Feasibility of direct promontory stimulation by bone conduction: A preliminary study of frequency-response characteristics in cats.

BACKGROUND: As an alternative pathway to air conduction, bone conduction is a multipathway process that transmits sound energy to the inner ear through the skull in general. Based on this mechanism, bone conduction devices (BCDs) have been used widely in the rehabilitation of hearing loss. Although great efforts have been devoted to improving BCDs, drawbacks still exist in most categories of BCDs due to the complicated process of bone conduction. We hypothesized that if a bone conduction transducer was placed on the cochlea to stimulate it directly, the attenuation would be minimized, and the

Diagnostic criteria of barotraumatic perilymph fistula based on clinical manifestations.

CONCLUSIONS: Patients with sudden hearing loss and dizziness after barotrauma should be suspected of barotraumatic perilymph fistula (PLF). Early surgical repair of PLF showed better hearing outcomes. Therefore, diagnostic criteria should help surgical indications of barotraumatic PLF. OBJECTIVES: The aim of this study was to establish diagnostic criteria for barotraumatic PLF. METHODS: Twenty-four patients (26 ears) underwent surgery on suspicion of barotraumatic PLF. The causes of barotrauma and clinical symptoms were analyzed by surgical findings. Diagnostic criteria of PLF were proposed

Role of stapes surgery in improving hearing loss caused by otosclerosis.

The aim of our study was to evaluate the functional results of stapes surgery and to compare the effectiveness of small fenestra stapedotomy with that of total stapedectomy in improving hearing in patients affected by otosclerosis. Three hundred and fifty-seven consecutive ears, in 265 patients affected by otosclerosis, underwent surgery. All cases underwent either primary small fenestra stapedotomy (group A, 196/357, 54.91 per cent) or stapedectomy (group B, 161/357, 45.09 per cent). After surgery, 256/357 (71.71 per cent) cases showed a 0-20 dB gap. There were no significant differences in h

Assessment of the Oval Window-Facial Canal Corridor and Mastoid Pneumatization: A Preliminary HRCT-Based Radiological-Anatomical Framework.

Background/Objectives: The anatomical relationship between the oval window (OW) and the facial nerve (FN) is of significant importance in the context of stapes and middle ear surgery. However, the influence of mastoid pneumatization on OW morphology and FN proximity remains to be elucidated. Methods: This retrospective study included 220 radiologically normal temporal bones from 110 adults. OW width, OW height, OW niche width, FNt-OW distance, FNm-OW distance, OW-mastoid cell distance, and OW-FNm-FNt corridor area were measured on HRCT images. Mastoid pneumatization was classified using a sigm

Short-process incudo-stapedioplasty in congenital ear malformation.

OBJECTIVES: Although various stapedotomy and stapedectomy techniques exist, anchoring the piston can be challenging. We present a novel surgical approach for treating congenital stapes malformations with an atypical facial nerve trajectory. METHODS: This is a case of a 7-year-old boy presenting with bilateral conductive hearing loss. Prior attempts at tympanoplasty had proven unsuccessful in improving his hearing. Presurgical imaging studies revealed an unusual anatomical configuration, with the facial nerve positioned inferior to the oval window. This anatomical variation precluded the use o

Facial nerve anomalies in association with congenital hearing loss.

OBJECTIVE: Facial nerve (FN) anomalies are exceedingly rare. The association between a hearing loss and an anomalous facial nerve has never been touched deeply in the literature. The aim of this study is to report facial nerve anomalies in patients presenting with congenital hearing loss (COHL) and/or facial nerve palsy. METHODS: A retrospective chart review over 10 years from two academic tertiary referral centers. Eight patients were diagnosed with facial nerve anomalies in association with congenital hearing loss. The age ranged from 7 months to 13 years. RESULTS: Five patients had conduc

[The use of intraoperative auditory evoked brainstem responses in the treatment of congenital ossicular anomalies].

Intraoperative auditory brainstem responses (ABRs) were monitored in 3 patients undergoing middle ear surgery for congenital ossicular anomalies. It was investigated whether this technique was capable of providing useful information to the operating surgeon. After the induction of anaesthesia and intubation, standard disc electrodes were attached to the vertex (active), ipsilateral mastoid (reference), and forehead (ground). These electrodes were draped out of the surgical field. The general anaesthesia was maintained by inhalation of enflurane. Stimulus intensity was decreased from 80 dBnHL i

Clinical feasibility of a novel test setup for objective measurements using the VIBRANT SOUNDBRIDGE.

OBJECTIVES: The VIBRANT SOUNDBRIDGE is a widely used active middle ear implant to treat hearing loss. The floating mass transducer is surgically coupled to the ossicles, the round or oval window. A reliable method to monitor the coupling efficiency intraoperatively is highly desired. Research groups have developed several methods, but limitations remain. This study aims to evaluate the clinical feasibility of a new research setup for auditory brainstem response measurement to evaluate the coupling efficiency. METHOD: In 14 subjects, the new tool was used to record VSB-evoked ABR thresholds du

The Stapes in Otosclerosis: Osteoarthritis of an Ear Ossicle.

Otosclerosis is a pagetoid proliferation of bone remodeling, vascular proliferation, bone resorption and new bone formation in the tympanic region of the temporal bone. The resulting anklyosis of the stapes footplate as it articulates with the oval window is the most common cause of conductive hearing loss in young to middle aged, predominantly Caucasian individuals. The characteristic histologic features have been well documented by autopsy studies of the temporal bone. Although stapedectomy is the surgical treatment for otosclerosis, the stapes specimen may be submitted for gross examination

[Long term results stape surgery].

For many years otosclerosis has been an interesting otologic problem in regard to pathological and clinical aspects. Many various conceptions of otosclerosis development which finally lead to the stapes immobility in the oval window were discussed. Resent genetic studies indicate that genetic factors as a cause of otosclerosis possibly in combination with other factors localize 3 genes responsible for otosclerosis to chromosom 15q25q-q26, 7q34-36, 6p21-22. The aim of the study was audiometric evaluation of 1380 patients out of 1716 stapes operations performed in years 1973-2004. Audiometric as

[Pathogenesis of otosclerosis. "State of the art"].

Women suffer from otosclerosis 1.6 times more often than males. Histologically, otosclerotic foci can be found in temporal bones of females 1.9 times more often than in those of males. Characteristic topographic regions are the oval window, round window niche and promontory. Otosclerosis can also occur principally in any area of the enchondral/periosteal layer of the otic capsule. Evidence is presented that otosclerosis is an inflammatory tissue reaction associated with macrophages, T- and B-lymphocytes, HLA-DR positive cells and plasma cells. Dependent on the stage of the osteolytic bone dise

Traumatic stapes fracture with rotation and subluxation into the vestibule and pneumolabyrinth.

A 41-year-old man presented after forceful penetrating ear injury. He had incapacitating vestibular symptoms. Computed tomography revealed pneumolabyrinth with a fractured stapes that was >90 degrees rotated and subluxed into the vestibule, such that the crura and capitulum could be seen in the vestibule. Surgical repair reversed the vestibular symptoms, but there was persistent hearing loss. Stapes fractures are unusual and rarely associated with subluxation into the vestibule. When this does occur, there is usually simple footplate depression. This case demonstrates a rare stapes fracture wi
