in the left maxillary second molar, along with burning pain extending from the left buccal region to the mandible. Pus discharge was noted from the periodontal pocket, and she was treated with anti-inflammatory medi

AIM: The mandibular division of the trigeminal nerve is the largest of the three major divisions of the trigeminal nerve (fifth cranial nerve). In this way, many health professionals belonged to different fields are commonly evaluating patients suffering with orofacial pain and stomatognatic dysfunction associated to this structure. But, in the most cases, it is difficult to establish a correct diagnosis due to the anatomical complexity of the head and neck surfaces, especially when the focus is the trigeminal nerve. Thus, the objective of this research was to present the anatomical variations

Perineural tumor spread of head and neck malignancies is a well known form of metastatic disease in which a lesion can migrate away from the primary site along the cranial nerves. Nerve function can be preserved even in advanced stages of the disease, making neuroradiological assessment of perineural tumor location and extension of utmost importance, as radiological or pathological examination may reveal normal or nonspecific nerve function. Computed Tomography is useful in detecting foraminal enlargement or more destructive bone patterns. Magnetic Resonance imaging is the modality of choice b

The authors show the interest of C.T. examination of pterygo-maxillary region, principally in the pre-therapeutic staging of the maxillo-facial neoplasy. The anatomical division of the fossa in four regions (pterygoïd muscle area, lateropharyngeal area, pterygo-palatine fossa and retro-maxillary-malar area) can be easily applied to C.T. study. Cuts must be taken axially, in the orbito-meatal plane, and coronally always if possible, before and after administration of contrast medium. Six, out of thirty six cases studied were demonstrative. It appears that C.T. examination must be performed imme

The infraorbital foramen (IOF) transmits the infraorbital nerve (ION) to specialized sensory cells (mechanoreceptors) in the maxillary region. The size of the IOF has been used in numerous paleoecological interpretations of the fossil record. However, these interpretations have been applied without an explicit analysis of the relationship between ecological variables and the IOF. ION and IOF cross-sectional area show a strong positive correlation. As a result, IOF area can be a proxy for ION area, and it is hypothesized that IOF area may be a good measure for maxillary somatosensory acuity. Di

Functional malocclusion that induces posterior condylar displacement may affect the remodeling processes of the temporomandibular joint structures. We tested the hypothesis that intermittent posterior condylar displacement due to functional malocclusion traumatizes condylar cartilage and joint innervated nerve fibers. Thirty-nine eight-week-old Wistar rats were used. To induce functional posterior condylar displacement, guiding appliances were attached to maxillary incisors of 24 rats for four, seven, and 14 days. Fifteen normal rats served as controls. Sections were stained with hematoxylin a

Sturge-Weber syndrome (SWS), also known as encephalotrigeminal angiomatosis, is a rare congenital neurocutaneous syndrome characterised by a triad of cutaneous, ocular and central nervous system findings. In the majority of the patients, it manifests as unilateral facial port-wine stain (PWS), which is a capillary malformation frequently affecting the first branch (ophthalmic division) of the trigeminal nerve. The ocular manifestations involve glaucoma and choroidal haemangiomas, whereas intracranial leptomeningeal venous angiomas form part of the central nervous system involvement. There are

During routine cadaveric dissection for undergraduate medical teaching, we observed dual roots of origin of the inferior alveolar nerve (IAN) on the right infratemporal fossa in an elderly male cadaver. The IAN originated by two roots encircling the second part of the maxillary artery (MA). The superior root of the ansa cervicalis (AC) originated from the ipsilateral vagus nerve and two independent muscular branches of the right vagus provided additional innervations to the anterior neck muscles. Awareness about the dual roots of origin of IAN is imperative for anaesthesiologists and dentists,
