Pneumatosis intestinalis during chemotherapy with nilotinib in a patient with chronic myeloid leukemia who tested positive for anti-topoisomerase I antibodies.

A 55-year-old man with several comorbidities including idiopathic interstitial pneumonia under long-term corticosteroid therapy, longstanding myocardial infarction, chronic heart failure, paroxysmal atrial fibrillation, gastro-esophageal reflux disease, constipation, and history of paralytic ileus, was diagnosed with chronic myelogenous leukemia (CML) in the chronic phase. He also tested positive for anti-topoisomerase I antibodies without clinical diagnosis of any connective tissue disease, including systemic sclerosis. Approximately 5 months after the initiation of nilotinib for CML, he deve

Pneumatosis intestinalis: a challenging biopsy diagnosis.

BACKGROUND: Pneumatosis intestinalis is characterized by the accumulation of gas in the gastrointestinal wall. The histopathologic diagnosis is easily made on resection specimens in which the presence of submucosal or subserosal empty spaces lined by histiocytes and giant cells presents little diagnostic dilemma. In biopsy material, though, the diagnosis is more challenging, as giant cells and histiocytes can be interpreted as granulomatous inflammation indicative of other conditions such as infection or Crohn disease. DESIGN: Nine gastrointestinal biopsies diagnosed as pneumatosis intestinal

[Disseminated pneumoperitoneum during the therapy of lymphoma with methotrexate and cytosine arabinoside].

HISTORY AND CLINICAL FINDINGS: A 58-year-old man was treated for abdominal and inguinal recurrence of a centroblastic non-Hodgkin lymphoma with high doses of methotrexate and cytosine arabinoside. A chest radiogram, taken on the 18th day of chemotherapy to exclude pulmonary infiltration, revealed pneumoperitoneum. The patients's complaints were merely of abdominal fullness and persisting diarrhoea. His general state was hardly impaired, the abdomen soft on palpation with active peristalsis. INVESTIGATIONS: There was aplastic anaemia (haemoglobin 9.9 g/dl, erythrocytes 3.4 x 10(6)/microliters,

Pneumatosis oculi and spontaneous hyphema in association with pneumatosis intestinalis.

A premature infant with acute necrotizing enterocolitis, Escherichia coli sepsis, and disseminated intravascular coagulation developed spontaneous bilateral hyphemas at 3 days of age. The necrotizing enterocolitis was associated with gas bubbles in the intestinal walls. The anterior chambers of both eyes also contained bubbles of gas, formed possibly by a mechanism similar to those in the intestine.

Gastrointestinal Disorders in Scleroderma.

Scleroderma, meaning "hard skin," refers to a heterogeneous group of disorders in which patients often experience skin thickening and fibrosis. In the systemic form of the disease, systemic sclerosis (SSc), internal organ involvement and vasculopathy are prominent features, with gastrointestinal (GI) involvement especially common and affecting the esophagus in approximately 90% of patients. Symptoms and clinical disorders that reflect GI involvement in the cardinal pathogenic features of scleroderma (ie, vasculopathy, immune-mediated inflammation, and neuropathy) emanate from every segment of

Potential Indicators of Intestinal Necrosis in Portal Venous Gas: A Case Report of an 82-Year-Old Woman on Long-Term Hemodialysis with Ascites and Pneumatosis Coli.

BACKGROUND Several factors have been reported as possible predictors of intestinal necrosis in patients with portal venous gas (PVG). We describe potential indicators of intestinal necrosis in PVG identified by contrasting 3 episodes of PVG in a patient on hemodialysis against previously verified factors. CASE REPORT An 82-year-old woman undergoing hemodialysis was admitted to our hospital thrice for acute abdominal pain. On first admission, she was alert, with a body temperature of 36.3°C, blood pressure (BP) of 125/53 mmHg, pulse rate of 60/min, respiratory rate of 18/min, and 100% oxygen sa

Computed tomography colonographic findings of pneumatosis cystoides coli.

Pneumatosis cystoides coli (PCC) is an uncommon condition in which multiple gas-filled cysts in the walls of the colon. Patients with PCC initially complain of various symptoms including abdominal discomfort, constipation, diarrhea, hematochezia, vomiting, and fever. We recently diagnosed PCC by computed tomography (CT) colonography in 2 patients. Two patients visited the outpatient clinic presenting with hematochezia, diarrhea, or abdominal pain. No underlying cause of PCC was found. Colonoscopy revealed multiple submucosal nodules covered with normal mucosa in the colon. However, CT colonogr

Mesenteric ischaemia: an overview of diagnostic and therapeutic best practice.

Mesenteric ischaemia is relatively uncommon, but is an important diagnosis to remember as it can lead to significant morbidity and mortality. Improvements in clinical imaging technology have made rapid diagnosis and treatment possible, but this relies on awareness of the condition. This article summarizes current best practice in diagnosis and management of mesenteric ischaemia to guide the clinician.

Negative Pressure Wound Therapy for a Complicated Abdominal Laparotomy in Neonatal Necrotizing Enterocolitis: A Case Report.

Necrotizing enterocolitis (NEC) is the most common surgical emergency in neonatal intensive care units, and patients who require surgery have high mortality and morbidity rates. The utility of negative pressure in the management of adults with complicated abdominal wounds has been documented, but there are few reports describing the use of negative pressure wound therapy (NPWT) in children or following neonatal surgery. The case of a 6 day old, 5-weeks premature neonate with NEC is presented. An exploratory midline laparotomy was performed on day 3 of life owing to rectal bleeding and abdomina

Gastroduodenal emphysema.

Pneumatosis intestinalis of the foregut is a very rare finding. It may represent emphysematous gastritis secondary to inflammatory conditions or gastric emphysema secondary to proximal foregut obstruction and high intragastric pressure. We present a case of gastroduodenal pneumatosis secondary to partial duodenal obstruction in an infant with Down's syndrome.

[Sonographic detection of gas in the vascular system of the liver: an image essay].

Gas within the vascular system of the liver was demonstrated sonographically in 5 patients. In contrast, plain films of the abdomen in these patients failed to demonstrate this serious finding. 3 patients presented with gas within the hepatic portal venous system, 1 patient with gas in the liver veins. In these patients, gas formation was probably caused by sepsis with gas-producing bacteria. In the last patient, air within the hepatic arterial system was caused by insufflation of air thru a port-a-cath catheter located in the main hepatic artery. The characteristic sonographic appearance of i

Bypass enteritis. A new complication of jejunoileal bypass for obesity.

Four patients who had jejunoileal bypass for morbid obesity had increased frequency of diarrhea, diffuse abdominal tenderness and distention, and fever to 104 degrees F. Roentgenographic studies disclosed multiple distended loops in the bypassed bowel with few air fluid levels. Two of these patients underwent operation for suspected peritonitis from abscess or obstruction. No abscess or mechanical obstruction was found. The bypassed bowel contained many subserosal gas-filled blebs. The remaining two patients were treated with antibiotics and showed prompt improvement. "Bypass enteritis" must b

[Treatment and results in necrotizing enterocolitis in infancy (author's transl)].

29 children aged between 1 day and 22 months were treated for enteritis with complications. 2 children were cured conservatively. In 27 cases surgery was needed; of these 13 children survived. In 19 patients bacteria were found in the stools. Operation was indicated when there was diffuse peritonitis or when perforation could be shown radiologically. In cases of intestinal perforation the following technique was used in the last few years: 1. resection of the segment involved and ileoor colostomy. 2. Anastomosis and closure of colostomy 4--6 weeks later. Among 29 children with necrotizing ente

Acute colonic pseudo-obstruction complicating chemotherapy in paediatric oncohaematological patients: clinical and imaging features.

OBJECTIVE: Although acute colonic pseudo-obstruction (ACPO) complicating chemotherapy is still a controversial entity, it is one with which radiologists should be familiar. We describe the imaging features of ACPO in children following chemotherapy for treatment of a haematological malignancy. METHODS: We retrospectively reviewed the imaging features of eight children (age 3-14 years) with chemotherapy-related ACPO, all of whom had undergone plain radiography and CT examinations. The diagnosis of ACPO was based on both clinical features and imaging findings. RESULTS: Abnormalities noted on p

[Pneumatosis coli--a rare complication of cytostatic therapy].

Recurring paroxysmal abdominal pain developed in a 19-year old woman suffering from acute lymphatic T-cell leukaemia, during induction chemotherapy with cyclophosphamide, cytarabine and mercaptopurine. Both the plain radiograph and CT of the abdomen showed pathognomonic intramural gas accumulations and free air below the right diaphragm, pointing to pneumatosis coli. There was marked pancytopenia (leucocytes 800/microliters,haemoglobin 7.6 g/dl, thrombocytes 10,000/microliters). Whereas the abdominal pain subsided rapidly under oxygen therapy and liquid nourishment, the radiological changes re

Gastrointestinal complications of cytosine-arabinoside chemotherapy: findings on plain abdominal radiographs.

We analyzed the findings on plain abdominal radiographs in 24 patients who had adverse gastrointestinal reactions after chemotherapy with cytosine-arabinoside (Ara-C) for treatment of acute leukemia or non-Hodgkin lymphoma. Ara-C was given with vincristine, VP 16-213, daunorubicin, amsacrine, adriamycin, or corticosteroids in various combinations and dosages. The abnormalities noted on plain abdominal radiographs included paralytic ileus (73%), cecal distension (38%), pneumatosis intestinalis (27%), thickened loops of small bowel (19%), and pneumoperitoneum (8%). One patient had small-bowel il
