European Chlamydia abortus livestock isolate genomes reveal unusual stability and limited diversity, reflected in geographical signatures.

BACKGROUND: Chlamydia abortus (formerly Chlamydophila abortus) is an economically important livestock pathogen, causing ovine enzootic abortion (OEA), and can also cause zoonotic infections in humans affecting pregnancy outcome. Large-scale genomic studies on other chlamydial species are giving insights into the biology of these organisms but have not yet been performed on C. abortus. Our aim was to investigate a broad collection of European isolates of C. abortus, using next generation sequencing methods, looking at diversity, geographic distribution and genome dynamics. RESULTS: Whole genom

Could there have been substantial declines in sexual risk behavior across sub-Saharan Africa in the mid-1990s?

BACKGROUND: HIV prevalence is decreasing in much of sub-Saharan Africa (SSA), but the drivers of the decline are subject to much dispute. Using mathematical modeling as a tool for hypothesis generation, we demonstrate how the hypothesis that the drop in prevalence reflects declines in sexual risk behavior is self-consistent. We characterize these potential declines in terms of their scale, duration, and timing, and theorize on how small changes in sexual behavior at the individual-level could have driven large declines in HIV prevalence. MATERIALS AND METHODS: A population-level deterministic

Adaptive immune and platelet activation during the maintenance phase of therapy in adults with B cell acute lymphoblastic leukemia.

BACKGROUND: Maintenance therapy is an integral component of treatment for B cell acute lymphoblastic leukemia (B-ALL) and has demonstrated clinical efficacy. However, features of immune activation and an increased thrombotic risk may persist beyond the induction phase of treatment. The mechanisms underlying immune activation during the maintenance phase of B-ALL remain unclear. Therefore, this study aimed to evaluate immune activation and thrombotic risk in adult patients with B-ALL undergoing maintenance therapy. METHOD: This prospective study was conducted at Windhoek Central Hospital and K

Longitudinal changes in health-related quality of life after a breast cancer diagnosis in sub-Saharan Africa: Evidence from the prospective ABC-DO cohort.

Breast cancer and its treatments impact on women's health-related quality of life, but few studies have assessed these outcomes among survivors in sub-Saharan Africa (SSA). We investigated longitudinal changes in Global Health Status (GHS) and their correlates within the African Breast Cancer-Disparities in Outcomes cohort. Newly diagnosed women (≥18 years) across five SSA countries were recruited in 2014-2017. In four countries, follow-up interviews were conducted 3-monthly over 7 years using the European Organisation for Research and Treatment of Cancer Quality of Life 30-item core questionn

Pairwise and three-country comparisons of high and low COVID-19 vaccination rates in 12 African countries: a mixed-method study protocol.

INTRODUCTION: The COVID-19 pandemic has highlighted significant disparities in vaccine uptake across sub-Saharan Africa, yet research exploring the regional and country-specific factors influencing these rates remains limited. Understanding these factors is crucial for developing targeted regional and global public health interventions to improve vaccination uptake and strengthen pandemic preparedness. This study aims to fill a critical knowledge gap by assessing the factors influencing vaccination rates across sub-Saharan Africa at the operational, systemic and socio-political levels. METHOD

Toxoplasma gondii seroprevalence among pregnant women in Africa: A systematic review and meta-analysis.

BACKGROUND: Toxoplasmosis is a serious endemic zoonotic disease caused by the protozoan parasite Toxoplasma gondii. Toxoplasma infection during pregnancy can result in congenital transmission and serious fetal and neonatal complications. This systematic review and meta-analysis aimed to assess the pooled seroprevalence of T. gondii infection and its determinants among pregnant women in African countries. METHODS: All articles reporting the seroprevalence of toxoplasmosis among pregnant women in African countries and published from 2010 to 2023 were searched using various databases. The pooled

Child vaccination in sub-Saharan Africa: Increasing coverage addresses inequalities.

BACKGROUND: Vaccines have substantially contributed to reducing morbidity and mortality among children, but inequality in coverage continues to persist. In this study, we aimed to examine inequalities in child vaccination coverage in sub-Saharan Africa. METHODS: We analysed Demographic and Health Survey data in 25 sub-Saharan African countries. We defined full vaccination coverage as a child who received one dose of bacille Calmette-Guérin vaccine (BCG), three doses of diphtheria, pertussis, and tetanus vaccine (DTP 3), three oral polio vaccine doses (OPV 3), and one dose of measles vaccine.

HIV-related discrimination among grade six students in nine Southern African countries.

BACKGROUND: HIV-related stigmatisation and discrimination by young children towards their peers have important consequences at the individual level and for our response to the epidemic, yet research on this area is limited. METHODS: We used nationally representative data to examine discrimination of HIV-positive children by grade six students (n = 39,664) across nine countries in Southern Africa: Botswana, Lesotho, Malawi, Mozambique, Namibia, South Africa, Swaziland, Zambia and Zimbabwe. Descriptive statistics are used to compare discrimination by country, gender, geographic location and soc

Ethnoveterinary Potential of Acacia (Vachellia and Senegalia) Species for Managing Livestock Health in Africa: From Traditional Uses to Therapeutic Applications.

In Africa, the folkloric practices involving plant-based remedies play a crucial role in livestock farming, often attributed to the limited access to modern veterinary services. The use of Acacia species (including those reclassified as Vachellia species) in ethnoveterinary medicine has garnered increasing interest due to their high protein content and medicinal (including anti-parasitic) properties, offering a sustainable source of fodder particularly in arid and semi-arid regions. However, scientific assessment of their efficacy and safety remains limited. This systematic review examines the

Assessment of the performance of primary healthcare in eight African countries: based on the Composite Fuzzy Index.

BACKGROUND: African countries face a dual burden of infectious and non-communicable diseases, placing substantial pressure on primary healthcare (PHC) systems. Existing indicator-weighting approaches are limited in addressing redundancy and non-linear relationships. In addition, conventional composite indices often rely on linear aggregation assumptions that may not adequately reflect complex relationships among PHC performance indicators. This study proposes a novel weighting method and composite index to improve facility-level assessment of PHC performance. METHODS: We analysed nationally r

Adolescent Attitudes, Peer Norms, and Teacher Corporal Punishment in Five Sub-Saharan African Countries.

PURPOSE: Corporal punishment by teachers remains one of the most pervasive forms of school-based violence in sub-Saharan Africa, undermining adolescent well-being and educational attainment despite statutory prohibitions. While most research has focused on prevalence, teacher behaviors, or household risk factors, little is known about how adolescents' own beliefs and peer normative environments shape their vulnerability to teacher violence. METHODS: This study analyzed nationally representative violence against children surveys from five countries: Eswatini (2021), Mozambique (2019), Kenya (2

Excess mortality associated with HIV: Survey estimates from the PHIA project.

BACKGROUND: Incomplete vital statistics systems in resource-limited countries hinder accurate HIV epidemic assessments. Population-based survey data combined with HIV infection biomarkers may partially address this gap, providing excess mortality estimates in households where people living with HIV (PLWH) reside. OBJECTIVE: Examine household-level excess HIV mortality in households with PLWH using population-based survey data, including mortality reported by heads of households, and HIV biomarkers. METHODS: We compared mortality between households with and without PLWH using publicly availab

A framework for integrating inferred movement behavior into disease risk models.

Movement behavior is an important contributor to habitat selection and its incorporation in disease risk models has been somewhat neglected. The habitat preferences of host individuals affect their probability of exposure to pathogens. If preference behavior can be incorporated in ecological niche models (ENMs) when data on pathogen distributions are available, then variation in such behavior may dramatically impact exposure risk. Here we use data from the anthrax endemic system of Etosha National Park, Namibia, to demonstrate how integrating inferred movement behavior alters the construction

Remoteness influences access to sexual partners and drives patterns of viral sexually transmitted infection prevalence among nomadic pastoralists.

Sexually transmitted infections (STIs) comprise a significant portion of the infectious-disease burden among rural people in the Global South. Particular characteristics of ruralness-low-density settlements and poor infrastructure-make healthcare provision difficult, and remoteness, typically a characteristic of ruralness, often compounds the difficultly. Remoteness may also accelerate STI transmission, particularly that of viral STIs, through formation of small, highly connected sexual networks through which pathogens can spread rapidly, especially when partner concurrency is broadly accepted

Violence against women is strongly associated with suicide attempts: evidence from the WHO multi-country study on women's health and domestic violence against women.

Suicidal behaviours are one of the most important contributors to the global burden of disease among women, but little is known about prevalence and modifiable risk factors in low and middle income countries. We use data from the WHO multi-country study on women's health and domestic violence against women to examine the prevalence of suicidal thoughts and attempts, and relationships between suicide attempts and mental health status, child sexual abuse, partner violence and other variables. Population representative cross-sectional household surveys were conducted from 2000-2003 in 13 provinci
