Prosthetic Rehabilitation of a Complex Maxillofacial Defect as an Alternative to Surgical Reconstruction: A Case History Report.

Free tissue transfers are used to restore maxillofacial resected tissues during tumor ablative surgery. The maxillofacial prosthodontist remains an integral member of the therapeutic team, since conventional retained facial prostheses are in certain cases the most practical, trouble-free, cost-efficient, and successful means of rehabilitation.

Fabrication of a movable obturator following maxillary reconstruction with slit-shaped fenestration.

Extensive maxillary resection has generally been reconstructed with free skin flaps. Because drooping of the transferred flap causes instability of the obturator prosthesis, maxillary reconstruction often incorporates a slit-shaped oronasal fenestration. Although obturator prostheses for edentulous patients are stabilized with the help of oronasal slits, those for dentate patients are unstable because of flap mobility, resulting in a harmful lateral force exerted on the abutment teeth, causing dislodging of the denture. This report evaluates the benefits of a movable obturator prosthesis for a

Nasolabial and alveolar morphology following presurgical orthopaedic treatment in complete unilateral clefts of lip, alveolus and palate.

INTRODUCTION: The purpose of this study was to assess the three-dimensional (3-D) facial and alveolar morphology of patients with unilateral clefts of lip, alveolus and palate by means of a computer-aided diagnosis system. PATIENTS AND METHODS: Maxillary orthopaedic treatment was performed using soft/hard acrylic plates (Hotz's) within 2 weeks of birth. The nasolabial and alveolar morphology of 15 patients was evaluated before orthopaedic treatment (2 weeks of age) and before cheiloplasty (3 months of age). Nasolabial form was measured using a 3-D optical scanner. Twenty-one landmarks were ex

Three-dimensional printed definitive cast for a silicone obturator prosthesis: A clinical report.

For patients with head and neck cancer requiring a maxillectomy, obturator prostheses help with quality of life. These patients routinely require adjuvant oncologic treatments with significant adverse effects. Treatment sequelae can leave patients with difficulty speaking and swallowing, reduced salivary function, reduction in maximal incisal opening, and at risk of osteoradionecrosis. A 55-year-old African-American woman presented with significant trismus and reduction in maximal incisal opening after treatment for squamous cell carcinoma of the left maxillary sinus. She had received a left t

Evaluation and comparison of retention and patient satisfaction with milled polyetheretherketone versus metal maxillary obturators.

STATEMENT OF PROBLEM: Metal and polyetheretherketone (PEEK) have been used to rehabilitate maxillary defects with obturators, each having advantages and disadvantages. However, patient satisfaction with the obturator retention of each material is unclear. PURPOSE: The purpose of this crossover clinical study was to compare retention and patient satisfaction between a milled cobalt chromium and a milled PEEK framework, each with a hollow bulb extension in polymethyl methacrylate (PMMA). Conservative Management of a Large Periapical Lesion Using Decompression Technique: A Case Report.

The management of extensive periapical lesions poses a significant challenge in endodontic therapy due to their potential cystic nature, anatomical proximity to critical structures, risk of cortical perforation, and complex infection dynamics. This report presents a successful case of a large through-and-through periapical lesion managed conservatively via decompression. Initial periapical radiographs revealed poor-quality root canal therapy and a large radiolucent lesion associated with a maxillary lateral incisor. Cone-beam computed tomography (CBCT) imaging confirmed a through-and-through l

Multiple retentive means for prosthetic restoration of a large facial defect - a case report.

Rotator cuff tears (RCTs) are common in shoulder disease and disability. Despite significant advances in surgical repair techniques, 20-70% of patients still have postoperative rotator cuff dysfunction. These functional defects may be related to retear or rotator cuff quality deterioration due to tendon retraction and scar tissue at the repair site. As an effective delivery system, hydrogel scaffolds may improve the healing of RCTs and be a useful treatment for irreparable rotator cuff injuries. Although many studies have tested this hypothesis, most are limited to laboratory animal experiment

Polymeric Sustained-Release Chlorhexidine Coating on Gutta-Percha Points for Prolonged Intracanal Antimicrobial Delivery: An In Vitro Study.

Background: Persistent endodontic infections involving Enterococcus faecalis and Candida albicans are a major cause of root canal treatment failure. Although conventional irrigants, such as sodium hypochlorite and chlorhexidine (CHX), exhibit strong immediate antimicrobial activity, microbes may survive and recover from the initial antimicrobial effect, hence limiting their effectiveness, especially in complex root canal anatomies and in the apical terminus of the tooth. Antibacterial dressing techniques were not proven satisfactory due to depletion of the antibacterial component or difficulty

Changing trends in the microvascular reconstruction and oral rehabilitation following maxillary cancer.

PURPOSE: The maxillectomy defect is complex and the best means to achieve optimal reconstruction, and dental rehabilitation is a source of debate. The refinements in zygomatic implant techniques have altered the means and speed by which rehabilitation can be achieved and has also influenced the choice regarding ideal flap reconstruction. The aim of this study is to report on how the method of reconstruction and oral rehabilitation of the maxilla has changed since 1994 in our Institution, and to reflect on case mix and survival. METHODS: Consecutive head and neck oncology cases involving maxil

Foreign body of endodontic origin in the maxillary sinus.

Foreign bodies found in the maxillary sinus include tooth roots, burs, dental impression material, root-filling materials, dental implants, and needles. The purpose of this paper was to present an unusual case of a large foreign body of endodontic origin (root-filling material) removed from the maxillary sinus. A 45-year-old generally healthy male patient reported to the Department of Conservative Dentistry with Endodontics of the Medical University of Silesia, because of orbital and buccal pain on the right side of his face and headaches in the preceding 6 months. Those symptoms were associat

An Alternative Prosthetic Approach for Rehabilitation of Two Edentulous Maxillectomy Patients: Clinical Report.

Obturator prosthesis is a common treatment method for maxillectomy patients for maintaining their oronasal separation and resuming their social lives. After tumor resection, the remaining anatomical structures have a significant effect on prosthesis retention. The present study describes the rehabilitation of two maxillectomy patients after cancer surgery using a prosthesis consisting of a denture and a special retentive obturator that is positioned in the anatomical undercuts of the nasal cavity. These patients have undergone total and subtotal maxillectomy surgery after the diagnosis of squa

Long-term treatment of severe dysarthria: a case study.

This case study describes the long-term treatment and changing symptoms in a single subject with dysarthria secondary to basilar artery thrombosis. Initially, the subject was anarthric. Treatment efforts thereafter were directed toward modifying speech respiration, velopharyngeal function, articulatory precision, speech intensity, and speech intelligibility. A variety of treatment and measurement techniques are illustrated. The behavioral change resulting from each of the treatments was small. However, when combined, these small gains in conjunction with some neurological recovery resulted in
